CFC is sincerely interested in your opinion of the training programs the Cabinet offers.

Your honest and thoughtful responses can provide a basis for improved training, curriculum revision, and the overall strengthening of our training program.

InstructionsYou can use a black or blue inked pen or

pencil to complete the evaluation form.

Do NOT use hi-lighters.

If you have lodging, write the name of the hotel in the space provided. Otherwise, leave this space blank.

Evaluate the Training

You are now ready to evaluate the training. For each item, choose the response with which you most agree. Make sure you fill in the entire answering bubble that coincides with your response.

This is important to ensure that each trainer receives the correct rating and will facilitate data entry, analysis, and reporting of results.
