OBJECTIVES

1. Toequiptraineeswiththeresearch,scientificknowledgeandskillstobecomeoutstanding researchers ininfectiousdiseasesandglobalhealth.

2. To create a novel and stimulating multidisciplinary and truly international research training environment that fosters creativity,opportunity,andinnovation,andonethatdemandsexcellence.

3.To harness the unique opportunity offered by the critical mass ofinfectious diseases and global health infrastructure, researchopportunities and outstanding scientists in the training of the nextgenerationofinfectiousdiseaseresearchers.

4.Tomake availablecollaborative international research sites for thetrainees’primaryresearchprojects,sitesforresearchpracticaandmajorcourseofferings.

5.To offer a shared learning environment, where trainees andmentorsfromallfourofCIHR’sresearchpillarsandthefourinternationaltraining sites work cooperatively to explore issues of internationalinfectiousdiseasesandglobalhealth.

TRAINEE SPOTLIGHTNadia Musimbi ChanzuisaPhDstudentinInfectiousDiseaseat theUniversityofNairobi,Kenyaunder thesupervisionofProf.WalterMwanda,Prof.OmuAnzalaandDr. JuliusOyugi.Nadia applied to the trainingprogrambecauseshewasinterestedintheopportunitytoinvestigateinfectiousdiseasesinKenyawhicharenowbecoming increasingly available froma technologicalstandpoint,however,themanpowerandexpertisewerelacking.Sincejoiningtheprogramin2011,NadiahasbeenawardedanHIV/AIDSPreventionTrialsCapac-ityBuildingGrant from theGlobalHealthResearchInitiative-InternationalDevelopmentResearchCentre(GHRI/IDRC).HerPhDresearchfocusesonSecretorStatus,Fucosyltransferase2(FUT2)GenePolymorphismsandSusceptibilitytoHIVInfectionamongFemaleSexWorkersinKenyawhichisdesignedtoexplorethevari-ableexpressionofbloodgroupantigensinthecontextofHIVresistanceinordertobetterunderstandthemucosalmechanismsofprotectionassociatedwithbloodgroupantigenexpressionprofiles. Inconductingthis researchNadiahopestoprovideadditionalinsightintothedevel-opmentofnewHIVpreventivetechnologies.

Nadia’s favouriteaspectof the trainingprogram is theInfectiousMindssessions.AsabasicscientistshefeelsthatthediversityoftheInfectiousMindssessionsprovideanexcellentopportunityforhertobroadenherscopeandknowledgebeyondHIVandImmunology/Haematology.Also shehas found that the InfectiousMinds sessionsallowedhertogainamorecomprehensiveunderstandingofscience,beyondbasicscienceandHIVresearch,whichshecreditstopresentationsgivenbyexpertguestspeakerswhohavediversebackgrounds.

Elizabeth Cooper, aPhDStudentinCommunityHealthSciencesattheUniversityofManitobaandatraineeintheIIDprogram,hasaBachelorsdegreeinReligiousStudiesandHistory,andaMastersdegreeinNativeStudies,bothfromtheUniversityofManitoba.

Elizabethisinterestedincommunitybased,participatoryactionandhealthdecision-makingamongIndigenouscom-munities.ThecommonthreadthathasalwaysconnectedtheworkElizabethdoesisthedesirefordifferentvoicestobevisiblewithintheoverarchingfabricofmain-streamsociety.ThroughthecourseworkinElizabeth’sPhDprogram,com-binedwiththeopportunitiesaffordedbytheIIDprogram,Elizabeth isgainingworkingknowledgeofbiostatistics,epidemiology,microbiology,economicsandpolicyforma-tion.Shebelievesthatworkingwithinaninterdisciplinaryacademicenvironmentiskeytocreatingopportunitiestoenactchangewithintheworld.Workingwith,andlearn-ingfrom,individualswithdiversebackgroundsallowsfordifferentviewstobeseenandactualized.

OneofthemostpoignantmomentsElizabethhasexperi-encedaspartofthetrainingprogramoccurredduringoneofthelearningvisitsinIndia.Membersfromalocalnon-governmentalorganizationsangfolk-songstothetraininggroupandElizabethandsomeofhertrainingcolleaguesandadvisorssangsomefolk-songsfromCanadainreturn.TheindividualsfromIndiasaidthattheyalwayssingforvisitors,butthisisthefirsttimetheycouldrememberpeoplesingingforthem.ThiseventhighlightedtoElizabeththeneed forongoingdiscoursebetweenorganizationsandindividualsaswellastheongoingneedfordifferentmodesofknowledge-translation and trust-building activities.BeingabletolearnaboutdifferenttechniquestoconnectwithpeopleinameaningfulwaythatisappropriatetothecontextisanintrinsicskillthattheIIDprogramworkstodevelopinallofitstrainees,andanimportantpiecethatisoftenmissingfromacademictrainingprograms.

Dr. Jayanna Krishnamurthy,aclinicianandcommunityhealthspecialistfromBangalore,India,joinedthetrainingprograminJanuary2011asaPost-DoctoralFellowtrainee.Hehas10yearsofexperienceimplementingpublichealthprogramsandconductingresearch in theareasofHIVandMother,NewbornandChildhealth(MNCH).His

researchfocusesonthequalityofMNCHinpublichealthfacilitiesofpoorperformingdistrictsinNorthKarnataka,India.Thestudyspecifically looksattheeffectivenessofonsitementoringprogramsbyacadreofnursementorstotheprimaryhealthcarecentersaboutthequalityofmotherandnewbornhealthservices.FindingsfromhisresearchhavethepotentialtoinfluencethepoliciesandprogramsinIndiainthewaytheylookatthequalityofMNCHcare.Asapartof the trainingprogram,Dr.Krishnamurthy attendedthe2011majorcourseinBangalore,IndiawhichwasaimedatprovidingthetraineeswithopportunitiestounderstandtheissuesandchallengesinimplementingHIVprograms inadevelopingcountry setting.Hesaid thatattendingthiscoursewithtraineesfrommultidisciplinarybackgroundsofferedauniqueadvantagetounderstandingdifferentperspectives,whichhefeelsisthebestpartoftheIIDprogram.

Dr.Krishnamurthyhadaveryeventfulyearin2011.Heco-authoredtwopeerreviewedarticleslookingatpositivepreventionpracticesandmorbidityprofilesofHIVinfectedindividualsincaresettingsinSouthIndia.HecontributedtoeightresearchreportsdescribingMNCHsituationsinNorthKarnatakadistrictsandhewasawardedthe“YoungScientistAward”fromtheKarnatakaAssociationofCom-munityHealth(KACH)fortheyear2010-11.ThisawardisaimedatencouragingyoungandbuddingpublichealthspecialistsinIndiatocontributetopublichealthprogramsandresearch.

Natalia Taborda-Vanegas isaPhDtraineeat theUni-versidaddeAntioquia,inMedellin,ColombiaunderthesupervisionofDr.MaríaTeresaRugeles.HerdoctoralthesisfocusesonHIVandthemechanismsrelatedtothecontrolofviralreplicationingut-associatedlymphoidtissueandperipheralbloodinHIVcontrollers.Dr.MaríaTeresaRugeles,whoisamentoroftheIIDprogramencouragedNataliatoenrollbecauseitwasagreatopportunityforhertolearnandworkwithresearchersfromseveralcountries.Nataliabelieves thatbecauseher fellow trainees come

fromdifferentepidemiological,socialandeconomicback-groundsthereareseveralpointsofviewcomingtogethertosolveacommonproblemandthisiswhatmakesthisprogramaninvaluablelifeexperience.

In2011,Natalia attend the“Planning, implementingandevaluatingscaledHIVpreventionprogramsamongvulnerablepopulationsinresource-poorsettings”courseinBangalore,India.Shesaiditwasanexcellentexperiencebecauseshewasabletoworkonaproposalaboutatopictotallydifferentfromherresearchareaand,therefore,wasabletolearnalotfromthecourse.Alltheactivitiesperformedintheprogramhaveenabledhertounderstandherresearchtopicfromawidepointofviewaswellashelpedhertoimproveherunderstandingoftheimportanceofherresearchandhowitcontributestosociety.Shebelievesthatthetrainingprogramwillhaveapositiveimpactinbothherfuturecareerasabiomedicalscientistandherpersonallife.

Dr.ArupDassuccessfullycompletedhisPhDinPopulationScience/DemographyattheInternationalInstituteforPopulationSciencesinMumbaiunderthesupervisionofDr.SKSingh.Aswell,hegraduatedfromtheIIDprograminDecember,2011.Sincecom-pletingthetrainingprogram,AruphasbeenworkingasaMonitoringandEvaluationexpertfortheBillandMelindaGatesfoundationfundedproject“Integratedfamilyhealthinitiative”whichisaimedatimprovingmaternal,neonatalandchildhealthoutcomesinBihar,India.Arup’sroleintheprojectistodesignandimple-mentdifferentevaluationstudiesincludingrandomizedcontroltrials.AccordingtoArup,“themostvaluablecomponentofthetrainingprogramisitsmultidisci-plinarynature.Asasocialscientist,Inowdonotfeelconfinedonlytomydiscipline,myhorizonsarebroaderaftercompletingthecourse.Iamconfidentingettinginsidetheclinicalorbasicscienceaspectofpublichealthproblems;atleastIwillmakeanattempt.”

ACADEMIC PROGRAM“Planning, implementing and evaluating scaled HIV prevention programs among vulnerable populations in resource-poor settings”by Dr. Srinath Maddur, Ms. Parinita Bhattacharjee and Dr. Stephen Moses

ThecourseconsistedofafewdaysoffoundationalpresentationsandsomesitevisitsinBangalore,Indiafollowedbylearningvisitstooneofsixfieldsites.ThetraineesweregivenanassignmentatthefieldsitestodesignandpresentHIVinterventionprogramsforaparticularhighlyvulnerablegroup.AworkshoponplanningimplementingandevaluatingscaledHIVprevention,careandsupportprogramsamongvulnerablepopulationsinIndia(femalesexworkers[FSWs]andmenwhohavesexwithmenandtransgenderedindividuals[MSM-T]),wasjointlyconductedbytheIIDprogramandtheKarnatakaHealthPromotionTrust(KHPT)forIIDfellowsfromFebruary14-25,2011,inKarnatakastate,India.Theworkshopwasdesignedtoinform,engageandchallenge the fellowswith respect tokey issues inHIV/AIDSprevention,careand supportprogramming. It consistedof severalmodules,usingaparticipatoryandexperientiallearningprocess.TheworkshopaimedatbuildingabroadknowledgebaseregardingHIV/AIDSprevention,careandsupportissuesinaresource-poorsetting.Fellowswereintroducedtotheprocessesofplanning,designing,imple-menting,monitoringandevaluatingHIV/AIDSprogramsfocusedonthemostvulnerablepopulations.

Theworkshopstressedthescientific,social,behaviouralandhealth-relatedaspectsofHIV/AIDSprogramming.Itbeganwithpresen-tationsandfieldvisitsinBangalore,whereKHPT’sheadofficeislocated,andlaidafoundationinunderstandingHIVepidemiol-ogyandimportantperspectivesaroundHIV/AIDSissues.Thisintroductiongavethefellowsafirst-handexperienceoftheHIV/AIDSepidemic.Thefellowswerethendividedintosixgroups,andtravelledtovarioussitesinruralKarnataka,wheretheywereexposedtoactualprogramsituations.Atthesefieldsites,theyweregiventhetaskofdevelopingvariousproposalsonrelevantandchallengingtopics.EachgroupspentapproximatelyfivedaysinteractingwithvulnerablecommunitiesofFSWsandMSM-T,assessingtheirneedsandunderstandingtheirchallenges.TheythendevelopedresearchandactionproposalsthataddressedaspectrumofHIV/AIDSpreventionandcareissues.ThefellowsthenreturnedtoBangalore,wherethegroupspresentedtheirproposalstoapanelofexperts.Proposalswereratedfortheirunderstanding,feasibilityandpracti-cability,andweregradedaccordingly.Thequalityoftheproposalsdevelopedwasuniformlyexcellent,andKHPT,incollaborationwithsomeofthefellows,hasfurtherdevelopedsomeoftheseproposalsforimplementation.

Feedbackfromthefellowsindicatedthattheyfoundtheexperiencetobevaluableandenriching,andthattheywereabletodevelopamuchdeeperunderstandingregardingavarietyofHIV/AIDSissuesintheIndiasetting.Theyalsoacquiredknowledgeanddevelopedskillsinprogramdesign,implementation,monitoringandevaluationwhichwillbeofgreatvaluetothemintheirfuturework.

InFebruary,theCIHRInternationalInfectiousDisease&GlobalHealthTrainingProgram(IID)offeredatendayintensivecourserunbytheKarnatakaHealthandPromotionTrust(KHPT)inBangaloreIndiaentitled:Planning,implementingandevaluatingscaledHIVpreventionprogramsamongvulnerablepopulationsinresourcepoorsettings.Thirty-onetraineesfromCanada(UniversityofManitoba),Colombia(UniversidaddeAntioquia),India(St.JohnsUniversity),andKenya(UniversityofNairobi)cametogetherforthisuniquelearningopportunity.TheCIHRIID’sthemeis“fourcontinents,onesharedexperience”andthecourseinIndiastrovetomakemultidisciplinaryinternationalhealthresearchanembodiededucationalexperienceforthoseinvolved.

InpreparationforafivedayfieldvisitinruralIndia,thefirstdayoftrainingfocusedonacademicpresentationscontextualisingHIV/AIDSwithinIndia.Onthesecondandthirddayoftrainingwewenttovisitthreesocialorganizations,ontheseconddaywevisitedanorganizationthatworkswithFSWandanorganizationthatworkswithMSM.Bothorganizationsprovideadvocacy,healthandsocialsupportfortheirmembersandextendedsocialnetworks.Wehadtheopportunitytohearapresentation,haveteaandvisitwithbothstaffandclients.ThestafffromKHPTtranslatedHindi/KannadaandEnglishaspeoplehappilysharedstoriesaboutvariousexperiencesrelatedtotheirworkandpersonalliveswiththetrainees.Inthelateafternoonwewentonawalkingtourtositeswherecondomsaredistributedtoindividualswhofrequentlyengageinsexworkorrecreationalsexualactivities.HalfofthetraineeswenttoalocalbusstationsituatedinabustlingnoisyareaofBangalorewithnumerous

shops,restaurants,publicwashrooms(wherecondomsareleftintubesforeasyaccessbyFSW),andhotels.Theotherhalfwenttoalargepublicpark,meetingwithMSMwhowererelaxingattheendofalongday(condomsareleftincasesunderidentifiedbambootreesforeaseofaccessbyMSM).AdaylongvisittoSnehadaan,aPLHA,palliativecarecentre,supportservicescentre,andorphanageprovidedfurtherinsightintodisparitiesfacingindividualsaffectedbyHIV/AIDSinKarnataka.Watchingtrainees,aswellasKeithFowketryingtolearnyogafromafewoftheHIV/AIDSaffectedorphanswascertainlyahighlightofthetrainingprogramforeveryone-especiallythechildren.

Ondayfourwepackedourbagsandboardedanovernighttrain(orbus)forruralIndia.ThetraineesweredividedintogroupsoffiveandsenttoruralareastoexaminekeyissuesaroundHIV/AIDS.Attheendofourfivedayfieldvisit,afternumeroustrainingopportunitiesincludinginformalinterviews,focusgroupsandmeetings;wecreated“outofthebox”interventionproposals,presentedattheendofthecourse.

Theissuesexaminedincludedidentificationofnewfemalesexworkers,condomusewithintimatepartners,economicstrategiestoassistwidowsaffected/infectedbyHIV,improvedaccesstohealthcareforMSM,improvedfostercareforchildrenaffected/infectedbyHIV,aswellasGISasadecisionmakingtool.

TheentireinternationallearningexperienceinIndiawasvaluable.Workingasinternationalgroups,withinaninternationalcontext,comingfrominterdisciplinaryacademiccontexts,learningside-by-side,eatingIndianfood(withtheoccasionalwesternKFCmealorpastadishprovidedbyKHPTforthosewhomissedtheirNorthAmericandiets),visitingtemples,shoppinginbazaars,laughingatmonkeysastheyclamoredonvehicles,lookinginaweattheelephantsandcowsthatwalkedby,andmostimportantlyhavingtheopportunitytospendtimewithpeopleworkingwith,livingwithandaffectedbyHIV/AIDS.Thisuniquetrainingofferedanexperiencenoneofuswillsoonforget.

CIHR IID Program 2011 Scientific SymposiumTheScientificSymposiumwasheldonFebruary25,2011at theKHPTofficesinBangalore,India.Thesymposiumhighlightedeachtrainee’sresearchandthetraineesinvitedaguestspeakertotakepartinthesymposium:Dr.SKSingh,AsstProfessor,DeptofMathematicalDemographyandStatistics,IIPS,Mumbai.

Thepurchaseofvideoconferenceequipmentforeachinternationalsitehasenabledimportantscientificdiscussionstotakeplaceinrealtimeandhasallowedthesmallersitesthebenefitof“criticalmass”. In2011,nine InfectiousMindsmeetingswereconductedwitheachsessionbeingorganizedbyateamofthreetrainees.Theteamresponsibleforthesessiondecidedonatopic,invitedaguestspeaker,presentedtothegroupandheldaquestionandanswer/discussionsession.TheInfectiousMindsmeetingsarecoordinatedbyDr.JavierMignone,amentorofthetrainingprogramandapasttrainee.

Practica 2011 ReportTheintentionofthepracticumportionoftheCIHRInternationalInfectiousDisease&GlobalHealth(IID)TrainingProgram,coordinatedbyDr.JohnWylie,istoexposetraineestoacompletelydifferenttypeoflearningexperienceapartfromtheirmainresearchareaandtofosterinterdisciplinaryresearch.Thepracticaareintendedtolastfromfourtotwelveweeksandtobetailoredtothespecificgoals,objectivesandtimelinesofeachtrainee.TodateallinformationaldocumentsrelatedtothePracticumportionoftheIIDprogramhavebeenprepared.Thesedocumentsareintendedtoactasaguidefortraineestoassisttheminsettinguptheirpracticum.Thesedocumentsconsistofastep-by-stepguideforthetraineestohelpthemsetuptheirpracticum,asetofquestionsthatthetraineescanusetohelpthemdevelopaframeworkforwhattheywouldliketogetoutoftheirindividualpracticumandalistof20possiblesitesthetraineescouldconsiderasapossibletrainingsite.Traineesarealsoencouragedtoseekoutothersites,asapplicable,toreachtheirrespectivepracticumgoals.Finally,apracticumevaluationformhasbeendevelopedwhichallowstraineestheopportunitytoprovidefeedbacktotheIIDprogramontheircompletedpracticumexperience.AsofSeptember2011,twotraineeshavecompletedtheirpractica.Abriefdescriptionofthesepracticaareprovidedbelow:

James SainsburyhascompletedhispracticuminOttawawithLiniAl-Kharki,PublicHealthLawandEthicsProgram,PubicHealthAgencyofCanada.HispracticumsupplementedhisbasicsciencePDFresearchbyprovidinghimwithexperienceinpolicydevelopment.

Zulma RuedapursuedherpracticumwithDr.Bangdiwala,DepartmentofBiostatistics,UniversityofNorthCarolinaatChapelHill.Herpracticumsupplementedherepidemiology-basedPhDResearchbyprovidingbasicexperienceinbiostatisticalmethodologyapplicabletotheanalysisofdataarisingfromherPhDresearch.

AvideoconferencemeetingwithalloftheremainingtraineesoccurredinNovember2011toreviewthepracticumrequirementoftheIIDprogramtoprovidefurtherassistancetotheminsettinguptheirpracticum.PossiblechangestothisportionoftheIIDprogramarebeingconsideredtoallowtraineestoengagein“skilldevelopment”andacquirenewresearchskillsthatarecloselyrelatedtotheirprimaryresearchprograms,butunavailablewithintheirsupervisor’sresearchteam.Thischangeisbeingconsideredtomaximizetheusefulnessofapracticumexperienceforboththetraineeandtheirsupervisor.

International HIV Prevention ExchangeTheUniversitiesofManitoba&NairobiHealthInitiative(UMNHI)collaboratedwiththeWinnipegSymphonyOrchestra(WSO)duringtheNewMusicFestival(NMF)tobringforththeInternationalHIVPreventionExchange.

OnJanuary29,2011,theWSObroughtbackJohnCorigliano’sSymphonyNo.1aspartoftheiropeningnighttotheNMF.ThisworkwascomposedbyCoriglianoashispersonalresponsetotheAIDSepidemicduringthelate1980s.

Tocoincidewiththeperformance,theUniversityofManitobaandtheIIDprogramorganizedanInternationalHIVPreventionExchange(TheExchange)whichwasheldinWinnipegonJanuary28-29,2011tocreateadiscussiononHIVresearch.Thetwo-daysymposiaandworkshopsculminatedwiththeopeningnightoftheWSO’sNMFandincludedapre-concertvideopresentationoftheHIVprojectworkinNairobiandIndia.

TheExchangewaschairedbyDr.KeithFowke,Dr.MarissaBeckerandDr.JamesBlanchardandwasorganizedbyagroupofpeoplefromtheMedicalMicrobiologyandCommunityHealthSciencedepartmentsattheUniversityofManitobaleadbyJudeZieske,LynnGauthier,andDr.AdrienneMeyers.

ManyofthepresentersandthoseinattendanceincludedtraineesandleadersintheCIHRInternationalInfectiousDiseaseandGlobalHealthTrainingProgram.ItbroughttogetherlocalandinternationalresearchersandcreatedawarenessofHIVresearchanditsglobalimpacttothosetakingintheWSOconcert.

PROGRAM ADVISORY COMMITTEE TheProgramAdvisoryCommittee (PAC) is composedof keystakeholders suchasgovernment, industry,communitymembers,non-profitsector,academia, internationalpartnersandoneofourtrainees.ThiscommitteewasstruckinMarch2010andDr.GaryGlavin,AssociateVice-President(Research)acceptedtheinvitationtoserveasthePACChair.TheresponsibilitiesofthePACaretoadvisethePrincipalInvestigatorandCoreCommittee(Drs.Fowke,Wylie,Mignone,andMoses)oftheprogram,providerecommendationsandfeedbackwithregardtotheprogramgoals,visionandresponsibility.ThefirstmeetingofthePACtookplaceonMay12,2010andasecondmeetingwasheldonOctober14,2011.

Dr.KeithFowkeasked the international sites (India,KenyaandColombia) toupdate thePACmembers on theprogram.ThehighlightsfromthemostrecentmeetingweretheInfectiousMindsScientificVideo-conferencesessionsandanupdateonframeworkofthePracticumportionoftheprogramrequirements.

PLANS FOR 2012ThetraineeswillbeinNairobi,Kenyatoattendthe“ClinicalTrialsandResearchEthicsinaDevelopingCountrySetting”coursefromJanuary29–February12,2012hostedbytheKenyasiteleaderDr.JuliusOyugi.
