THE TAYSIDE PUBLIC DENTAL SERVICE
Provisional Training Programme for StR in Special Care Dentistry
The Tayside Public Dental Service (TPDS) TPDS is a Tayside-wide service currently contained in the Access Directorate of NHS Tayside. The service operates from 3 main bases, one within each CHP area, with clinical care provided in a range of different locations based in hospitals and the wider community. The main role of the Public Dental Service is to:
- promote oral health; - monitor the oral health of the population; - inspect selected populations for oral disease; - provide general dental services, including specialist services, for those who
cannot obtain this from general dental practice; complementing the current GDS and specialist services, especially in remote and rural areas
- provide emergency and urgent dental care as well as a short course of treatment and follow up care where appropriate.
Defining the client group In the recent past the service has provided care to patient groups defined as:
- anyone with a physical or learning disability who has difficulty accessing care in general practice;
- anyone with a medically compromising condition who has difficulty accessing care in the general dental service;
- anyone with a drug or alcohol abuse problem whose condition makes it difficult for them to access the general dental service;
- any child whose family cannot or will not take them to a dentist; - school children under the NDIP inspection programme; - anyone with a mental illness who cannot access care in general dental
practice - any patient in hospital - anyone in a care home who has difficulty accessing care in general practice - anyone whose fear of dentistry discourages them from seeking care - homeless people who have difficulty accessing care and - other special needs patient groups such as people with haemophilia
As access to general dental practice improves, and as directed by the Scottish Government, there is an increasing emphasis on concentrating the delivery of care to those patients who have a moderate to severe disability. Tayside is a large geographical area, covering 755,813 hectares in total and with a population of more than 400,000. The region is geographically diverse with sparsely populated rural areas, a number of small towns and two major population centres (Dundee and Perth). Dundee is an industrial city and Perth a county market town and half of the population of Tayside live in these two urban areas. NHS Tayside therefore serves diverse populations with a range of different needs. There are significant pockets of deprivation and NHST is one of only two Scottish Health Board areas with populations living in the greatest material deprivation
2
(DEPCAT 7). There are also issues with overt urban, and hidden rural, homelessness; substance abuse; mental health problems and we serve a significant prison and forensic psychiatric community. There are a number of community homes for people with learning difficulty, dementia and complex needs. This means that the StR will be exposed to a wide range of medical needs, oral and general pathologies, and a high level of dental need. Also there are major demographic changes predicted, increasing Tayside’s population by 15% over the next 15 years. The population increase is most marked in the very young and very old.
The StR Timetable The StR timetable is based on a weekly pattern. However there is considerable flexibility within this to ensure access to all aspects of training needs. This will allow the StR to be work alongside all of the Specialists in Special Care Dentistry and Consultant colleagues on an individual basis,
Induction The first month of the training programme will be dedicated to mandatory induction training at a local level as part of TPDS and at a National level as part of the NES Scotland Training programme. This will allow for full orientation within the Special Care Dentistry team and the wider PDS.
Hospitals
Ninewells Hospital, University of Dundee Ninewells Hospital is the major teaching hospital in Tayside, with a full range of clinical specialities. Public dental service has a number of general anaesthetic and anaesthetist-led sedation lists within Ninewells, to which the StR will have regular access. There is access to other clinical specialties, with opportunities for shared care, and access to in-patient beds as necessary. Tayside OMFS are based in Ninewells. There is a Maggie’s Centre within the grounds.
Perth Royal Infirmary, Perth This District General hospital provides services to the population of the west of Tayside, including a number of dental general anaesthetic and anaesthetist-led sedation lists. There is access to other clinical specialties, with opportunities for shared care, and access to in-patient beds as necessary There is a four-surgery OMFS and orthodontic unit, to which the StR will have access. There is a palliative care centre in the grounds.
Stracathro Hospital, Brechin This is an elective ambulatory care unit, with monthly general anaesthetic and sedation lists for patients with dental anxiety and mild to moderate special care needs.
3
Royal Victoria Hospital, Dundee This comprises a hospice for adults (Roxburghe House), brain injury rehabilitation unit and psycho-geriatric services (in-patient and day centre). Dental care is provided via a surgery within the hospital complex, and in the wards as necessary.
Long-stay hospitals There are a number of long-stay geriatric and psychogeriatric wards across Tayside, where dental treatment is provided on a domiciliary-type basis.
Murray Royal Hospital, Perth and Carseview Centre, Dundee In and out-patients are provided with appropriate care within the wards if necessary, or in the nearest PDS clinic.
Special Care Dental Clinics Patients with special care needs are fundamental to the PDS and are seen and treated in all localities. Patients with moderate to severe needs are seen by one of the five specialists in Special Care Dentistry. The StR will usually work alongside one of the Specialists in Special Care Dentistry. Patients with widely varying special care and additional needs will be seen, and appropriate full range of care provided for, or onwards referral where necessary.
Conscious Sedation Training Intravenous and inhalational sedation will be provided in practice by initially Iain Hay (SDO and Specialist in Special Care Dentistry), and Wendy George (SSGDP for sedation). Iain Hay has the Diploma in Dental Sedation from Guy's Hospital, University of London and a number of other experienced staff hold the Diploma in Conscious Sedation from the University of Newcastle. Mr Hay’s main area of interest is conscious sedation for adults requiring special care. Conscious sedation experience for the more medically complex patient will also be provided by the anaesthetists who are responsible for the hospital theatre lists.
General Anaesthesia Training in provision of dentistry for patients with special care needs will commence with Anna Macdonald, Gillian Elliot, Heather MacRitchie and Iain Hay (Specialists in Special Care Dentistry) and Wendy George (SSGDP in Sedation). These are day case lists, but there is provision for access to inpatient care as and when required.
Domiciliary Provision of Care This will be under the direction of the locality specialists in Special Care Dentistry and will encompass care homes, people living independently and others who find it physically impossible to access a surgery.
Prison Care NHS Tayside is responsible for one secure prison, and one open estate. The secure prison encompasses high security and local prisoners, all of whom are treated by PDS within the prison health centre (Perth Prison). Patients from HMP Castle Huntly attend the clinic in Dundee as part of their preparation for liberation. The StR will be
4
able to take advantage of both, subject to the constraints of the additional training required to work independently within the prison.
Adolescent Care (16-18 years) Experience will be provided by both the Specialist in Special Care Dentistry, Anna Macdonald, and the Specialist in Paediatric Dentistry, Morag Curnow.
Emergency Care for Patients This will be through the normal provision of care for Special Care Dentistry, but also with occasional sessions in Broxden Dental Centre or Kings Cross dental department.
Other dental specialities (Oral surgery, oral medicine, orthodontics, restorative) Dundee Dental Hospital consultants and clinical leads will enable attachments as necessary. These will include observation and participation in consultant-led clinics in community settings (eg oral surgery in Broxden).
Training and Supervision An element of the StR training will include teaching and supervision of undergraduate dental students and dental therapy students within a community setting.
Mandatory Training There are in-house sessions for all aspects of mandatory training, and the StR will be expected to attend as directed. These include BLS (adult and paediatric), ILS, working with vulnerable adults, child protection etc. In addition they will be expected to complete all mandatory Learn pro modules as defined by Tayside PDS.
Special Care External Training It has been agreed in principle with Clinical Director, Morag Curnow, that the StR will be funded to attend appropriate national conferences and training events.
Academic Training This will be accessed through the University of Glasgow. There will be locality-based tutorials to complement the National programme. The StR will be required to participate in audit and the clinical governance agenda. Anna Macdonald Senior Dental Officer Specialist in Special Care Dentistry February 2014

THE HIGHLAND PUBLIC DENTAL SERVICE
Provisional Training Programme for StRs in Special Care Dentistry
The Highland Public Dental Service (HPDS)
HPDS is a comprehensive service which lies within the structure of Highland Health Board covering all four regions (South and Mid, North, West and Argyll and Bute). The geographic and demographic spread covers both urban and remote and rural areas which presents unique challenges for service planning and delivery The service operates from multiple locations based in hospitals, health centres, primary care/healthy living centres, Special Needs schools, and has an established centre within the Centre for Health Science in Inverness which delivers under-graduate training for students of Oral Health sciences and final year dental students from Dundee and Glasgow Dental Hospitals. The main role of the Public Dental Service, is to:
- promote oral health; - monitor the oral health of the population; - inspect selected populations for oral disease; - provide general dental services, including specialist services, for those who
cannot obtain this from general dental practice; complementing the current GDS and specialist services, especially in remote and rural areas
- provide emergency and urgent dental care as well as a short course of treatment and follow up care where appropriate.
Defining the client group Whilst in the past the service has provided care to patient groups defined as :
- anyone with a physical or learning disability who has difficulty accessing care in general practice;
- anyone with a medically compromising condition who has difficulty accessing care in the general dental service;
- anyone with a drug or alcohol abuse problem whose condition makes it difficult for them to access the general dental service;
- any child whose family cannot or will not take them to a dentist; - school children under the NDIP inspection programme; - anyone with a mental illness who cannot access care in general dental
practice - any patient in hospital - anyone in a care home who has difficulty accessing care in general practice - anyone whose fear of dentistry discourages them from seeking care - homeless people who have difficulty accessing care and - other special needs patient groups such as Haemophiliacs.
There is an increasing emphasis on concentrating the delivery of care to only those patients who have a moderate to severe disability as access to general dental practice improves and as directed by the Scottish Government , with the formation of the Public Dental Service.
2
NHS Highland contains significant areas of greatest multiple deprivation and remote and rural locations in Scotland and there are major demographic changes predicted, with population increases especially in the very young and very old – populations which make most use of Public Dental Service. In particular, there is an expected 60%+ increase in over 65s in this time period
The StR Timetable The StR timetable is based on a fortnightly pattern. This is to ensure flexibility of StR training and access to all to all aspects of their training needs. This will allow the StR to be trained alongside the Specialist in Special Care Dentistry and Consultant colleagues on an individual basis. Induction The first month /( 3-4 weeks ) of the training programme will be dedicated to allow for mandatory induction training at a local level as part of HPDS and at a National level as part of the NES Scotland Training programme. This will allow for full orientation within the Special Care Dentistry team – location, staff, facilities etc. and the completion of mandatory e-learning modules and Immediate life support training in order to participate in IV sedation.
Major Acute Teaching Hospital Raigmore Hospital, Inverness The Medically compromised Dental Unit is located within the OMFS out-patient department adjacent to the orthodontic department with restorative consultant input also, and will allow the StR to work on a hospital site with access to hospital services, such as
- Maxillo facial Surgery Services are based here , with the establishment of a specialist head and neck unit, and other surgical specialties including ENT.
- Radiology – full range of radiography techniques, and Consultant reporting
where needed. - Phlebotomy services – including use of haematology and biochemistry
laboratories. - Back-up support of medical teams, in both general and emergency care. The
possibility of patient admission to wards and day surgery unit with prior arrangement with medical teams.
- Access to haematology care – such as, factor replacement , DDAVP and tranexamic acid provision.
- Specialist services include on-site renal dialysis, on-site Maggies Centre Highland has a diverse spread of socio-economic need, with areas of high level deprivation including homeless, travellers and also drug-taking communities.
3
There are a number of community homes for people with learning difficulty, dementia and complex needs. In the wider area, there is a significant rural population. This means that out StR’s will be exposed to a wide range of medical needs, oral and general pathologies, and a high level of dental need. Inverness Dental Centre, Centre for Health Science, Inverness This is a state of the art facility opened in 2008 which combines an 8-surgery service wing, with comprehensive bariatric service, conscious sedation service, difficult extraction and oral surgery service, visiting paediatric consultant service. There is also a 13-unit poly clinic teaching facility which provides training for students completing the Bachelor in Oral Health Science, and outreach training for final year dental students from both Dundee and Glasgow Dental Hospitals with phantom head lab, and a Restorative consultant service. The Centre for Health Science is the main teaching facility for nurse training, medical post-graduate teaching, BLS/ILS training, and houses a large library (overseen by The University of Stirling), several lecture theatres and numerous tutorial rooms and practical training units and has extensive VC facilities. It is located on the Raigmore campus adjacent to the specialist Diabetes centre. Ambulance transport is available for those who require it. New Craigs Hospital, Inverness This is the psychiatric in-patient facility in Highland, which has adult in-patient facilities including a secure unit for forensic psychiatry and patients, psychogeriatric units and an acute treatment unit for learning disability patients, and provides ECT. It provides many out-patient services relating to mental health including CBT, clozapine clinics, CPN services, psychiatry and psychology services, TAG therapy service, art therapy and physiotherapy. The dental department is a single surgery unit located within the treatment suite, from which the clozapine clinic, ECT service and ECG service are delivered. The dental department includes facilities to provide dentist-led IV sedation, inhalational sedation, anaesthetist-led GA and IV services, as well as general special care services for a wide range of special care patient groups. Ambulance transport is available for those who require it. There is a doctor-led crash team on site, and an emergency barrel system for challenging patients.
4
Royal Northern Infirmary, Inverness The RNI is a community hospital with in-patient wards, a day hospital and physiotherapy, occupational therapy and CPN services. The medical and nursing service for the homeless is located here. The dental department is a single surgery unit seeing widely varying special care and additional needs patients, and appropriate full range of care provided for, and ambulance transport is available for those who require this.
Conscious Sedation Training Intravenous and inhalational sedation will be provided in practice by initially Joanna McKerrow , Senior dentist (Special Care and Sedation and Specialist in Special Care Dentistry), alongwith Fiona MacCallum and Dan Sowter, who all hold the Diploma in Conscious Sedation from the University of Newcastle Conscious sedation training for the more medically complex patient will be provided by Dr Lisa Handcock Consultant Anaesthetist at Raigmore and New Craigs Hospitals in year 3.
General Anaesthesia Training in provision of dentistry for patients with special care needs will commence with Rhona Brown (Specialist in Special Care Dentistry) at New Craigs Hospital and Dan Sowter at Raigmore Hospital, Inverness. These are primarily day case lists but there is facility at Raigmore for admission if required.
Domiciliary Provision of Care This will be in tandem with both the Specialist in Special Care Dentistry and, where appropriate, with senior clinical colleagues and necessary nursing support.
Particular reference will be given to: Risk assessment Cross-infection control Medical Emergency Training and Equipment Adult with Incapacity Act Best interest case conferences
(These are not exclusive to domiciliary care – but it is pertinent that these areas are not forgotten in the domiciliary environment).
5
Prison Care This will be accessed via prior arrangement and necessary security precautions at Her Majesty’s Prison Porterfield in Inverness. StR’s will be with the Senior Dentist Dan Sowter for prison services and the Dental Nurse team at the prison .
Emergency Care for Patients This will be through the normal provision of care for Special Care Dentistry, and involvement in the Out of Hours rota covering Unscheduled Care is expected. .
Oral Surgery Dento-alveolar surgery, including apical surgery and biopsy technique, will be taught by Specialists in Oral Surgery (Helen Devennie, and the OMFS team) both at IDC and Raigmore Hospital, under LA, conscious sedation, and GA.
Oral Pathology & Oral Medicine The Special Care Team within HPDS has an excellent working relationship with the Oral Surgery Department at Raigmore Hospital.
Medical and Surgical Training It is hoped that multi-disciplinary team working will be extended to the StR’s for Haematology and Oncology teams - for StR’s to attend ward rounds, consulting clinics, Oral and Maxillofacial Surgery - for StR;s to attend ward rounds, consulting clinics and theatre lists – led by Mr Adrian Farrow, Renal and dialysis unit, Cardiology and Palliative Care team
Highland Hospice, Inverness Special Care Team within HPDS provides emergency and urgent dental to patients within the hospice The StR will have the opportunity to assess and treat patients within the hospice on an adhoc basis whilst based at the RNI clinic under the mentorship of Rhona Brown, Senior Dentist and Specialist in Special Care Dentistry .
6
Training and Supervision Occasional teaching and supervisions, with prior discussion and arrangement with the Specialist in Special Care Dentistry, for undergraduate therapists , dental therapists, dental hygienists, dental health educators and dental nurses in special Care Dentistry, in order that the trainee can enhance their own development in this respect.
Involvement with Community Learning Disability Team and Community Psychiatric Teams The Special Care team within HPDS has excellent and mutually supportive networks with both CLDT and CPN teams in the area. Early involvement with these teams in the StR training programme will be strongly encouraged, in order to facilitate the StR’s development and holistic patient care. THE LOTHIAN PUBLIC DENTAL SERVICE
Provisional Training Programme for StRs in Special Care Dentistry
The Lothian Public Dental Service (LPDS) SPCDS is a Lothian-wide service currently hosted by West Lothian CHCP. The service operates from 33 locations based in hospitals, health centres, primary care/healthy living centres, Special Needs schools, as well as two dedicated dental centres, one in central Edinburgh and one in Midlothian.
- anyone with a physical or learning disability who has difficulty accessing care in general practice;
- anyone with a medically compromising condition who has difficulty accessing care in the general dental service;
- anyone with a drug or alcohol abuse problem whose condition makes it difficult for them to access the general dental service;
NHS Lothian contains 9% of the areas of greatest multiple deprivation in Scotland and there are major demographic changes predicted, increasing Lothian’s population by 17% by 2031. The population increase is most marked in the very young and very old – populations which make most use of Public Dental Service. In particular, there is an expected 63% increase in over 65s in this time period
2
The StR Timetable The StR timetable is based on a fortnightly pattern. This is to ensure flexibility of StR training and access to all to all aspects of their training needs. This will allow the StR to be trained alongside the Specialist in Special Care Dentistry and Consultant colleagues on an individual basis, Induction The first month /( 3-4 weeks ) of the training programme will be dedicated to allow for mandatory induction training at a local level as part of LPDS and at a National level as part of the NES Scotland Training programme. This will allow for full orientation within the Special Care Dentistry team – location, staff, facilities etc. and the completion of mandatory e-learning modules and Immediate life support training in order to participate in IV sedation.
Major Acute Teaching Hospitals The Royal Infirmary of Edinburgh The Special Care Dental Unit at the Royal Infirmary allows the StR to work on a hospital site with access to hospital services, such as - Radiology – full range of radiography techniques, and Consultant reporting
where needed.
- Specialist services include East of Scotland cardiac surgery unit , Scottish Liver Transplant Unit, kidney and pancreas transplants, with on-site renal dialysis
Lothian itself is an area of mixed socio-economic need, with areas of high level deprivation including homeless, travellers and also drug-taking communities. There are a number of community homes for people with learning difficulty, dementia and complex needs. In the wider area, there is a significant rural population. This means that out StR’s will be exposed to a wide range of medical needs, oral and general pathologies, and a high level of dental need.
3
St Johns Hospital in Livingston Has a dedicated dental department , with 2 dental surgeries providing Specialist led Orthodontic and Special Care Dentistry services as well as 3 teaching bays which provides training facilities for the StR in Special Care Dentistry , LDFTs, Dental Care Professionals and junior members of staff. The Special Care Dental Unit at St John’s Hospital In Livingston , allows the StR to work on a hospital site with access to hospital services, such as - Ear, nose and throat (ENT) surgery and Maxillo facial Surgery Services are

The 3-surgery Special Care Dental Department is adjacent to one of three LPDS laboratories and provides an excellent opportunity for the trainee to develop a close working relationship with our experienced senior dental technicians fro the more complex prosthodontic cases. The Western General Hospital is the South east Scotland Regional Centre for Oncology services , Stroke rehabilitation ,department of clinical neurology , Intensive Care and Coronary Care Units , assessment and rehabilitation services for Older People, Gastroentrology and general medical and surgery units in addition to the regional infectious diseases unit.
