. . ' . MONTHLY NARRATIVE REPORT OF OPERATION AND MAJOR MAINTENANCE EXPERIENCE This report describes the operating and major maintenance experience for the month of October,1986. This narrative report was prepared by the plant staff and is submitted in accordance with Section 6.9.1.7 of the i Units 1 and 2 Technical Specifications. On October 1, 1986 Unit 1 started the month shutdown for its first refueling outage. Unit 2 started the month at 50% ; power. Unit 2 returned to full power then reduced power to 50% to locate saltwater leakage into the main condenser. i Unit 1 experienced an ESF actuation when a ! spurious high radiation level alarm actuated the containment ventilation isolation system. On October 2, 1986 Unit 2 returned to full power. Unit 2 experienced an ESF actuation when diesel generator 2-2 automatically started during an automatic bus transfer due to an electrician j inadvertently grounding a terminal in a component ' cooling water pump breaker cubicle while performing preventive maintenance. On October 3, 1986 Unit 2 reduced power to 50% to locate saltwater leakage into the main condenser. AND MAJOR MAINTENANCE EXPERIENCE

This report describes the operating and major maintenance experience forthe month of October,1986. This narrative report was prepared by theplant staff and is submitted in accordance with Section 6.9.1.7 of the

On October 1, 1986 Unit 1 started the month shutdown for its firstrefueling outage. Unit 2 started the month at 50%

; power.

Unit 2 returned to full power then reduced powerto 50% to locate saltwater leakage into the maincondenser.

Unit 1 experienced an ESF actuation when a! spurious high radiation level alarm actuated the

containment ventilation isolation system.

On October 2, 1986 Unit 2 returned to full power.

Unit 2 experienced an ESF actuation when dieselgenerator 2-2 automatically started during anautomatic bus transfer due to an electricianj

inadvertently grounding a terminal in a component' cooling water pump breaker cubicle while

performing preventive maintenance.

On October 3, 1986 Unit 2 reduced power to 50% to locate saltwaterleakage into the main condenser.

On October 4,1986 Unit 2 returned to full power.t

On October 10, 1986 Unit 2 returned to full power then reduced powerto 60% to locate saltwater leakage into the maincondenser.-

On October 11, 1986 Unit 2 returned to full power.

On October 15, 1986 Commenced reloading Unit 1 core.

On October 19, 1986 Unit I core reload completed.

On October 31, 1986 Unit 1 ended the month shutdown for refueling.Unit 2 ended the month at full power.

Unit I was continuing its first refueling outage which started August 29.The core offload to the spent fuel pool commenced on September 30 and wascompleted on October 7. The core reload commenced on October 15 and wascompleted on October 19. The only problem experienced was during the coreoffload a fuel assembly was tilted. After a minor rigging operation thetilted fuel assembly was righted.

Unit 2 operated this month with a unit availability factor of 100% and aunit capacity factor of 92.8%. Unit 2 reduced power six times this month,twice to improve steam generator chemistry and four times to investigateand repair main condenser tube leaks.

No challenges to the steam generator safety valves or pressurizer poweroperated relief valves have been made. Unit I has numerous safety relatedmaintenance activities performed in support of the refueling outage,including:

o Steam generator feedwater sparger "J" tube replacemento Steam generator eddy current tube inspectiono Steam generator sludge removalo Reactor disassembly for refueling

'o Overhaul of two containment fan cooler unit motorso Nuclear instrument inverter changeout of inverters 1-3 and 1-4o Installation of redundant overcurrent devices for containment

penetration conductorso Diesel Generator 1-1, 1-2 and 1-3 18 month inspectionso 4 kV vital bus wiring inspectiono Setting of limit switches on motor operated valveso Anchor Darling model 5500 snubber modifications

No major safety related maintenance was performed on Unit 2 this month.
