MoKan Dial Telephone Company’sLifeline Service

Lifeline service is a government assistance program that provides discounts on monthly telephone service for eligible low-income consumers. Lifeline is supported by the Federal Universal Service Fund (USF). Only one Lifeline benefi t is available per household. A household is defi ned, for purposes of the Lifeline program, as any individual or group of individuals who live together at the same address and share income and expenses. A household is not permitted to receive lifeline benefi ts from multiple providers. Violation of the one-per-household limitation constitutes a violation of ‘the Lifeline rules and will result in the subscriber’s de-enrollment from the program. Lifeline is a non-transferable benefi t and the subscriber may not transfer his or her benefi t to any other person. Willfully making false statements to obtain the benefi t can result in fi nes, imprisonment, de-enrollment or being barred from the program.

Who qualifi es for reduced Telephone Charges? To participate in the program, consumers must either have an income that is at or below 150% of the Federal PovertyGuidelines or participate in one of the following assistance programs: Medicaid Supplemental Nutrition Assistance Program (SNAP) (formerly listed as Food Stamps) Supplemental Security Income (SSI) Federal Public Housing Assistance (Section 8) Low-Income Heat and Energy Assistance Program (LIHEAP) National School Lunch Program (free lunch program only) Temporary Assistance for Needy Families (TANF) General Assistance Food Distribution Program Additional eligibility criteria may apply to residents of federally recognized tribal lands.

In most cases, consumers will have to provide proof of eligibility upon application for Lifeline service. In addition, consumers must complete a Lifeline application and make certain certifi cations upon signing up for Lifeline and agree to re-certify their continued eligibility on an annual basis.

Healthy Aging brought to you by

Vintage Park Assisted Living Communities

Are Swollen Feet a Warning Sign? What Is Fibromyalgia?

Fibromyalgia is a condition that can be diffi cult to diagnose because it looks different in every patient. There are some com-mon symptoms that defi ne the condition, such as widespread musculoskeletal pain and fatigue, but there are also some other signs to look for that could be the result of fi bromyalgia or common co-occurring conditions. It is believed that fi bromyalgia causes patients to feel chronic pain because their brains have a higher sensitivity to pain sig-nals. Dull aching may be present most of the time, and tender points such as the back of the head, area between the shoul-ders, hips, and inner knees will feel severe pain when pressure is applied. The nature of your pain will be critical in a diagnosis of fi bromyalgia, because it is defi ned by widespread pain that has persisted for at least three months and 11 tender points out of a possible 18 in the body. Keep a diary of your pain to discuss in detail with your doctor. Other symptoms to watch for include swollen feet, tingling and numbness, and redness of the skin. Symptoms of this nature may arise due to lack of sleep or physical stress from fi bromyalgia, which may affect different patients in distinctive ways. If you are living with fi bromyalgia, you can fi nd the care you need to maintain a higher quality of life at Vintage Park Assisted Living Communities.

If life at an assisted living residence sounds like something you might be interested in, come and visit Vintage Park Assisted Living Communities. The staff at each of our locations is dedi-cated to providing residents with an exciting and rewarding environment to call home. Call one of our three
