Registered Nurse responsible for the application of the Framework of the Nursing Process to deliver high standards of client / patient nursing care for patients with a range of mental disorders, substance misuse, learning disability or co-morbidity, under the clinical supervision of the caseload holder / charge nurse / team leader, senior charge nurse or practitioner. The registered nurse has delegated responsibility for the appraisal and clinical supervision of students and clinical / healthcare support workers in provision of care and treatment within a defined clinical service, under the management and leadership of the team leader / senior charge nurse or practitioner

Additional for those with 2 years’ post registration Acting as Named Nurse (or equivalent), responsible for the application of the framework of the Nursing Process to ensure a high standard of client / patient care, delegated by the charge nurse / team leader, senior charge nurse or practitioner who provides clinical supervision to the post holder and retains overall responsibility for patient care. Co-ordinating and supervising a small team of junior nurses or practitioners, students and clinical support workers in delivering care. Where required the post holder will undertake a delegated range of duties of the charge nurse, team leader / senior charge nurse or practitioner in their absence, including taking charge of the clinical area.

Actively and meaningfully involve relevant people, including the client / patient and carers in a shared responsibility for assessment, prevention and management of risk and positive risk taking, and individual vulnerabilities within the scope, range, skills and competencies of the post.

Escorting and providing intervention to clients / patients as directed by the named nurse (or equivalent), charge nurse, senior charge nurse or practitioner without direct supervision.

Maximise therapeutic contact time to deliver a range of evidence based and recovery focussed therapeutic interventions and treatments on a one-to-one or group basis as agreed with the charge nurse, team leader / senior charge nurse or practitioner, defined by the department’s philosophy, and in accordance with the agreed care plan, and the choices of clients / patients, carers and significant others where possible.

Deliver direct patient centred care including physical, emotional and personal care in a variety of settings including hospital wards or departments, A&E, community venues, residential establishments, and home environments as appropriate to the service and client / patient group

Provide information in a range of formats regarding pharmacological treatments, and their benefits, undesired effects and contraindications to support clients / patients’ choice regarding treatment within post holder knowledge, skills and competencies.

Support clients / patients and carers, working with other health, local authority and voluntary sector colleagues / organisations to facilitate smooth transitions of care and social inclusion.

Contribute to multidisciplinary team case discussions and reviews on a regular basis

Bring to the attention of the line manager any inadequacy in the system, which may be to the detriment of the client’s welfare and safety.

. Actively contribute to the development of an atmosphere and

environment, which promotes and facilitates learning, regularly liaising with Practice Education Facilitators and Clinical and Professional Development nurses to support learning and practice development in the clinical area

Participate in all in-service and mandatory training/study days as required in response to client / patient and service need.

Undertake appropriate continuing professional development as identified within the personal development planning process and KSF, including Flying Start Development Programme for newly qualified nurses where appropriate.

Demonstrate a willingness to perform formal sign-off Mentorship role for pre-registration Nursing Students in their final year of training and for newly qualified nurses undertaking the Flying Start Development Programme.

Participate in delivering induction programme and educational and training programmes/materials, relevant to the clinical field for colleagues, other disciplines, students and staff from other agencies and organisations in agreement with the team leader / senior charge nurse or practitioner

ANNUAL LEAVE: The annual leave entitlement is 27 working days, rising to 29 working days after 5 years service and 33 days after 10 years service. In addition to this you are entitled to 8 statutory holidays per annum to be taken between the period 1 April to 31 March each year. It is the practice of NHS Lanarkshire to calculate annual leave in hours.

MEDICAL EXAMINATION:

New entrants to NHS Lanarkshire are required to pass a pre-employment medical screen. The right is reserved to require the successful candidate to undergo a medical examination and/or x-ray at any time if it is considered necessary.

IMMUNISATION REQUIREMENTS,HEPATITIS BANDHEPATITIS C STATUS

In order to protect patients, healthcare workers who carry out Exposure Prone Procedures (EPPs) as part of their role, must provide documented evidence that they do not have a blood borne virus that they may pass on to patients whilst carrying out EPP work (i.e. that they are not infectious for hepatitis B, hepatitis C and HIV). Blood test results must be from “identified validated samples” (IVS) and be documented as such.In some cases, it is possible for healthcare workers who have a blood borne virus, to carry out a range of EPP work.  This will be assessed and monitored by an Occupational Health Consultant and the healthcare workers’ treating physician.  

Healthcare workers who perform EPPs have a professional, ethical and legal duty to protect their patients and inform occupational health if they know (or suspect) that they are infected with a blood borne virus (hepatitis B, hepatitis C or HIV) now or at any time in the future. Further guidance is given by the relevant Regulatory Bodies.

DISCLOSURE OF CRIMINAL CONVICTIONS OF NHS STAFF

Dependant on the nature of the post we will need to conduct a criminal record check. This could be a basic or standard level or a PVG for regulated work.
