DESCRIPTION

European Aviation Safety Agency Insert date Insert title Slide 3 Example 2 The Gulfstream Aerospace G-IV aircraft, registration N-XYZ, was westbound at FL 400 when its transponder failed. A Cessna 550 aircraft, registration N-BBB, was 35 miles behind and also proceeding westbound at the same altitude. N-XYZ and the concerned ACC identified the transponder unserviceability. The flight crew switched over to the secondary transponder and the aircraft was located with less than the required non radar separation of 10 minutes. This also resulted in an airspace incursion with the adjacent Air Traffic Control facility.

Example 3

The Boeing 767-300 aircraft, N-AAA operating as ZZZ999, was en route from MYTOWN to YOURTOWN and at a position 110 NM NW of SOMEWHERE when an "oil filter clog" message appeared on ECAM for the number one engine (General Electric CF6-80). The crew secured the engine, declared an emergency with the concerned ACC, and requested and received clearance for a diversion to SOMEWHERE. The aircraft landed at SOMEWHERE 25 minutes later without Incident. Maintenance inspection revealed that it was an indication problem and the aircraft was dispatched under the MEL for a ferry flight to THEIRBASE.

Example 4

The Embraer ERJ-170-100 SE aircraft, N-AAA, was on a flight from MYTOWN to YOURTOWN and when the flaps were selected down during the approach to YOURTOWN, a slat fail message appeared. The crew declared an emergency and the aircraft was placed in a holding pattern while the crew referred to QRH to try and resolve the problem. Their attempts were unsuccessful and a zero slats, zero flap landing was carried out without incident. .

Example 5

The Cessna 172 aircraft suffered damage as a result of being upset by the jet blast of a twin-engine Global Express (GX) aircraft as the Cessna, while taxiing to exit the runway, crossed the jet blast from the GX aircraft. The GX aircraft was positioned off the runway on an engine ground run pad. The Cessna right wing and nose were lifted from, and subsequently dropped back to, the runway surface twice in rapid succession as it taxied through the jet blast from each of the GX engines, both of which were at high power. The Cessna nose-wheel assembly and engine firewall were damaged

Example 6

The aircraft, with two pilots and five passengers on board, was en route on an IFR flight plan from AAA to BBB. At around 30 nm north northeast of BBB, the pilot became concerned about the low state of the aircraft fuel supply at which point the flight crew cancelled the IFR flight plan and diverted the aircraft for a landing in CCC. Approximately two nm on final approach for Runway 27 at CCC, the left engine stopped and the right engine began surging. The pilot feathered the left engine and landed the aircraft without further incident.

Example 9

The Cessna 152 aircraft, C-GCGI, took off from AAA for a VFR flight to BBB. Soon after take-off the pilot noticed that the low voltage warning light was on. The pilot checked circuit breakers and found that the voltage regulator circuit breaker was popped. He decided to return to AAA where the aircraft landed without further difficulty. The voltage regulator was found to have failed and is to be changed.

Example 10

At FL200 the aircraft encountered severe icing. The crew requested and received clearance to FL240 but the higher the aircraft climbed, the worse the icing became. The propellers began to vibrate loudly and both windscreens became covered with up to an inch of ice. Several attempts were made to shed ice from the propellers and windscreens without success. The crew decided to divert to AAA, the nearest suitable airport, where the aircraft landed without further event.

Example 11

The a/c was landing in AAA. During the landing roll, at approximately 1500 feet, the brakes appeared to lose effectiveness and the emergency brakes were applied. The aircraft departed the end of the runway at approximately 5 MPH and came to rest against a fence. No damage or injuries were reported. The aircraft was recovered and prepared for the next leg .

Example 13

The Boeing 757 aircraft was en route from AAA to BBB when the crew declared a PAN due to a smell of smoke in the cockpit. The flight was vectored for a return to AAA and landed without incident 23 minutes later with AFF on standby. During the return to AAA, the crew had isolated the source of the smoke smell to the number one air conditioning pack. After landing, maintenance found that the number one air conditioning pack had seized.

Example 15As the aircraft (767-300) was cruising over the Atlantic, 400 miles from Gardner cabin crew informed the flight deck that there was an unruly passenger in seat B48. Although the passenger was warned several times that he would be restrained or prosecuted he became more violent and vulgar. The crew decided with the assistance of passengers to restrain him. During the procedure, one of the  passengers assisting got hurt by the unruly passenger (cuts and bruises in the face and nose bleeding). The unruly passenger was successfully restrained. And he was taken by police upon arrival at Qwer.  The injured passenger was treated by medical services and was hospitalised for 3 days.

Example 16An A340, entered a runway at Qwer Airport without traffic control permission just before a A320-100 was about to land on the runway, The A320’s crew aborted the landing. The Airbus A340 was instructed by an air traffic controller to hold its position just before the runway to accommodate the arriving A320-100. The crew of the A340, however, did not repeat the air traffic controller's instruction to wait - as normally done - but instead said "Position 24L," the code for the runway, and then entered the runway. The incident took place at around 6:10 p.m. The air traffic controller thus ordered the A320-100 plane go-around. The A340, at the instruction of the controller, moved to a taxiway and took off about 10 minutes later. The controller asked the pilot just before the takeoff whether the pilot had obtained permission to enter the runway, and the pilot replied he thought it had been granted and repeated the instruction to that effect.
