Theo phân tích của thử nghiệm IRIS, Pioglitzone được chỉ ra có vai trò giảm
nguy cơ phát triển đái tháo đường type 2 và các biến cố tim mạch chính ở các bệnh
nhân có tình trạng đề kháng Insulin có biến cố thiếu máu não hoặc tai biến thoáng
qua gần đây.
Theo Silvio Inzucchi, giáo sư chuyên ngành nội tiết, giám đốc trung tâm đái
tháo đường và trường đại học Y Yale, Pioglitazone giảm một nửa nguy cơ đái tháo
đường và đột quỵ đối với các bệnh lý mạch máu não có kèm theo tình trạng đề
kháng insulin.
Nguy cơ tuyệt đối mắc đái tháo đường cao nhất ở bệnh nhân suy giảm dung
nạp đường huyết lúc đói, tăng HbA1C, tăng chỉ số kháng Insulin, và hội chứng
chuyển hóa, vậy thì hiệu quả dự phòng của Pioglitazone sẽ có ý nghĩa ở nhóm bệnh
nhân có nguy cơ cao hơn
Inzucchi và cộng sự đã phân tích dữ liệu từ 3876 bệnh nhân tham gia trong thử
nghiệm IRIS (Can thiệp tình trạng kháng Insulin sau đột quỵ), một nghiên cứu thực
hiện tại 179 thành phố ở 7 quốc gia. Ứng viên được lựa chọn ngẫu nhiên vào nghiên
cứu bao gồm những bệnh nhân bị đột quỵ nhồi máu não hoặc thiếu máu não thoáng
qua trong vòng 6 tháng với tuổi trẻ nhất là 40, không có tiền sử bệnh đái tháo đường
và có đường huyết lúc đói thấp hơn hoặc bằng 126mg/dl (6.9mmol/l) (tuổi trung bình

phát ngẫu nhiên cho bệnh nhân kí cam kết tham gia chương trình liệu pháp
pioglitazone hằng ngày (tại Actos, Takeda, với 1939 người tăng liều từ 15 đến 45mg
mỗi ngày ) hoặc placebo (với 1937 người) trong thời gian 5 năm. Những người tham
gia nghiên cứu đều thử máu lúc trước khi dùng thuốc, mỗi năm sau đó và đồng ý
phỏng vấn đều đặn theo hẹn. Sau 1 năm, nhóm được dùng pioglitazone có chỉ số
kháng Insulin (HOMA- IR) giảm 24%, từ 5,4 xuống 4,1 đường huyết lúc đói giảm từ
98,2 mg/dl xuống 95,1 mg/dl. Nhóm dùng placebo cho thấy tăng chỉ số kháng Insulin
7% từ 5,3 lên 5,7 và tăng đường huyết lúc đói từ 98,2mg/dl lên 99,7mg/dl (khác biệt
có ý nghĩa thống kê với p<0,001 đối với cả 2 nhóm)
Trong vòng thời gian theo dõi trung bình là 4.8 năm, có vài bệnh nhân ở cả 2
nhóm tiến triển thành đái tháo đường (với tỉ lệ 3.8% ở nhóm dùng pioglitazone so
với 7,7% ở nhóm dùng giả dược với tỉ suất nguy cơ HR 0.48, khoảng tin cậy 95%).
Những bệnh nhân có tình trạng suy giảm đường huyết lúc đói hoặc tăng HbA1C có
hiệu quả kém, theo nhận định của những nhà nghiên cứu. Đồng thời, các nhà
nghiên cứu cũng ghi nhận chưa tìm thấy rõ ràng cơ chế cải thiện tiên lượng tim

“Pioglitazone approximately halved the risk of developing diabetes in patients with
insulin resistance and cerebrovascular disease,” Silvio Inzucchi, MD,professor of
endocrinology and director of the Yale Diabetes Center at Yale School of
Medicine, and colleagues wrote. “The absolute diabetes risk was highest in those
with [impaired fasting glucose], increased HbA1c, higher [homeostatic model
assessment of insulin resistance] (indicating more insulin resistance) and metabolic
syndrome, so the preventive effect of pioglitazone was predominately driven by
patients in these higher risk categories.”

Inzucchi and colleagues analyzed data from 3,876 patients participating in the
Insulin Resistance Intervention after Stroke (IRIS) trial, a study conducted at 179
sites in seven countries. Participants were aged at least 40 years with a qualifying
ischemic stroke or TIA within 6 months prior to randomization, no history of
diabetes and a fasting plasma glucose of 126 mg/dL or less (mean age, 64 years;
65% men; mean BMI, 30 kg/m²; mean fasting glucose, 98 mg/dL; mean HbA1c,
5.8%). Researchers randomly assigned patients to either daily pioglitazone therapy
(Actos, Takeda; n = 1,939; increasing from 15 mg daily to 45 mg daily), or placebo
(n = 1,937) for 5 years. Participants provided blood samples at baseline and yearly
and agreed to periodic interviews.
At 1 year, participants in the pioglitazone group saw mean homeostatic model
assessment of insulin resistance (HOMA-IR) decrease by 24%, from 5.4 to 4.1;
FPG decreased from 98.2 mg/dL to 95.1 mg/dL. Participants in the placebo group
saw a 7% increase in HOMA-IR, from 5.3 to 5.7, and an increase in FPG from
98.2 mg/dL to 99.7 mg/dL (P < .0001 for all).
During a median of 4.8 years of follow-up, fewer patients went on to develop type
2 diabetes in the pioglitazone group vs. placebo group (3.8% vs. 7.7%) for an HR
of 0.48 (95% CI, 0.33-0.69). Those with IFG or an elevated HbA1c drove the
effect, according to researchers.
The researchers noted that the mechanism by which pioglitazone improved CV
outcomes in IRIS remains uncertain.
“Our findings to date cannot easily elucidate the extent to which insulin
sensitization may have mediated the observed CV benefits,” the researchers wrote.
“However, it seems unlikely that glucose lowering in this range or diabetes
prevention per se can be directly credited, based on prior studies involving the
therapy of early diabetes in patients at high CV risk.” – by Regina Schaffer
