Dear Friends, this past year could be described with five words: growing, learning, sharing,

leading and excellence. We are very grateful for the accomplishments Holland Home achieved in 2016, providing excellent care to those we serve and also for the contributions we have been able to make to our community.

HERE ARE A FEW HIGHLIGHTS:

GROWING: Construction is underway at Breton Homes North for the new townhomes that will expand our ability to offer independent living on that campus. We also broke ground for a second extended care building on the Breton Woods campus to meet the growing needs of our residents.

Our seven-year renovation and improvement plan for the Raybrook campus kicked off this year and will involve creating four state-of-the-art memory care neighborhoods as well as redecorating the entire campus.

LEARNING: We expanded our knowledge about memory care through The Dementia Journey, a simulation allowing participants to experience some of the effects of dementia. Afterwards, our employees participated in Positive Approach to Care (PAC) training by Teepa Snow.

SHARING: Holland Home has been active in the community, sharing knowledge and giving back by participating in or providing many events, including:

• Rethinking Dementia–Accelerating Change • Pathways to Caregiving • Family Caregiver University • Joint Interfaith Memorial • Caregiver services classes in conjunction with the ALS Association Michigan and the Susan Mast ALS Foundation • Holland Home Speakers Bureau—54 presentations • Trillium Institute—40 presentations

We also gave presentations at numerous state and national conferences.

LEADING: We have embraced cutting-edge Smart Home technology, which

makes homes safer and more comfortable for older adults.

EXCELLENCE: HomeCare of Holland Home has maintained

its five-star rating on the Medicare Home Health Compare website.

We received the Association of Fundraising Professionals West Michigan

Chapter Award for Excellence in Fundraising 2016.

Faith Hospice achieved accreditation by the Community Health Accreditation Partners.

Now we look forward to our 125th year and know that with God’s faithfulness, Holland Home will continue to grow, learn, share, lead and excel.

CAMPUS UPDATES 2016

A new resident website was launched, which

allows our independent living residents on

our Breton and Raybrook campuses to access

schedules and sign up for activities and events,

receive important announcements, access a

resident directory and place facility work orders.

Users can also search the directory for residents

with shared interests or hobbies. A resident

wellness portal is planned for 2017.

On the memory care front, Holland Home has

adopted the Positive Approach to Care (PAC) in

caring for residents with dementia. The approach

was created by Teepa Snow, national authority

on dementia, and involves recognizing that it is

the responsibility of the care partner to change

their approach and discover what can be done

differently in dealing with challenging situations

involving those with memory loss. It also

encourages the care partner to be aware of the

environment and make the necessary changes for

those dealing with dementia.

The Raybrook Project, a seven-year revitalization

plan for the campus, is underway. Plans include

converting four floors to state-of-the-art memory

care neighborhood units as well as redesigning

and redecorating of the existing buildings.

After its opening in 2015, Breton Extended Care

became home to 20 residents and the newly

expanded and renovated Breton Rehabilitation

& Living Centre saw 328 admissions. Ground

was broken for a second extended care building

anticipated to open in the fall of 2017.

CAMPUS PERFORMANCE IN 2016

INDEPENDENT LIVING

Occupancy continued to be strong in 2016. The year ended with Raybrook Estates and Homes at 98.6 percent occupancy and Breton Woods at 98.8 percent occupancy. At year’s end, only nine units remained available out of a total of 723.

The sales model and sales office triplex townhome at Breton Homes North was

completed and two additional buildings are under construction—a triplex on Osage and a single on Norway Maple.

ASSISTED LIVING

2016 Raybrook Manor Admissions: 26 external admissions and 37 internal from independent living.

2016 Fulton Manor Admissions: 33 external admissions.

HomeCare of Holland Home provides

treatment of medical conditions in the

comfort of the client’s own home, with the goals

of speeding recovery, restoring independence and

preventing rehospitalization.

HomeCare of Holland Home has maintained its

five-star rating on the Medicare Home Health

Compare website, putting it in the top 4 percent of

home health care agencies in the country.

HomeCare provided management support and

clinical staffing to two area home health agencies

to provide both mental health services and

medical/surgical nursing.

HomeCare shared with the community by

providing education on topics such as fall

prevention, caring for those with dementia, normal

aging and more.

Helpers of Holland Home supports the

independence of older adults by providing

affordable professional services ranging from

housekeeping, shopping and meal preparation to

hands-on personal care, allowing them to stay in

their home.

In 2016, Helpers expanded

the list of devices available

through Guardian Medical

Monitoring Equipment

and developed knowledge in Smart Home

technology.

Helpers of Holland Home participates actively in

the community through the Michigan Homecare

and Hospice Association and as a member of the

Caregiver Advocacy Group.

Helpers of Holland Home shared its expertise

through Holland Home’s Resident Enrichment

Academy for Learning (REAL) Program, as a

presenter at the Pathways

to Caregiving conference,

the Family Caregiver

University and the

Caregiver Network Radio.

In collaboration with the ALS Association of West

Michigan and the Susan Mast ALS Foundation,

Helpers of Holland Home instituted specialized

training classes for the caregivers of ALS clients.

Faith Hospice provides support to patients and

their families as they navigate the last chapter

of life. Hospice support gives patients the option of

spending their final days in the peace and comfort

of their own home, surrounded by those they love.

Faith Hospice achieved high-quality outcomes

in the Hospice Quality Reporting Program, with

seven of seven measures above both the state and

national averages, including five of the seven in the

99th percentile.

Faith Hospice once again collaborated with other

area hospices and the interfaith community to

provide an Interfaith Memorial Service at the

Dominican Center.

Faith Hospice’s palliative care program is

expanding and now includes a full-time nurse

practitioner who sees patients in a variety of

settings, including a weekly clinic.

At Reliance we connect those we serve with the

care they need. We coordinate and provide

supportive services to help disabled and older adults live

as meaningfully and independently as possible.

2016 Statistics

– Performed 500 OBRA assessments– Case managed 1,250 individuals in the community– Transitioned 60 individuals from nursing facilities back into the community– Supported more than 200 individuals in psychiatric outpatient clinics– Staff turnover of less than 10%– Served individuals in 12 counties

T rillium Institute educates both medical and lay

communities on issues related to the challenges of

living with a serious disease, including topics of hope,

death and dying, symptom management, advanced care

planning and grief.

Trillium Institute strives to educate the community

about palliative care while at the same time preparing

clinicians with the knowledge and skills to meet the

community’s needs.

We serve the community by giving people the

information, knowledge and skills to better address the

issues and challenges of advanced illness, death and grief.

Holland Home embraced Smart Home

technology this year with Breton Homes

North, the new construction on our Breton Woods

campus, featuring many Smart Home innovations.

Smart Home technology is allowing older adults to

stay safe and independent in their own homes with

technological tools and adaptive devices. Many

Smart Home products are now available to our

own residents as well as those in the community.

Smart Home tools range from those designed to

keep you healthy and safe, like our Telehealth

program to those offering convenience, like

automated thermostats and door locks.

DEMENTIA JOURNEY—Holland Home is a

leader in our industry—especially with regard

to dementia care. In 2016, we implemented

Dementia Journey training to help all staff

members, regardless of their role in the

organization, better understand and provide the

best care possible for dementia patents.

The Dementia Journey allows participants to

literally walk in the shoes of a person with

dementia and simulates the challenges those

suffering from dementia face every day. Starting

with a $300,000 dementia grant for skilled

nursing training, Holland Home has expanded

the program to train every staff member to ensure

quality care and understanding at every level of

the organization.

VAN’S MARKET—Van’s Market, created

in honor of Shirley Vanderson through her

family’s generosity, is a wonderful addition to

the Raybrook campus. A wide array of items

are available for sale and provide a tremendous

convenience to Raybrook residents when they are

in need of household or food items.

FAITH HOSPICE LUNCHEON—The first

Faith Hospice Fundraising Luncheon was held on

October 5, 2016, at Cascade Hills Country Club

with approximately 190 guests. Over $60,000

was raised, and we welcomed 83 new donors to

our Faith Hospice family. With help from Erhardt

Construction Company as our underwriter and

a matching gift from Dan and Eunice Pfeiffer,

we achieved our financial goal and deepened our

guests’ understanding of Faith Hospice’s impact on

the patients and families we serve. Honoring her

late husband Scott’s memory, Dianne Reed shared

her family’s story and the loving, compassionate

care they received from Faith Hospice.

EXCELLENCE IN FUNDRAISING AWARD—On November 9, 2016, Holland Home Foundation

team was honored with the Association for Fundraising Professionals (AFP) West Michigan Chapter

Award for Excellence in Fundraising. This award recognizes a nonprofit organization that has developed

creative, resourceful and inspired campaigns, initiatives, projects or techniques that have grown and

increased the organization’s fundraising infrastructure, program and results.

EVENTS —Our events connect us with our

community and provide opportunities to share

who we are with attendees.

GOLF OUTING—Great fellowship and

competition takes place at our annual Golf

Outing for a great cause. Most important, over

$90,000 was raised from many sponsors and

golfers in 2016.

SUMMER HOUSE CONCERT—Our 2016

Summer Concert on the beautiful banks of

the Grand River was enjoyable for all. More

than 280 listened to Buddy Greene, Jeff Taylor

and Trisha Walker entertain into the evening.

Sponsors and guests helped raise $21,000 for

Faith Hospice and Trillium Institute.

CHRISTMAS DINNER—Over 800 guests were

immersed in the blessed season and enjoyed the

talent of Eric Genuis and his musicians as they

touched the crowd with harmonious beauty.

More than $250,000 was raised to support our

Resident Assistance Fund. Mina Breuker kicked

off Holland Home’s 125th Anniversary in

celebration of this huge milestone in 2017. Mark

your calendar for this year’s Christmas Dinner

& 125th Anniversary Celebration on Monday,

December 11.

GROWING

Overall revenues have continued to grow for the Christian Living Services organization by nearly 5 percent from $122,238,000 in 2015 to $128,546,000 in 2016. Facility revenues remained level in 2016 compared to 2015, while overall community-based programs increased with the most significant revenue increase with Faith Hospice. Holland Home had a tremendous year with contributions and investment performance. Year-over-year giving was up substantially due to the ongoing support of our constituency and

driven by the receipt of the generous gift of our corporate office building to Holland Home by Paul Buiten and Rich Postma. In addition, investment performance provided more than $2.4 million in 2016 versus 2015.

The Holland Home Foundation Fund also continues to grow. The fund grew to over $13 million in assets in 2016. The chart below shows the Foundation Fund growth in the past six years.

HOLLAND HOME FOUNDATION TOTAL ASSETS

Over the 30 years that the Foundation Fund has been around, it has received $10,236,361 in gifts, provided $5 million in resident assistance to Holland Home and accumulated $3,099,780 in excess earnings that have been invested for future assistance.

LEARNING

We continue to learn to better serve seniors under the changing environment we work in. Much of this learning is with how to work with Medicare as it covers services today through Advantage Programs, Accountable Care Organizations, and Bundled Payment Providers. In addition, we continue to learn to utilize Medicaid dollars in different ways through Reliance CCP with the Medicaid Waiver Program and Care Resources (Program of All-Inclusive Care for the Elderly) through the capitated funds it receives.

We have also learned that collaborative efforts can help achieve higher care for seniors as is evident with Tandem365 and Care Resources. We also believe that bringing the best together from Holland Home, Resthaven and Clark with Atrio Home Care will allow us to serve more in the community where we become stronger, more resilient and better able to provide clients with life-enhancing services and staff with a secure place in which to thrive.

SHARING

We continue to share our promise of care for many who are unable to pay for the full cost of care. As the following chart shows, 2016 was similar to

past years in which 207 or 35 percent of residents served in licensed facilities received $5.51 million in benevolent care.

CHRISTIAN LIVING SERVICES 2016 REVENUES: $128,546,007

EXCELLENCE

The Holland Home audit report for year ended 2016 resulted in the highest “clean” or unqualified opinion and in full compliance with its financial covenants. This audit report is available to anyone requesting a copy. In addition, during the past year an annual

review from an independent national bond rating agency assessed the financial performance of Holland Home and upgraded the rating to an investment-grade rating. Holland Home is one of only 150 nonprofit senior living communities in the country that has any kind of rating.

2016 REVENUE

The following charts provide 2016 revenue data for all of the organizations associated with Christian Living Services and Holland Home. Holland Home facility revenue from Fulton, Raybrook and Breton Woods campuses totaled $52.4 million. Revenue from Reliance Community Care Partners,

Faith Hospice, Care Resources, HomeCare of Holland Home, Tandem365, Helpers of Holland Home and Rehab Dimensions totaled $62.0 million. The remaining $14 million was received through corporate services, contributions and investment performance.
