In multiple instances, our 2016 performance was significantly better than the annual goal that we set for ourselves. The latest Online With Bryan also looks ahead to 2017. As mentioned in the video, we’re following the same metrics in 2017 that we did in 2016, with the numeric goals designed to be both aspirational and realistic, based on the trends of recent years. Here is a rundown of the network’s 2017 goals: EMPLOYEE ENGAGEMENT • • • We will achieve an employee engagement score of 78% or greater. PHYSICIAN ENGAGEMENT • • • Our physician engagement score will be 43.5% or greater. INPATIENT LOYALTY • • • Our goal is that at least 80.4% of patients surveyed will answer “yes, definitely would recommend facility.” PROVIDER RATING • • • At least 85.2% of patients surveyed will rate their provider either 9 or 10. REDUCE HARM • • • The network will achieve a cumulative harm score of 1.308 or less. INDIVIDUAL PATIENT LIVES • • • Community will achieve 539,000 or more patient lives served during 2017. OPERATING MARGIN • • • The network will achieve an operating margin of 5.5% or higher. SERVE360° • • • Leaders will achieve 100% participation in at least one Serve360° event during 2017.

Celebrating 2016, Anticipating 2017

It was an amazing 2016 here at Community! Of the eight major goals that we follow, seven were well into green-light territory, thanks to the hard work and dedication of our approximately 15,000 team members. In multiple instances, our 2016 performance was significantly better than the annual goal that we set for ourselves. Bryan Mills, president and CEO, reviews 2016 in his latest video message, available at the links below, and there are also links below to view a separate 2016 video recap.

The latest Online With Bryan also looks ahead to 2017. As mentioned in the video, we’re following the same metrics in 2017 that we did in 2016, with the numeric goals designed to be both aspirational and realistic, based on the trends of recent years.

Here is a rundown of the network’s 2017 goals:

ANOTHER GREAT UNITED WAY PERFORMANCE • • • Congratulations and thank you to the many Community team members who pledged donations in the

latest United Way campaigns in Central Indiana, Anderson and Kokomo! Altogether, our team contributed $546,201 to support the great work of United Way. That includes $413,475 for United Way of Central Indiana, $81,094 for United Way of Madison County, and $51,632 for United Way of Howard County. The Central Indiana campaign attracted 221 brand-new donors, who collectively pledged $51,848. A LITTLE R-E-S-P-E-C-T FOR OUR GIVING GIG TEAM • • • On February 11, more than 1,000 donors gathered at the Indianapolis Downtown Marriott for The Giving Gig, Community Health Network Foundation’s annual event which raised $1.6 million to help cancer patients in financial need access healthy food, lifesaving medicine and gas to get to and from treatment. To round out a record-breaking evening of giving, the Queen of Soul Aretha Franklin joined us and performed some of her classic hits for the sold-out crowd. To keep the giving going, the foundation partnered with the Indy Fuel for The Giving Game, which took place on February 24. The team wore special jerseys designed by the relative of a Community cancer patient, and proceeds from ticket sales and a post-game jersey auction will benefit Community’s oncology patients in need. A huge thank you to the dozens of Community employees who volunteered at The Giving Gig and the many families who turned out in support of the Indy Fuel and Community’s brave cancer fighters! COMMUNITY REHABILITATION HOSPITAL IN THE TOP 10 PERCENT • • • Every year, information on the majority of U.S. inpatient rehabilitation hospitals is collected and facilities are scored based on several key indicators. Community Rehabilitation Hospital recently learned that it scored among the elite top 10 percent of freestanding inpatient rehabilitation hospitals for 2016. The tally compared Community Rehabilitation Hospital with nearly 800 other facilities across the country, and the top 10 percent ranking is a strong indication of excellent patient outcomes and exceptional care. Congratulations!

COMMUNITY ANDERSON “BEST PLACE TO WORK” • • • Community Anderson was voted “Best Place to Work in Madison County” in the latest Best of Madison County survey conducted by the Herald Bulletin. Dr. Traci Anderson was voted best doctor, while Holly Renz was voted best nurse and best civil servant. “Really, I think for many of us who work here, this is not a job—this is really a calling,” Beth Tharp, president and CEO of Community Anderson, told the Anderson newspaper. “We cannot do what we do here without the committed employees we have. It’s almost just who we are. We want them to feel like they are owners of what we do.”

EPIC IS A HIT AT COMMUNITY HOWARD • • • The go-live of our CareConnect Epic Systems applications at Community Howard was quite a success last month, so much so that the command center was able to shut down earlier than originally planned. And now that the new electronic medical record system is up and running, it’s getting rave reviews from users. For example:

• “Being on Epic has enabled our department to integrate with Indy in a way that allows us to better serve our patients and our community.”—Staci Vervaka, R.N., Emergency Department

• “Epic has made it much easier when rooming patients. Everything you need is right there on the top of the screen when you’re in the room with the patient. We are able to obtain all of the necessary information from the patient for our intake process much smoother. Working in a cardiology office, I have really enjoyed the new way of doing EKGs and how you can send the results directly to the physician. HAND SCAN TECHNOLOGY COMING TO ANDERSON • • • Community Anderson is set to launch PatientSecure, a positive patient identification solution to accurately and securely identify patients. The system uses the latest biometric technology for patient registration and identification, which will be particularly helpful for patients who return for regular care. Once a patient is in the system, a fast and easy palm scan is all it takes for positive ID and check-in. And it even works in emergency situations with patients who are unconscious or unable to speak.

PATIENT EXPERIENCE WORK IN THE SPOTLIGHT • • • Community’s efforts to boost the patient experience are among the initiatives profiled in a recent report titled “Patient Experience 2.0, Expanding Your Horizons,” produced by the healthcare consulting firm of Jarrard Phillips Cate & Hancock. Community had significant input in the 64-page report, according to Tom Malasto, who heads the Office of Patient Experience. Most notable is a profile of that office’s work. According to the report, Malasto’s “team relies heavily on a wide range of patient-centered metrics, reaching out, for example, to all patients who give the system a Net Promoter Score lower than 7. They’re also diligent to remember that a system’s employees and providers are at the sharp end, and that they must be well-supported by their organization to be able to provide a top-quality customer experience.”

OPEN HOUSE HONORS THE NEW ANDERSON PAVILION • • • Congratulations to the teams from Community Hospital Anderson, Community Health Network and all of our partners who have worked tirelessly over the last year to successfully construct and open Community Health Pavilion Anderson. A March 1 open house attracted more than 150 visitors, employees, leaders and patients who came to celebrate the new three-story, 68,000-square-foot medical facility. Attendees received information about the services offered at the new pavilion, toured clinical spaces and were able to meet healthcare providers who work at our newest facility in Anderson. It’s evidence of the strong appetite for more convenient, patient-focused care in our communities.

A FOCUS ON PATIENT SAFETY • • • Patient Safety Awareness Week is March 12 to 18, and we’re joining with the Indiana Patient Safety Center and the Indiana Hospital Association to highlight key areas of patient

safety. We’ll be sharing daily messages and safety tips on Facebook and Twitter to help educate and engage the public about opioid awareness, proper use of medication, fall prevention, hand hygiene, safe antibiotic use, sepsis, and safe infant sleep habits. Please join us in promoting safe healthcare practices as we target our aim of zero patient harm!

A BUSY HEART MONTH • • • Community Heart and Vascular last month celebrated Heart Month by providing screenings across Central Indiana. The Know Your Numbers screening team provided five screenings across our regions, including supporting the Go Red Luncheon, and a special Diabetes and Heart Disease in Women-A1c screening was held at Community Heart and Vascular Hospital. Community Howard Regional Health hosted the area’s First Friday event with Community Howard’s new interventional cardiologist, Anil Ranginani , M.D., speaking on overall heart health. Then Dr. Ranginani and Rajesh Mallela, M.D., along with the CV team at Community Howard, supported a free community heart screening event that included free Know Your Number screening, ABI and electrocardiogram. And our Center for Advance Heart Care’s Angela Cornelius, N.P., presented at the Fort Benjamin Harrison YMCA to the seniors program on the topic of sodium content in your food and the relationship to your heart health.

DR. KELLY GOES TO WASHINGTON • • • Community’s Tim Kelly, M.D., a leading

practitioner of addiction medicine and thought leader in the fight against the opioid

epidemic, was invited to attend President Trump’s recent address before a Joint

Session of Congress, as a guest of U.S. Sen. Joe Donnelly. According to the senator,

“I invited Dr. Kelly to highlight the critical work he and other providers are doing

across Indiana on the frontlines to help treat Hoosiers battling opioid addiction and

other dangerous substances. It’s important to shine a light on providers like Dr. Kelly

because there is an overwhelming demand for treatment services in our state.”

SUSAN G. KOMEN® ANNOUNCES NEARLY $33 MILLION IN NEW RESEARCH FUNDING TO SUPPORT BOLD GOAL OF CUTTING BREAST CANCER MORTALITY BY 50 PERCENT

Indiana Researchers Receive $705,000 in Research Funding

DALLAS – September 19, 2016 – Building on its bold goal to reduce current breast cancer deaths by 50 percent in the U.S. over the next decade, Susan G. Komen, the world’s leading breast cancer organization, today announced $32.7 million in new research grants for 2016. Awarded across 23 states and 7 countries, the projects span the entire continuum of breast cancer research, including research into metastatic disease, novel treatments for aggressive types of breast cancer, new technologies and health equity – areas that will make a significant impact in achieving the 50 percent goal. The grants include $705,000 in new funding for research at Indiana University in Indiana, bringing Komen’s total research investment in Indiana to $27,488,932 since 1982. “For nearly 35 years our organization has been a leader in the fight to end breast cancer, changing how people think about, talk about and treat this disease. Now, with a sharpened focus on our organization’s new strategic direction, we are delighted to announce new research funding that will play a significant role in making our bold goal a reality,” said Komen President and CEO Judy Salerno, M.D., M.S. “Not only will these grants accelerate our understanding of key areas in breast cancer research, but they include funding for early-career investigators. As federal research dollars become increasingly difficult to secure, these awards give promising young researchers an opportunity to establish their careers, and help ensure breakthrough breast cancer research continues for years to come,” Dr. Salerno added. “Their work is essential to achieving our vision of a world without breast cancer.” Grants from Komen’s nearly $33 million 2016 research portfolio* – including more than $16 million to early-career investigators – will focus on promising areas in research that have the greatest potential to save lives, including:

- 38 grants expanding our knowledge of metastatic breast cancer and how to stop it. - 15 grants looking into novel treatments for aggressive types of breast cancer (specifically, triple

negative, Luminal B and inflammatory breast cancer).

WEST REGION LEADER DR. WILLIAMS TO RETIRE • • • David Williams, D.O., who leads our West region, has announced his plans to retire as of April 21. Dr. Williams’ leadership and dedication have been vital as we’ve grown our services and connections in the West region and built upon our mission of enhancing health and well-being across the communities we serve. A career dedicated to health and well-being has included about three decades as part of Community Westview Hospital. Dr. Williams joined the medical staff in the practice of diagnostic radiology, and emerged quickly as a leader. He served on the hospital’s board for 20 consecutive years, served 10 years as chairman of credentials and six as chief of staff. He became part of the Community team when the hospital joined the network in 2011. In 2013, he was named president of the West region. Upon his retirement from Community, he’ll be expanding his teaching commitment at Marian University’s College of Osteopathic Medicine.

WILSON NAMED VP OF RETAIL SERVICES • • • Nichole Wilson, DPT, MBA, is assuming the new role of vice president of retail services. In this role, she is responsible for a wide range of retail patient services, including outpatient services for rehabilitation, physical therapy, sports medicine, imaging, telehealth and employer health. That includes Community MedCheck, health promotion, employee health, onsite/employer clinics, school nursing and the 12 new Community Clinic at Walgreens locations we’re rolling out later this year. She joined Community in 2005 as a physical therapist and athletic trainer.

KEITH NAMED EXECUTIVE DIRECTOR OF COMMUNITY BENEFIT • • • Priscilla Keith has been named Community’s new executive director of community benefit. It’s a role that is vital to our mission of enhancing health and well-being in the communities we serve, and she’ll play an essential part in ensuring the success of Community’s 2025

Corporate Societal Responsibility Strategic Plan that guides community benefit programs, social investments and partnerships across the network. She has been with the network since 2013, most recently serving as assistant general counsel.

MALASTO ON THE STARFISH BOARD • • • Tom Malasto, Community’s chief patient experience officer, has joined the board of the Starfish Initiative. The organization is a college access and readiness program serving economically disadvantaged high school students in Marion County through volunteer adult mentoring.
