LEVEL OF THE PARKING STRUCTURE.  SEE ATTACHED MAP FOR LOCATION.      

This form is required to park REQUESTER’S vehicle at the 1 Gateway Plaza Garage in excess of 3 days and for no more than 30 days.  Vehicle will be charged the maximum daily rate each day the vehicle is parked.  Overnight  parking  is  at  the  REQUESTER’S  sole  risk.    ABM  Parking  (Parking  Operator),  Morlin  Asset Management,  LP  (Property  Management)  and  Los  Angeles  County  Metropolitan  Transportation Authority (Owner) are not responsible for fire, theft, loss or damage to the vehicle or its contents.  If the vehicle remains past 30 days, it will be towed at vehicle owner’s expense.
