This Provider Resource Guide was designed to be a resource tool for individuals receiving mental health services at Allentown State Hospital as they and their treatment teams make plans for their community re-entry. Each county in the Lehigh Valley Service Area (Carbon, Lehigh, Monroe, Northampton and Pike) has their unique combination of community providers of residential services for adults receiving mental health services; these are profiled individually. In addition, different kinds of case management services are available and explained.

Consumers at ASH were asked what kind of information they would like to know about the community residential opportunities available in their county of residence. Their answers led to the creation of a template that includes the following information for each location: type of residence, number of residents who live there, setting (urban vs. rural), accessibility (steps, ramps), pets permitted, amount of supervision, medication support, level of independence, bedroom accommodations (private vs. shared rooms), transportation available, accessibility to local establishments, criteria needed to be considered for transition and funding available.

Each social worker will have copies of the directory for reference in individual or group meetings and will explain the information that is provided and answer any related questions.

Tours of the community residences are always an available option but often times an individual is reluctant to schedule a tour with their social worker because it means leaving a familiar environment for an unfamiliar one. The hope of the CIT Team is that this directory will provide valuable information to the individual and create familiarity with a residence that has the potential to become the individual’s next residence during their recovery journey.

What is CTT/ACT? This program consists of a team of professionals who provide intensive, integrated, case management and Psychiatric treatment to individuals who live in the community who are living with mental health issues. The program is community based, and provides services to individuals in the setting most natural and comfortable to them. The team operates from a “whatever it takes” attitude, and staff are available 24/7.

What does CTT/ACT do? The team offers therapy, crisis management, wellness planning, coordinating and assisting with benefits and entitlements, linkage and referral, skills teaching, Psychiatric services internal to the team, nursing services, peer support services, and many other supports and services.

Criteria Needed/Expected to be Considered or Accepted: This program is voluntary; you need to meet certain criteria to quality for program. Those criteria are; - A diagnosis of a major Axis I diagnosis (Schizophrenia, Major mood disorder, etc) - GAF of 50 or below - Lack of success at lower levels of care (ICM, RC, OP, etc)

Ejection/Rejection Policy: CTT/ACT has a “no eject/no reject” policy. Individuals must meet criteria in order to receive funding and are expected to participate in the program

What is Case Management? Case Management is a service that offers linkage, referral and connection to local resources and services in the community. It is offered to adults (and kids) who are living in the community, and receiving Psychiatric services. The program can change in intensity, frequency of contact, and caseload size depending if it is an ICM, BCM, or RC program. All three levels of care are community based, and provide services to individuals in the setting most natural and comfortable to them. In ICM and BCM, there is the availability of an after hours on call system, while traditional RC level of care does not offer an on-call program.

What does an ICM, BCM, RC do? A Case Manager can offer linkage, referral and coordination to services and resources in the community. A case manager can assist you with life goals, educational and vocational pursuits, assistance with securing benefits, and entitlements, general skills teaching, and accompaniment to appointments if needed. A case manager can also help with general support to manage the stressors of every day life.

Criteria Needed/Expected to be Considered or Accepted: This program is voluntary; you need to meet certain criteria to quality for program. Those criteria are; - A diagnosis of a major Axis I diagnosis (Schizophrenia, Major mood disorder, etc) - GAF of 50 or below - 3 or more hospital admissions in the last 6 months

Level of Independence: This program is a personal care home and as such, allows for staff assistance (when necessary) in assisting residents with daily hygiene tasks. The staff provides all of the daily living chores which allows for the resident to focus on their mental

health/medical needs, as well as, goals for independence.

Medication Needs: Staff Supervision – Staff reminds the consumer of the time to take their medications and monitors the process to assure compliance. Staff also reminds the consumer to sign the medication sheet.

Amount of Supervision: 24-Hr

Level of Independence: It is Hope Spring’s mission to assist individuals to attain and maintain success and satisfaction with their quality of life in the community and to reduce or eliminate the need for psychiatric hospitalization. Hope Springs’ philosophy, which is firmly rooted in principles of Mental Health Recovery, is based on the belief that consumers have: the ability to control their psychiatric symptoms with appropriate therapeutic and medical support, the ability to live in residential settings that are normalizing and non-stigmatizing and the potential to engage in useful and produce work activity. Hope Springs provides a high level of supervision and most activities are conducted in a group with staff present.

Transportation: Staff transports residents to appointments, work and recreational activities

Criteria needed/expected to be considered or accepted (skills level, etc): Hope Springs is designed to serve persons with serious and persistent mental illness with histories of institutionalization. Candidates are referred by Lehigh County MH/MR/D&A office in conjunction with Allentown State Hospital, or other agencies, which are providing supportive services for that specific consumer such as community treatment teams or ICM services. Candidates are interviewed by representatives from Lehigh County MH/MR/D&A and Hope Springs to determine appropriateness. Primary criteria for admission include whether the consumer can maintain adequate self-control to

avoid danger to other residents, is capable of self-preservation in case of an emergency and the consumer’s desire to live in the group home setting.

Ejection/Rejection Policy: Hope Springs embraces a “no reject, no eject” policy to the greatest extent possible and is designed to provide permanent housing for as long as the persons served wishes to remain in the home. In the event that an individual is able and wishes to move, staff will assist in

linking him or her with necessary services in the person’s community of choice. The county CHIPP Coordinator will be involved in that process.

Description of Programs: The Clubhouse of Lehigh County offers psychosocial rehabilitation services that provide hope and healing for men and women recovering from mental illness. It is a place where adults come together to meet friends, gain support and learn valuable job skills.

At the Clubhouse, we focus on developing the strength of our members and helping them journey from isolation and dependence to self-reliance and productivity, building confidence along the way.

Membership is open to any Lehigh County resident who is18 or older and has a history of mental illness. Membership is free and is good for life and attendance and participation are completely voluntary. There is no traditional therapy session or medication distribution. The work-ordered day is the core of the Clubhouse program. It is carried out by three work units, which are responsible for the day-to-day running of the house. Within the unit, members build relationships while organizing and performing the work of the day.

Candidates are referred by Northampton County MH/MR/D&A office in conjunction with Allentown State Hospital, or other agencies, which are providing supportive services for that

specific consumer such as community treatment teams or ICM services. Candidates are interviewed by representatives from Northampton County MH/MR/D&A and Hope Springs to determine appropriateness. Primary criteria for admission include whether the consumer can maintain adequate self-control to avoid danger to other residents, is capable of self-preservation in case of an emergency and the consumer’s desire to live in the group home setting.

Ejection/Rejection Policy: Hope Springs embraces a “no reject, no eject” policy to the greatest extent possible and is designed to provide permanent housing for as long as the persons served wishes to remain in the home. In the event that an individual is able and wishes to move, staff will assist in

linking him or her with necessary services in the person’s community of choice. The county CHIPP Coordinator will be involved in that process.

The clubhouse enables its members to return to paid work through Transitional Employment, Supported Employment and Independent Employment. It assists members to further their vocational and educational goals by helping them take advantage of adult education opportunities in the community, in addition to in-house educational support programs that greatly utilize the teaching and tutoring skills of our members.

Member participation is entirely voluntary. There are no agreements, contracts, schedules or rules intended to enforce participation of members. Each person chooses the way he/she wants to utilize the clubhouse.
