I agree to honour the offer as shown. I understand that I will receive a draft copy of the offer before it is released to the public. I also understand that the offer will not be released until I authorise the offer to be accurate and correct.
My Rewards® is owned and operated by My Rewards International Limited ABN 47 095 009 742.
The supplier will be included on an ongoing basis on the My Rewards® International internet database and will be automatically renewed on the 1st January each year unless notified in writing by the supplier.
The supplier may be included in any My Rewards® digital or print publication as an offer listing.
Promotion of the offer will be via member handbook, website promotion, printed newsletters, e-newsletters, mobile coupons and member materials distributed by My Rewards® partners and/or clients unless otherwise notified by the supplier.
For promotion of the offer on the My Rewards® internet platform and phone platform the supplier acknowledges that to terminate the agreement, no less than 30 days written notice must be given to My Rewards®, PO Box 622, Victoria 3163.
The supplier acknowledges that it is bound to honour the offer listed on the application to all members of My Rewards® and agrees to indemnify and keep indemnified My Rewards® for all costs incurred by it for any claims that relate to a failure by a supplier to honour the terms of this agreement. Failure to honour discounts may also result in removal from the My Rewards® database and future member books.
Notification in writing must be given by the supplier to My Rewards® no later than 7 working days after any change of the proprietor, address or contact details of the supplier.
My Rewards International Ltd authorises the supplier to use the My Rewards® trade mark in promotion to members for the duration of the agreement.

These are sourced from the form you fill in or over the telephone by one of our Product Specialists. No information of a sensitive nature is collected or stored.

HOW ACCURATE IS THE INFORMATION WE HOLD?We make every effort to keep Suppliers’ details accurate and up-to-date. Information is immediately updated if errors or changes are brought to our attention. Keep us informed of changes to personal information such as your name, address or telephone number.
DO WE DISCLOSE YOUR DETAILS TO ANYONE ELSE?No personal information is taken off-site or otherwise disclosed to third parties.
HOW DO YOU GAIN ACCESS TO YOUR PERSONAL INFORMATION?It is your legal right to know exactly what personal information of yours we hold. Member information is available at all times in writing, accompanied by proof of identity.
