    All fresh water and prospectively valuable minerals (asdescribed by BLMat onsite) encountered during drilling, willbe recorded by depth and adequately protected. All oil andgas shows will be tested to determined commercial potential.

    3. PRESSURECONTROLEQUIPMENT: (See attached diagram)Operator's minimum specifications for pressure controlequipment requires an 11-inch 3000 psi double gatehydraulically operated blowout preventer and an 11-inch 3000psi annular preventer. Surface casing and all preventer ramswill be pressure tested to 1000 psi for 15 minutes using rigpump and mud. NOTE: The surface casing will be pressuretested to a minimum of 1000 psi; or one psi per foot; or 70percent of the internal yield of the casing, whichever isapplicable. B0P's will be checked daily as to mechanicaloperating condition and will be tested by rig equipment aftereach string of casing is run. All ram t.ype preventers willhave hand wheels which will be operative at the time thepreventers are installed.

    B0P systems will be consistent with API RP53. Pressure testswill be conducted before drilling out from under casingstrings which have been set and cemented in place. Blowoutpreventer controls will be installed prior to drilling thesurface casing plug and will remain in use until the well iscompleted or abandoned. Preventers will be inspected andoperated at least daily to ensure good mechanical workingorder, and this inspection will be recorded on the dailydrilling report. Preventers will be pressure tested beforedrilling casing cement plugs.

    The District Office will be notified, with sufficient leadtime, in order to have a BLMrepresentative on location duringpressure

    5. MUDPROGRAM: A gel chemical water base mud will be used fromsurface casing to total depth.

    Sufficient mud materials to maintain mud properties, controllost circulation and to contain blowout will be available atthe wellsite.

    No chromate additives will be used in the mud system onFederal and Indian lands without prior BLMapproval to ensureadequate protection of fresh water aquifers.

    Whether the we13 is completed as a dry hole or as a producer,"Well Completion and Recompletion Report and Log" (Form3160-4) will be submitted not later than 30 days aftercompletion of the well or after completion of operations beingperformed, in accordance with 43 CFR 3164. Two copies of alllogs, core descriptions, core analyses, well-test

  • geologic summaries, sample description, and all other surveysor data obtained and compiled during the drilling, workover,and/or completion operations, will be filed with Form 3160-4.Samples (cuttings, fluids, and/or gases) will be submittedwhen requested by the authorized officer (AO).

    7. ABNORMALPRESSUREANDTEMPERATURE:No abnormal pressures areexpected; a BHT of 150°F. is anticipated.

    8. ANTICIPATEDSTARTINGDATE: May 15, 1985.

    DURATIONOF OPERATION: 12 days drilling.

    No location will be constructed or moved, no well will beplugged, and no drilling or workover equipment will be removedfrom a well to be placed in a suspended status without priorapproval of the AO. If operations are to be suspended, priorapproval of the A0 will be obtained and notification givenbefore resumption of operations.

    The spud date will be reported orally to the A0 within 48hours after spudding. If the spudding occurs on a weekend orholiday, the report will be submitted on the following regularwork day. The oral report will be followed up with a SundryNotice.

    In accordance with Onshore Oil and Gas Order No. 1, this wellwill be reported on Form 3160-6 "Monthly Report ofOperations", starting with the month in which operationscommence and continue each month until the well is physicallyplugged and abandoned. This report will be filed with theVernal BLMDistrict Office, 170 South 500 East, Vernal, Utah84078.

    Immediate Report: Spills, blowouts, fires, leaks, accidents,or any other unusual occurrences shall be promptly reported inaccordance with the requirements of NTL-3A or its revision.

    If a replacement rig is contemplated for completionoperations, a "Sundry Notice" (Form 3160-5) to that effectwill be filed, for prior approval of the AO, and allconditions of this approval plan are applicable during alloperations conducted with the replacement rig.

    Should the well be successfully completed for production, theA0 will be notified when the well is placed in a producingstatus. Such notification will be sent by telegram or otherwritten communication, not later than five days following thedate on which the well is placed on

    Pursuant to NTL-2B, with the approval of the DistrictEngineer, produced water may be temporarily disposed of intounlined pits for a period of up to 90 days. During the periodso authorized, an application for approval of the permanentdisposal method, along with the required water analysis andother information, will be submitted to the District Engineer.

    Pursuant to NTL-4A, lessees or operators are authorized tovent/flare gas during initial well evaluation tests, notexceeding a priod of 30 days or the production of 50 MMCFofgas, whichever occurs first. An application will be filedwith the District Engineer and approval received, for anyventing/flaring of gas beyond the initial 30 day or authorizedtest period.

    A schematic facilities diagram as required by 43 CFR 3162.7-2,3162.7-3, and 3162.7-4 shall be submitted to the appropriateDistrict Office within 30 days of installation or firstproduction, whichever occurs first. All site securityregulations as specified in 43 CFR 3162.7 shall be adhered to.All product lines entering and leaving hydrocarbon storagetanks will be effectively sealed in accordance with 43 CFR3162.7-4.

    A first production conference will be scheduled within 15 daysafter receipt of the first production notice.

    No well abandonment operations will be commencedwithout theprior approval of the A0. In the case of newly drilled dryholes or failures, and in emergency situations, oral approvalwill be obtained from the SO. A "Subsequent Report ofAbandonment" Form 3160-5, will be filed with the A0 within 30days following completion of the well for abandonment. Thisreport will indicate where plugs were placed and the currentstatus of surface restoration. Final abandonment will not beapproved until the surface reclamation work required by theapproved APD or approved abandonment notice has been completedto the satisfaction of the A0 or his representative, or theappropriate Surface Managing Agency.

    All lease and/or unit operations will be conducted in such a manner thatfull compliance is made with applicable laws, regulations (43 CFR 3100),Onshore Oil and Gas Order No. 1, and the approved plan of operations. Theoperator is fully responsible for the actions of his subcontractors. A copyof these conditions will be furnished the field representative to insurecompliance.

    A. DRILLING PROGRAM

    A paleontology survey was required by the BIA prior to granting BIAconcurrence. BIA has arranged with Wexpro for a consultant to re-

    view the work of the location construction and report any subsequentfindings regarding paleontology to them upon completion of theconstruction.

    All roads constructed by operators on the Uintah and Ouray IndianReservation will have appropriate signs. Signs will be neat and ofsound construction. They will state: (a) that the land is owned bythe Ute Indian Tribe, (b) the name of the operator, (c) that fire-

    arms are prohibited to all non-Ute Tribal members, (d) that permitsmust be obtained from the B.I.A. before cutting firewood or othertimber products and (e) only authorized personnel permitted.

    Approval to drill the above referenced gas well is hereby granted inaccordance with Section 40-6-18, Utah Code Annotated, as amended 1983; andpredicated on Rule A-3, General Rules and Regulations and Rules of Practiceand Procedure, subject to the following stipulations:

    1. Prior to commencement of drilling, receipt by the Division ofevidence providing assurance of an adequate and approved supply ofwater.

    2. Special attention is directed to compliance with the Order for CauseNo. 190-5 dated January 27, 1983, which prescribes drillingprocedures for oil shale designated areas.

    In addition, the following actions are necessary to fully comply with thisapproval:

    1. Spudding notification to the Division within 24 hours after drillingoperations commence.

    2. Submittal to the Division of completed Form OGC-8-X, Report of WaterEncountered During Drilling.

Had good returns while cementing. Preceded cementwith 1500 gallons mud flush & 50 barrels spacer. Casing was rotated while circulating,mixing and displacing. Returned mud flush and 10 barrels spacer to surface whiledisplacing. Bumped plug with 157.5 barrels and 2200 psi or 500 psi over displac mentpressure. Floats held. Plug down @8:45 a.m., 8-25-85.

    Wexpro Co. requests an extension of the TA (Temporary Abandonment ) status for this well.The reason for the request is that the well is loaded and will not produce. An attempt to swab the well in wasunsuccessful. Since then the entire field has been put on compression. Our plans are to make one more attemptto restore the well to producing status. This work will involve raising the tubing string and swabbing the wellin to see how it will perform against the lower operating pressures we now have in the field. This work has beenincluded in our 2005 workover budget and we anticipate doing the work sometime during the Aug. - Oct., 2005time period. The decision to plug, produce or recomplete the well will be made based upon the results of thiswork.

    The following information is provided in support of the enclosed Sundry Notice requesting an extension of theSI/TA status of the subject well.

    1.) A wellbore diagram is attached for reference.

    2.) A copy of the portion of the CBL showing well cemented casing across the Birds Nest aquifer. The top ofthe Birds Nest is picked at 2,078' on the CBL.

    3.) A copy of a static BHP survey conducted August 29, 2004. It shows a BHP of 2172 psi at a depth of6,162' KBM. The fluid level in the tubing is high, just above 2,000'.

    4.) A check of shut-in surface pressures made on November 19, 2004 showed the following.2-3/8" tubing - 0 psig2-3/8" x 5-1/2" annulus - 1,500 psig5-1/2" x 9-5/8" annulus - unable to record pressure, casing head is buried

    5.) An annular fluid level was not taken. However, one can be estimated based on the results of the pressuresrecorded in 3.) and 4.) above. Assuming a gas gradient of 0.036 psilft and a water gradient of 0.43 psi/ft,the fluid level in the 2-3/8" x 5-1/2" annulus on November 19, 2004 is approximately 5,000'. This ofcourse assumes The BHP has not significantly changed since August 29, 2004.

    The present wellbore configuration does not allow for a casing integrity test to be conducted without pulling tubing.The information provided here does not prove wellbore integrity, but it does suggest that the likelihood of a casingintegrity problem is small. The 5,000 foot gas column in the annulus and high fluid level in the tubing is what onewould expect to find in a loaded gas well. And the production casing is well protected, having been cemented towithin 1,100' of surface. Please contact me at the phone number shown above if you have any questions regardingthis information.
