Borderline location • spesso considerati insieme:
- Tipo I + Tipo II- Tipo II + Tipo III- Tipo I + Tipo II + Tipo III- ca esofago + ca giunzione esofago-gastrica
• tipo II stadiato a volte come ca esofago e a volte come ca stomaco
Difficile valutare e confrontare i risultati ottenuti con le diverse tecniche chirurgiche
Strong individual preference and some degree of surgical mystique often govern selection of operation for resection
of GE junction adenocarcinoma ( Rusch 2004 )
The optimal surgical strategy remains controversial

– almeno 8-10 cm di esofago indenne a monte del tumore,
per via toracotomica dx o sx
– esofagectomia totale a torace chiuso
– resezione esofagea distale per via transiatale ( sec. Pinotti )
– resezione esofago addominale per via laparotomica esclusiva

Problemi controversi relativi all’intervento chirurgico
• volume di resezione gastrica:– resezione gastrica polare superiore (per k di tipo I e II)
minore prevalenza di trancia di sezione esofagea neoplastica
bassa prevalenza di trancia di sezione gastrica neoplastica
bassa prevalenza di LN + grande curvatura e sottopilorici
sopravvivenza a 5 anni = a quella dopo gastrectomia totale
– gastrectomia totale (per k di tipo II e III)eliminato il rischio di trancia di sezione gastrica neoplastica
possibile linfoadenectomia grande curva e sottopilorica
eliminato il rischio di reflusso gastro-esofageo

• linfoadenectomia (almeno 15 linfonodi):
– LN paracardiali, piccola curva gastrica, tripode celiaco,
origine arteria epatica e splenica, periesofagei medi e inferiori,
sottocarenali, paratracheali dx ( linfoadenectomia D2 )
– ( linfoadenectomia D1 = solo LN # 1-4 )
– ( linfoadenectomia D3 o D4: + LN recurrenziali, sovraclaveari,
paraortici )

Conclusioni
• la resezione esofago-gastrica per via laparo-toracotomica destra con linfoadenectomia D2 e’ l’intervento di riferimento
• nei centri specializzati ad alto volume di pazienti il rischio di M & M postoperatorie e’ ridotto al minimo, con miglioramento della prognosi a breve termine
• la chemioterapia o chemio-radioterapia neoadiuvante potra’ consentire di ottenere un downstaging del tumore tale da aumentare il tasso di resezioni R0 e migliorare la prognosi a distanza
