MSKCC – Manhattan Locations – Day 1

Importance Memorial Sloan-Kettering Cancer Center is the world's oldest and largest

private institution devoted to the prevention, treatment and cure of cancer. To make Memorial Sloan-Kettering's expert care accessible and convenient for our patients, we have developed a network of community-based cancer treatment centers throughout our geographical region, including Manhattan, Brooklyn, Long Island, New Jersey and Westchester.

Introduction For each job function you will need to identify the locations of the services

rendered to the patient. Furthermore, as an employee of MSKCC it is necessary to build a foundation of knowledge regarding cancer and cancer treatment at MSKCC.

For this lesson trainees will be using the MSKCC.ORG website to learn about the locations and treatment offered at each location.

1. Have the trainees open up the internet and point their browser to http://www.mskcc.org. Once at the website review the following points:

• This is the public website for MSKCC. • This site has information on kinds of cancer, cancer

treatments and stories of current and past MSKCC patients.

• This site has information on how to make an appointment and a list of all the physicians working at MSKCC.

2. Have the trainees navigate to the top right tab that says LOCATION-> Manhattan.

3. Review 3 locations focusing on what kinds of treatments take place at the location, what billing areas are located there and any other interesting/unusual facts about the location.

4. While you review, click on the links to the specific page for that site and review what you can find on the individual pages.

General info for the first 3 locations: Main Campus – Across the street from where they had orientation at Rockefeller Research Center. This is where a large majority of services both inpatient and outpatient are offered. Outpatient - Physician services (Head & Neck & Orthopedic surgery, Bone Marrow Transplant (BMT), Pre-Admission Testing – basic testing a patient has before any surgery, Chemotherapy for some cancers, Dentistry & Radiation Services for all cancers. Inpatient – all inpatient floors, OR and SDH. This is also where the MSK gift shop is located and the cafeteria. Also the recreation center with a large TV, ping pong and the patient library.

Rockefeller Outpatient Pavilion – often called 53rd Street since it is on 53rd & 3rd. It is one of our new buildings designed specifically for outpatient care. A large amount of outpatient services are offered here including dermatology, gastrointestinal oncology, genitourinary oncology, gynecology, plastic and reconstructive surgery, thoracic oncology, and cardiology and pulmonary services, chemotherapy, radiology and the International Center. Evelyn H. Lauder – Often called BAIC for short. Our newest state of the art building which offers all outpatient services needed for patients including ancillary services such as Rehab, Psychiatry, Nutrition, Social Work and Art Therapy. There is also a special boutique that specializes in fitting women with in all the extensive choices of prostheses, surgical bras, camisoles, swim wear, day wear, and other necessities. The Boutique offers a large selection of head wear and head coverings, scarves and hats.

Exercise

1. Now put the class into groups of two. If there are only two people in the class then they will do this individually.

2. Open their book to the exercise page 5 for this lesson. 3. Ask them to choose one Manhattan location not yet

reviewed from the website list that they will read about. 4. Give the group 2 minutes to pick a location and tell the

class which one they want. *They cannot pick Imaging because there isn’t additional info on it**

Give these directions: The group must report back to the class with answers to the following questions. Bonus “points” will be awarded for the bonus questions. Let them know they have 20 minutes to use the website to answer these questions. Before they begin, one person should be appointed the presenter for each group. Review the questions with the PowerPoint slide up. Where is this building located? What kinds of cancer are treated there and/or what services offered? Is there any supplemental services offered? Provide information on how a cancer at the location is diagnosed or treated. Any other interesting facts about the location The facilitator should be the time keeper. Announce when it has been 10 minutes and let them know they should start discussing what to share with the class. 15 minutes into the exercise let them know they have five minutes left to wrap up. Here are some possible answers (but not all) for the other locations: Sidney Kimmel Center for Prostate and Urologic Cancer – This building is across the street from the Main Campus. The cancers treated are Prostate, Bladder and Testicular cancer. Both surgery and medicine doctors treat here. There is a chemotherapy unit for these patients as well. Genetic Services – Offers testing for hereditary cancer risk assessment, genetic counseling, and genetic testing by specially trained genetic counselors and physicians.

Post Treatment Resource Program –Services address a range of social, practical, and personal concerns, such as advice on insurance and employment issues and discussions for caregivers, or on survivorship as a single person. Examples of programs via the website are Building a Family After Cancer Treatment: What Are Your Options?, Prostate Cancer Support Group: Life After Treatment, Dating and Disclosure

Integrative Medicine Outpatient Program - Complements mainstream medical care by addressing the physical and emotional symptoms associated with cancer and its treatment. Including acupuncture, Reiki (Japanese technique for stress reduction and relaxation that also promotes healing), several kinds of massage, nutrition counseling, and meditation. Also, group activities such as Chair Aerobics and Slow Flow Yoga

Counseling Center – Individual, group and family counseling, smoking cessation, referrals OT pain/palliative physician.

Outpatient Rehab Center – Rehabilitation provides a means to gain function in the safest, and most effective, pain-free way. For patients receiving chemotherapy or radiation for the treatment of their cancer, therapeutic exercises can improve their strength, decreases fatigue, and help them maintain physical well-being while they are undergoing treatment. They offer Occupational and physical therapy, lymphedema and rehab medicines.

Guttman Diagnostic Center – Located in Greenwich Village this facility offers screening for breast, cervical and skin cancers. They offer mammograms as well as physical examinations and breast self-exam

instruction by a skilled nurse practitioner. The John and Maxine Bendheim and Robert Bendheim Prostate Cancer Diagnostic Center was established for men who have high prostate specific antigen (PSA) levels, a family history of prostate cancer, or the finding of a pre-malignant condition on biopsy, or who are concerned about their risk for the disease and want a comprehensive evaluation.

Breast Examination Center of Harlem – Outreach facility that offers routine Breast cancer screening which includes mammograms, pelvic exams, pap smears, counseling and follow up care. It is always looking for volunteers at BECH to assist staff in many ways, such as performing clerical duties, answering telephones, or helping to implement the Center's outreach programs.

The group that gives the most detailed information for the bonus points gets a small prize from the prize bin. The facilitator should then review the areas that the groups did not present in the same way they review the first 3 locations.

Trainees will be using the MSKCC.ORG website to learn about the locations and treatment offered at each of the regional locations. Have the website up at the same time on the projector. Have the trainees open up the internet and point their browser to http://www.mskcc.org.

Have the trainees navigate to the top right tab that says LOCATION-> Brooklyn. Review the Brooklyn location focusing on what kind of treatments take place at the location, what billing areas are located there and any other interesting/unusual facts about the location. While you review, click on the link to the Brooklyn page for that site and review what you can find on the individual page. Brooklyn Infusion Center - The Brooklyn Infusion Center provides leading-edge chemotherapy services to current patients. This new model for cancer treatment allows current Memorial Sloan-Kettering patients who live in or near the borough to receive their chemotherapy at the Brooklyn site and the remainder of their cancer care at locations in Manhattan. The facility has the capacity to accommodate 30 patients and their visitors each day, with 12 treatment rooms enclosing an indoor garden and adjoining library.

Here are possible answers: Basking Ridge , NJ - Patients have access to cancer screening, diagnostic imaging, medical oncology, radiation oncology, radiology, and dermatology services in one location. Neurological consults are also available. Additionally, the center provides educational and support programs, as well as integrative medicine resources. Commack, Long Island - Patients can receive cancer diagnosis, chemotherapy, radiotherapy, and surgical consultations. Certain clinical trials are also available to patients at Commack. Services include radiation oncology, (intensity-modulated radiation therapy, image-guided radiation therapy, neurology, breast surgical consultations, comprehensive, diagnostic radiology, genetic counseling and lymphedema therapy. Members of the Commack medical team meet weekly to review and discuss each new patient's case and treatment plan. The team is joined by other specialists who focus on the nonmedical needs of both patients and their caregivers, such as social support.

Rockville, Long Island - Memorial Sloan-Kettering Cancer Center Rockville Centre has offered medical oncology and radiation oncology services since 1997. The center houses two state-of-the-art linear accelerators that use high-energy radiation to treat patients. We also use CT-based treatment planning to help map and deliver intensity modulated radiation treatments. These techniques hone the dose of radiation around the target while avoiding healthy tissue, allowing higher doses to be delivered safely.

Hauppauge, Long Island - We offer a variety of screening methods to detect skin cancer. We use several imaging technologies to assist in the detection of skin cancer. These include total body photography, individual lesion photography (photographing a specific area of abnormal skin), and dermoscopy (the use of a hand-held instrument that enables doctors to see structures below the superficial layer of the skin). Confocal microscopy, which uses imaging to diagnose skin cancers at the cellular level, is also available, however this tool remains investigational.

Sleepy Hollow, NY - Physicians at Sleepy Hollow provide medical oncology and radiation oncology services to patients on an outpatient basis. The location is especially convenient for area residents who need daily radiation or frequent chemotherapy. Patients who need surgery or certain specialized diagnostic tests can go to Memorial Sloan-Kettering's main hospital in Manhattan for these procedures and then have their follow-up care and treatment in Sleepy Hollow.

As we discussed MSKCC has many locations in the metro area. Here at PBD we are all involved in the billing and collection of all the services at these locations. We are going to talk about how they are all organized. Ask: At your Center-wide New Employee Orientation (NEO) tour, many of you toured the clinical areas. What places did you go to? If possible relate the places they went to the chart. If they say “Pathology” affirm that Pathology is both a billing area and a division. Or Lymphoma – is a billing area. For the first example on the poster use Surgery, Head and Neck, Dr. Shaha and Dr. Singh. MSKCC is composed of several Divisions. These divisions are how we compartmentalize all of the different specialties. So, each one of these divisions cover a certain specialty. This is at the top of the organization. Put up the Surgery card on the poster. So one of the biggest divisions we have is Surgery. The division of surgery includes all the groups of physicians who specialize in surgery. But there are many different surgery groups at MSK. For example Head & Neck, which is surgery for everything from your voice box to your nose, Since it is a part of surgery it goes under the surgery division. We call the specialties within each division Billing Areas. Put up Head and Neck card. Head and Neck is a billing area in the surgery division, so only Head & Neck charges are in the Head and Neck billing area. Then we can drill down one more level. Each Billing Area is made up of a number of providers or physicians. Most providers belong to only one billing area, but some belong to more than one. In Head and Neck there are eight physicians or providers. Two of them are (put up cards) Dr. Shaha and Dr. Singh.

As you can see there are abbreviations under each one of the cards I just put up. Each one of this is how they are represented in the billing systems we use here. Usually they are just shorter versions of the word they stand for. This is how we will start seeing them in the system. Hold up two more division cards, Medicine and Radiology. Let’s look at another example. Medicine is another large division at Sloan Kettering. Put up the Medicine card on the poster. Medicine covers all medical oncology providers. That means they treat patients that need chemotherapy or radiation. It also covers our general medicine providers that support the other services such as Cardiology and Pulmonary. One of the Medicine Divisions is Lymphoma. Put up Lymphoma card. Lymphoma is when immune system cells become cancerous. Two of the physicians at Sloan that treat this are Dr. Portlock and Dr. Hamlin. Put up those cards. Any questions? Okay, one more. Radiology. Does anyone know what kind of tests we do at MSKCC under the Radiology division? Answers are X-rays, bone density scans, CT Scan, MRI, Point out that when cancer metastasizes it is often first found in the lungs, so chest xrays are very common. Mention that every cancer has a different standard radiology testing to find the extent of the cancer. Also, Nuclear Medicine studies such as a PET SCAN, bone scan are also under the radiology division. Put up Radiology card. Radiology is both a division and a billing area.
