ITEM DESCRIPTION Model OT3606 (34 x 36 with 1-6 Helix) Model OTMSP2P (11 Stabilizer Plate)

TEST METHODOLOGY OT3606 was installed in class 4a soil per iostaUation instructions

TEST DATE January 162003

Mr Oliver - We have concluded testing on the above referenced anchor

A total of 3 anchors were tested AU 3 were tested at a 45-degree angle utiHzing OTMSP2P Loading was shyperformed incrementally The load uplift and deflection were recorded at each step or hold point A calibrated-Dynamometer was used to test and ascertain loading data

Tbis anchor is designed to be installed no more than 10 degrees away from the home During testing these anchors were installed at a slight back angle

A specified minimum load required was 4725 Ibs with no more than 2 movenient in a vertical direction and 3 movement in borizontal direction

RESULTS Model OT3606 was found to be acceptable based on previously mentioned acceptance criteria (see attached data log) Performance test revealed no horizontal or vertical movement in excess of specified limits

TEST METHODOLOGY OT3606 was installed in class 4a soil per iostaUation instructions

TEST DATE January 162003

Mr Oliver - We have concluded testing on the above referenced anchor

A total of 3 anchors were tested AU 3 were tested at a 45-degree angle utiHzing OTMSP2P Loading was shyperformed incrementally The load uplift and deflection were recorded at each step or hold point A calibrated-Dynamometer was used to test and ascertain loading data

Tbis anchor is designed to be installed no more than 10 degrees away from the home During testing these anchors were installed at a slight back angle

A specified minimum load required was 4725 Ibs with no more than 2 movenient in a vertical direction and 3 movement in borizontal direction

RESULTS Model OT3606 was found to be acceptable based on previously mentioned acceptance criteria (see attached data log) Performance test revealed no horizontal or vertical movement in excess of specified limits
