JOB DESCRIPTION

5-YEAR SPECIALIST REGISTRAR TRAINING SCHEME IN GASTROENTEROLOGY AND GENERAL (INTERNAL) MEDICINE

OUTLINE OF SCHEME

This is a five 5-year rotation which provide training in Gastroenterology and General Medicine as a Specialist Registrar. It will give the training and experience necessary to fulfil the training needs for a CCT in both specialities. A new 12-month post in Hepatology has been added recently to provide subspecialty training. There are 2 lecturers posts based at St James’s Hospital offering training in Gastroenterology in addition to the academic component.

The Scheme offers experience in a wide range of gastrointestinal disorders, diagnostic and therapeutic endoscopy, in addition to training in acute medicine. There are postgraduate centres incorporating libraries offering textbooks and periodicals in all the hospitals. The trainee will usually spend one year in each post starting at the beginning of October.

At present the trainees are allocated each year to a hospital where their training requirement is met, this is usually decided after the annual RITA assessments (will be replaced by ARCP). Each trainee will have an appropriate balance of hospitals in their five year period.

The training scheme is co-ordinated by the Regional Speciality Advisor for Gastroenterology/Programme Director (presently Dr M Dakkak).

With the agreement of the trainee, the Deanery, and the training committee, an SpR may be allocated to a post on another rotation, or approved out of programme experience for research or training abroad.

ERCPExcellent facilities are available for ERCP, but not all trainees can be selected for training. ERCP training is supervised by (Dr M Denyer) who is the co-ordinating consultant in conjunction with the programme director (Dr M Dakkak). A similar arrangement may developed for EUS.

AIREDALE (2 posts)

Airedale General Hospital is a purpose built district general hospital of 620 beds, opened in 1970, and is situated in the valley of the river Aire, midway between Keighley and Skipton and serving a population of approximately 200,000.

BRADFORD ROYAL INFIRMARY (2 posts)

The 6 Gastroenterology Physicians (Dr A Manning, Dr L Juby, Dr R Reynolds, Dr S Jowett, and Dr C Beckett and DR S Moreea) work within the Digestive Diseases Centre in collaboration with the GI surgeons, radiologists, histopathologists and Specialist Nurses.

On the Gastro Unit, Bradford Royal Infirmary are carried out both upper and lower GI endoscopy as well as outpatient clinics. Further endoscopy, outpatient clinics and breath tests are currently performed at St Lukes Hospital, Bradford. Training and experience may be offered for diagnostic and therapeutic UGI endoscopy, flexible sigmoidoscopy, colonoscopy, ERCP and Endoscopic Ultrasound, depending on the SpRs needs as identified at RITA and by initial appraisal. The Unit has a commitment to endoscopy training; simulators are available.

The range of clinical experience available is broad. The special interests of the consultant gastroenterologists include liver disease, nutrition, IBD, therapeutic endoscopy and medical education. The Trust is Upper GI Cancer Centre for Western West Yorkshire and weekly MDT are held as are further weekly MDTs for Colorectal Cancer. Weekly GI Radiology meetings are held jointly with the GI surgeons as well as Histology meetings every 2 weeks. Clinical

Governance meetings are held on the Unit monthly; involvement in audit is expected.

Medical undergraduate teaching involves 2nd, 3rd and 5th year students.

Junior staff currently attached to the Unit include 2 SpRs, 3 SHOs and 3 PRHOs. The weekly timetable will be negotiated with each SpR depending on their training needs.

G(I)M experience is gained on the MAU, with continuing care for a proportion of in-patients; MAU involvement entails attending CCU and ITU.

HULL ROYAL INFIRMARY and CASTLE HILL HOSPITAL (4 posts)

The 2 hospitals serve a population of 580,000. The Department of Gastroenterology has its in-patient base at this hospital, but some of its out-patient and endoscopy activity are based at Castle hill Hospital where the unit is a designated national training centre for Endoscopy. CHH also is the base for the new cancer centre.

Arrangements for post:

SpRs do 6 months gastroenterology followed by 6 months hepatology, or vice versa.

In-patients: Dr Denyer and Dr Heatley are on the wards continuously, whilst Professor Howdle, Dr Hull and Professor Sandle each spend 3 months on and 6 months off in rotation.

Dr Denyer and Dr Heatley share an SHO and HP and each have nominally 6-8 in-patients at a time, whilst the other consultant has his own SHO and HP and looks after the remaining 14-16 beds.

On-take arrangements:The SpR will be RMO 2 [resident] for acute medicine roughly 2-3 times a month. He/She will also be expected to participate in the on-call endoscopy emergency bleeder rota, which is non-resident approximately 1 in 6. The on-call duties are made concurrent as often as possible. A full shift system operates.

Out-Patients: 3 clinics per week.

Endoscopy: 2 lists per week, both consultant supervised. There may also be the opportunity to do extra lists if the SpR is fully trained in any one form of endoscopy. Some preliminary training in enteroscopy is also available.

ERCP training: can only be offered to 2 of the 6 SpRs [Hepatology and GI] and 2 Lecturers at any one time and only those thoroughly competent at upper GI endoscopy and with some expertise and colonoscopy will be considered for this. It maybe possible to arrange off site continued training for a 3rd SpR.

Training in Endoscopic Ultrasound is Available but, at present, Limited.

HEPATOLOGY – ST JAMES’S UNIVERSITY HOSPITAL – LEEDS (4 posts)

St James’s University Hospital is a large teaching hospital.

Two of these SpRs are part of a six-month internal rotation with the SJUH gastroenterology unit (luminal). The third SpR spends 1 year in post. The fourth SpR who spends 6 months in this post and 6 months in Sheffield is appointed in a competitive interview from the cohort of Gastroenterology SpRs in the UK.

The Specialist Registrar will be responsible to 4 Consultant Hepatologists: Dr M H Davies, Dr C E Millson, Dr M Aldersley and D R Jones.

The Friarage Hospital, Northallerton is a modern district general hospital providing a full range of acute medical and surgical services to a predominately rural population. SCARBOROUGH GENERAL HOSPITAL (2 posts)

The hospital serves a resident population of 220,000 but this increases considerably during the holiday season. The Gastroenterology services have been completely integrated between Medicine and Surgery for the past 10 years so that middle grade staff look after surgical as well as medical patients. A new two-theatre Endoscopy Unit containing office accommodation for all staff including SpRs and outpatient consulting facilities is currently being built and should open in 2005. Coincident with this a state-of-the-art MRI scanner will be installed.

The Gastroenterology staff at Consultant level are Dr C J Mitchell and Dr P S Babu as Physicians and Mr J MacFie, Mr K Mainprize and Mr C Mahon as Surgeons.

CONDITIONS OF SERVICE

General

The appointment is covered by the Terms and Conditions of Service for Hospital Medical and Dental Staff and by the Whitley Councils for the Health Services (Great Britain) Conditions of Service, as well as local agreements as amended from time to time.

Hours and Salary

The standard hours of duty will be the working week of forty hours, for which the salary scale currently applicable is £26,760 - £39,000 (wef 1.4.2003)

In addition, the appropriate intensity multiplier will be payable commensurate with the on-call duty performed.

Additional Duty Hours

Applicants will be aware of the current moves to facilitate a reduction in the out-of-hours work which junior doctors are asked to undertake. As a result, most rotas are under review and it is not possible to give details of rotas at each placement.

While on call the trainee may be expected to be resident in the hospital.

Study and Annual Leave

Study leave will be granted according to the nationally agreed Terms and Conditions.

The appropriate annual leave should only be taken after agreement with Consultant colleagues.

Travelling Expenses

Trainees may be eligible to claim travelling expenses in accordance with Section 23 (except paragraph 2.4 and 4) of the GWC Conditions, and paragraph 227-302 of The Hospital Medicine and Dental Staff Terms and Conditions.

Any offer of appointment is subject to satisfactory medical clearance and confirmation that the successful candidate is Hepatitis B immune. Titre levels are checked by the Occupational Health Service prior to formal offer of appointment.

Medical Indemnity

The employee is normally covered by the NHS Hospital and Community Health Service indemnity against the claims of medical negligence. Health Departments advise that employees maintain membership of a defence organisation, as in certain circumstances (especially in services for which a fee is paid) the employee may not be covered by the indemnity.

The employee must hold current registration (full or limited) with the General Medical Council.

Rehabilitation of Offenders Act 1974

Because of the nature of the work, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975. Applicants for the post are not entitled to withhold information about convictions which for other purposes are 'spent' under the provisions of the Act and, in the event of employment, any failure to disclose such convictions could result in disciplinary action or dismissal by the Authority. Any information given will be completely confidential and will be considered only in relation to an application for a position to which the order applies.

ROTATIONS

All rotations are subject to revision in the light of recommendations by the Speciality Training Committee.

ACCOMMODATION

Whilst hospital accommodation may be available for non-resident staff, this will be determined by staff making their own local enquiries. Such accommodation MUST be vacated when the trainee leaves or moves on to the next stage of the rotation.

TRAINING IN RADIATION PROTECTION

Any person who has direct control over the exposure of a patient must have a 'Core of Knowledge'.

NOTICE

A period of three months' notice is applicable to this appointment (Paragraph 197a of the Terms and Conditions of Service for Hospital Medical and Dental Staff refers). Resignation letters must be sent to Yorkshire Deanery Medical Personnel Department.

The Leeds Teaching Hospitals NHS Trust holds the employer's responsibility for all Registrars, Senior Registrars and Specialist Registrars within the Yorkshire Deanery and therefore doctors appointed to this post will be employed by the Leeds Teaching Hospitals NHS Trust.
