A Participation Agreement is required for corporate lines. For current AT&T customers with a corporate
agreement, the existing agreement must expire before activation of a new account. Exceptions can be
made for large commercial opportunities.

Employee Ordering:
Employees of member hospitals do not need to sign a Participation Agreement. Employees must
provide a Foundation Account Number (FAN). To register for a FAN go to
http://www.corp.att.com/marcomms/gpo/index.html?gpo=premier.
Employees sign up for the discount online at www.att.com/wireless/premiergpo and enter the eight-
digit FAN code to access the online portal.
Note: As long as an employee is the primary account holder, his/her family is eligible to receive the
discount.

of $39.99 or higher, unless the plan specifically states that the discount is not allowed.
Saving of 22 percent off corporate subscriber data features on $24.99 data features combined with a $34.99 monthly access or higher
Saving of 22 percent off employee subscriber (employee’s personal plan) with monthly
access of 39.99 or higher. The account must be in the employee’s name. If not, you need to call
Customer Service at 800.922.0204 and have it changed.
Free texting packages available by calling Verizon at 800.922.0204 (This can’t be done on
Verizon’s website). Owner of a calling plan can get free texting package (employee subscribers
qualify for 250 texts per month at $0.00. Corporate subscriber lines qualify for 500 per month at
$0.00. Overage on each $0.20 per message sent and received and includes text only and not
pictures or video.)
2,500 custom text message package offer: corporate subscribers only, activating new or
renewing and existing line of service, may select this option of 2,500 text messages for $14.99,
overage is $0.20 per message, per address, sent and received.

$100 bill incentive credit opportunity: corporate subscribers only, designed to help members
bringing over a large opportunity to Verizon Wireless. Members achieving 250 new activations
within a 60 period on price plans of $34.99 or higher will qualify for one time subscriber credit on
each new line which will be applied to a single corporate account within two billing cycles from
the date of calculation. Customer to qualify must send an email to the Verizon Business
representative stating their intent to participate along with their start date.
Waived activations for employee subscribers selecting 2-year term, and for all corporate

Inactive handsets: corporate subscribers only. Each member that has, on its own, at least
300 corporate subscribers will be given the option to purchase up to 2 inactive handsets, at the
then current corporate equipment matrix price. Verizon Wireless will allow the member to
replace inactive handsets once it has been activated. Member may have no more than 2

Who qualifies:
Members registered with Premier with one of the following classes of trade qualify to use the
healthcare agreement:
First responders, ambulatory care centers, behavioral health, community health center, dialysis
center, oncology center, prison correction health, public health department, student/employee
health service, substance abuse treatment, dentistry, durable medical equipment and supply
dealer, health plan/HMO/PPO that are not fortune 500 companies, healthcare corporate office,
healthcare management service organization, home health care agency, home infusion, hospice
(home care), hospitals, imaging centers, freestanding healthcare laboratory, adult day care
center, assisted living, behavioral health – inpatient, continuing care retirement communities,
closed door pharmacy, physicians, college and university – only those parts that are hospitals
and medical schools, veterinary.
Eligibility:
To be eligible for the Verizon Wireless corporate line benefits, member must maintain, at all times
minimum of 5 lines corporate subscribers and/or employee subscribers (or combination of) to access
this agreement activated on calling plans with a monthly access fee of $39.99 or higher. For details on
additional savings, please refer to the Quick Reference Guide in Supply Chain Advisor.

Who qualifies:

Private colleges and universities, private K-12, private day care, hotels, motels and casinos
Recreation: golf course, country clubs, camps, convention centers, auditoriums, museums,
parks, fairgrounds, stadiums, arenas, zoos
How to activate PP-IT-084 agreement for healthcare and non-healthcare:
Each member facility will need to execute a Verizon Wireless member agreement signed electronically
This process doesn’t need to occur for those who are already signed up on the previous agreement
(PP-IT-033) that expired on July 31, 2012. Although, Members still on the agreement that expired on
July 31, 2009 (PP-IT-209) must sign the current agreement (PP-IT-084). All members that have not
previously signed PP-IT-033, must execute this agreement.
How to activate PP-IT-084 agreement - Participating Member sign-up instructions:
1. Register for access to Participating Member content on www.premierinc.com. If you are already
registered, skip to step 2. If not, follow the bullets below:
Go to www.premierinc.com and click on the “Register” link located in the upper-right hand corner of the home page
Read and agree to the Terms of Use
Complete Sections 1-3 of the registration form.
Skip Sections 4 and 5 of the registration form and click the “Submit” button at the bottom of the form
You will receive an email from Premier in 1-3 business days containing your user ID and password
2. Once you are registered, and have received your username and password, go to https://premierconnect.premierinc.com to gain access to the Verizon Wireless contract portal site located in PremierConnect
Log-in using your premierinc.com login and password
Click the “Knowledge” tab at the top
Search “Verizon Wireless Agreement” in the top right field and click “Go”
Select the category launch document link at the top
Click the “Verizon sign-up” link on the right 3. You are now in the Verizon Wireless contract portal site for Participating Members. Please
complete the online form and accept the Member Agreement
Verizon Wireless will create or update an account, and notify the individual who accepted the Member Agreement of their approval within 2 – 5 business days. If additional information is needed, or if information entered in the online form does not match the Verizon Wireless system, Verizon Wireless will send notification to the e-mail address of the individual accepting the Member Agreement

Employee Ordering Process:
Note: Before rolling this program out to your employees, an authorized
person from your organization must first electronically activate the Verizon
Wireless agreement (See above instructions “How to activate PP-IT-084 agreement - Participating
Member sign-up instructions”) to enable availability of this program to your employees.
How to register your employee line: Member’s employees (with proof of employment) may
activate new or register existing lines of Wireless Service on plans, options, features, and
applications that Verizon Wireless makes generally available to consumers and obtain the applicable
monthly access fee discounts on eligible plans and eligible data features as well as corporate
equipment pricing, not to exceed 5 lines per employee, provided such employee first: (a) registers
under Member’s account at www.verizonwireless.com/getdiscounts; (b) executes a Verizon Wireless
retail customer agreement; and (c) qualifies under Verizon Wireless consumer credit criteria.
Employee discounts and pricing may not be available in retail stores or through Verizon Wireless
agents. Upon request, Member shall confirm the employment status of employee subscribers.
Unlimited voice plans and mobile broadband lines do NOT qualify for monthly reoccurring discounts.
If you do not have a corporate email, then use the End User Form and provide it with a current copy of a paystub or badge for proof of employment. (Allow 1-2 billing cycles to take effect). Email the form to: [email protected]
Once you register for the discount:
Go to www.verizonwireless.com and register for My Verizon. Here you will be able to maintenance your account, order devices, accessories and make any changes needed to your account. The online transactions will insure your discounts. Add lines, upgrade and by all equipment through the My Account.
