invitation from the Plainfield North National Honor Society (Not to be confused with other “National Honor Societies” that send invitations through the mail and request a fee to join their organization)

EXPECTATIONS of NHS Member
Maintain 4.5 or higher GPA
Commitment:
Attend ALL monthly meetings immediately after school for 1 hour, usually the
2nd Tuesday of every school month
Excused absence from school or competition only
1 hour service credit for attendance, 2 hour deduction for unexcused absence
Participate in all NHS events
Expectations continued Accumulate a total of 30 services hours* A variety of service related opportunities are
available through NHS meetings, in school and in the community.*15 hours are due at the January membership meeting; the remaining 15 hours are due at the March membership meeting. Not meeting these requirements may result in a “member not in good standing” status which could prevent the member from being recognized at graduation.*Early Graduates – 30 hours must be completed by December to be recognized at Inductions and Graduation

This information is extremely important. We are going to review each section of the packet and important information is included
Evaluation PacketHINT- in all of the areas listed below, be specific and provide clarifying
details. Provide ALL required information; missing information will beregarded as an incomplete packet and will not be accepted
CO-CURRICULAR ACTIVITIES:
• List all activities in which you have participated in during high school. Include clubs, teams, musical groups, church groups (can be school related or outside of school) etc., and any major accomplishments in each. You MUST include at least 2 examples of co-curricular activities. For example if you are a member of the marching band in 9, 10, and 11th grades, that is considered 1 co-curricular activity. Students will be required to list contact information for all coaches and sponsors AND signatures of CURRENT coaches or sponsors.
Evaluation packetIII. LEADERSHIP POSITIONS:• List all elected or appointed leadership positions held in school,
community, or work activities. Only those positions in which you were RESPONSIBLE FOR DIRECTING OR MOTIVATING OTHERS and or directly responsible for coordinating activities should be included.
• For example, class or club officer, committee chairperson, team captain or co-captain, organizer of activities in school, church, or community groups, etc. At least 2 examples of leadership positions MUST be included. Each position is an individual example of leadership; i.e. if you are promoted to a managerial position at your job during your sophomore year and maintain that position your junior year, it counts as 1 example of a leadership position. Students will be required to list contact information for all coaches and sponsors AND signatures of CURRENT coaches or sponsors
• Provide clarifying details!• DOES NOT include babysitting or group leader in one of your classes.
Evaluation PacketIV. SERVICE• List all volunteer work that you have completed for school, church, or
community organizations. For example, tutor for Media Center/MTV, Big Brothers/ Big Sisters, School Recycling Project, etc. At least 2 examples of voluntary activities MUST be included. Each position is an individual example; i.e. if you volunteered as an outside tutor to help a middle school student in math and you tutored another student in English, “tutoring” would be 1 example of a service related activity. Students will be required to list contact information for AND signatures of individuals currently sponsoring each event.
• Some activities MAY qualify in several categories – example: Volunteer on a mission trip as a group leader should be listed in both leadership and service.
• SENIORS – must have proof, including contact information and sponsors’ signatures, of 30 hours service to be considered for membership. Seniors will be recognized at induction and graduation.

Must be typed on separate pages and turned in with the packet
2 – 3 paragraphs in length
• Signatures verifying accurate information Student – your signature verifies that all the provided
information is accurate and true.
Parent – your signature verifies that you have reviewed the information provided and that it is accurate and true.
Recommendations
MUST have 1 recommendation from a current PNHS coach, adviser, or sponsor (NOT A TEACHER unless the teacher is also your coach or sponsor)
Must have 2 outside of school recommendationsUnrelated Adult (21 or older)
NOT affiliated with Plainfield North
Examples – Travelling team coach, minister or church leader, neighbor, friend’s parent, co-worker, employer.
REMEMBER• Only COMPLETE PACKETS ARE ACCEPTED!!
– Make sure you have at least 2 activities for co-curricular, service, leadership – the more the better!!
– Include all contact information and required signatures
– Complete 2 essays
– All the required recommendations
INCOMPLETE PACKETS WILL NOT BE ACCEPTED AND YOU WILL NOT BE ELIGIBLE FOR MEMBERSHIP

• Mrs. Brest and Mrs. Hidalgo will be available to help you complete your evaluation packets on the following days:
• February 12 in the media center conference room periods 4, 5, and 6
• February 21 in the media center conference room periods 4, 5, and 6
SELECTION PROCESS
Cumulative GPA of 4.5 or higher Invitation to attend Informational Meeting and receive an
Informational Packet Timely completion of Packet – February 23, 2018, room 168 or 171,
by 2:30 pm – no exceptions 2 outside recommendations – MAY NOT BE AFFILIATED WITH
PNHS IN ANYWAY. 1 recommendation from a current coach, advisor, or club sponsor.
(If the student is involved ONLY in activities outside PNHS, then a rec from that coach, advisor, or club sponsor is acceptable)
All Recommendations will remain confidential to parents and students. Due to YOU by February 20 to be included with information packet

SELECTION PROCESS Continued
Staff Evaluation – every staff member at PNHS receives a form with all potential NHS members. Staff rates these students based on their experience with the student as related to the characteristics of an NHS member –In order to maintain the integrity
of the selection process THESE EVALUATIONS ARE CONFIDENTIAL AND WILL NOT BE SHARED WITH STUDENT OR PARENT
Incidents of academic dishonesty MAY influence membership

• Faculty Selection Committee Consists of 5 teachers (Mrs. Brest and Mrs.
Hidalgo facilitate committee but are not part of the selection committee vote)
Committee reviews Student GPA, Evaluation packet, Staff evaluation, discuss candidates individually, and make final membership decisions
Decisions regarding selection are confidential
