CKD Prognosis Consortium: Lancet, Sept 24, 2012 and JAMA, Oct 30, 2012
CKD should be regarded as elevant risk factor for mortality and
ESRD in individuals without hypertension as it is in those with
hypertension
The relative risks of mortality and ESRD by eGFR and ACR are the
same irrespective of the presence or absence of diabetes
Both low eGFR and high albuminuria were independently associated
with mortality and ESRD regardless of age across a wide range of
populations
Cardiorenal risk increases well before
patients reach dialytic stages

“…It is possible that nephrologic care decreases mortality by
better controlling traditional risk factors (anemia,
hypertension, proteinuria, activated RAS, abnormal mineral
metabolism, or secondary hyperparathyroidism)…and superior
care of acidemia, malnutrition, and electrolyte
imbalances…”

Treatment
From epidemiology to clinical practice…
Woman born on 1947, low education level
• Type 1 DM when was 27 yrs old (1974)
• Right nephrectomy due to renal TB in 1994
• Nephrology care started at 52 yrs age (1999) with eGFR 19.8
From 1999 to May 2015: 125 visits with changes of therapy in 53%
• Low Protein (0.8-0.6 g/kg/day) -Low Salt Diet (5-6 g/day)
• Fe and NaHCO3 supplementation
• Insulin, Allopurinol, Ramipril, Torsemide, Amlodipine, Carvedilol,
Sevelamer, Paricalcitol, Atorvastatin, C.E.R.A.

• BMI 31 to 27 kg/m2
• Systolic BP: 130-140 mmHg• Serum K: 3.8-5.2 mmoL/L• Hb 10.7-12.4 g/dL• HbA1c: 6.7-8.3 %• Proteinuria reduction (from 2.5 to 0.6 g/24h)

Chronic Kidney Disease
Nephrology Care to prevent ESRD and CVD
…not only pharmacological
• Diagnosis: sCreat (to estimate eGFR) + Albuminuria
• Referral: eGFR <30 or Ualb >300 (or complications)
L’elevata mortalità (10% anno), i costi elevati sociali ed
economici (50.000 euro/pz/anno) della dialisi e l’elevato
rischio CV nella fase non-dialitica impongono la
NECESSITÀ DI MINIMIZZARE LA
PROGRESSIONE DELLA CKD
…Identificare e trattare i pazienti con
GFR ridotto e/o Ualb-Uprot !!!

Fisiologia
Renale

Depurazione dalle tossine (catabolismo proteico)
Funzioni del Rene
2) RIASSORBIMENTO/SECREZIONE TUBULARE:
Processi di trasporto, attivi e passivi, di acqua ed elettroliti a livello dei tubuli renali
Recupero del 99% circa dell’ultrafiltrato con i costituenti utili all’organismo (acqua ed elettroliti)
F.G. = 120 mL/min (170 L/die) Urine 1.5 L/die

Funzioni del Rene
3) PRODUZIONE DI SOSTANZE ORMONALI:
Vitamina D, Eritropoietina, Angiotensina II, Prostaglandine, Ossido Nitrico, Endotelina-1…
Controllo di metabolismo osseo, funzione eritropoietica, pressione arteriosa sistemica, microcircolo locale…
