What is Vitality?

Step 0: Register your HumanaVitality Account
1. Already Registered: Sign in with User Name and Password. 2. Need to Register? Click on “Register Now”

Register: Click on the “member” box

Click on “all other members”, enter personal information, and click “continue”.
• Your 9-digit member ID will look like this H12345678. The second character following the H in the member is a number and not a letter. (i.e. 0 and not o).
OR • Your Social Security number • Enter the date of birth with all eight digits (i.e.
10/01/1987). • Zip code must match the home zip code currently
on file with Humana. If you have recently moved and your new zip code is not being accepted, try using the old one.

Create a Username/Password
Username requirements: • 6 to 15 characters (letters
and/or numbers) • No spaces or special
characters • Do not use your member ID
or Social Security Number Password requirements: • Minimum of 8 characters • Must include at least 1
number • Case sensitive
Be sure to write down your username and password for future use!

Agree to the website terms by clicking “I agree”.

Sign in--After completing the registration process, return to Humana.com to sign
in using the username and password you just created.

Step 1: Take the Health Assessment
Signed In: Dashboard NOTE: Once you’re signed in, click on the alert to “Take the Health Assessment” or look for the “Health Assessment” link under the “Get Healthy” tab.

Click on “Get Started” The Health Assessment takes about 10-15 minutes to complete. It’s a series of questions about your current mental and physical well-being, your day-to-day lifestyle, and how you feel about your current health levels.

Complete the Health Assessment Questions If you know your medical history and key measurements have them ready to help you to complete your Health Assessment. Don’t worry if you do not have your key measurements you’ll still be able to complete the Health Assessment.
Note: If you had a Vitality Check, a blood screening and biometric assessment, which measures your Body Mass Index (BMI), blood pressure, blood glucose and total cholesterol, within the last 18 months, you will see that those results have pre-populated into your Health Assessment. These cannot be updated until a new Vitality Check is submitted.

younger than your actual age

• Achieve Silver Status – 5,000 Vitality Points for employee coverage, 8,000 Vitality Points for
employee/spouse
Step 2: Complete a Wellness Option

• Biometric assessment and screening allowing you to learn more about your overall health and possible risk areas.
• Find out total cholesterol, HDL cholesterol, blood pressure, blood glucose, and body mass index (BMI).
• Vitality Check appointments can be completed – Well@Work clinic
• Please make an appointment online at www.healthpartners.com or by calling HealthPartners appointment line at 952-967-6857.
– During a preventative care visit or other appointment where biometrics are conducted
• Health screening results must be reported to Vitality to qualify

Vitality Points from the On-line Course
Based on your personal health results, HumanaVitality will recommend goals. Course must be part of an ACTIVE goal to receive Vitality Points
Goals are separated into “recommended” and “active” goals. Set the goals you are ready to attempt.

Once a goal is set you can return to the goal for activities to help you achieve the goal including a recommended on-line course.

You earn Vitality Points for Courses that are recommended to you for a particular Goal. Points will be displayed within your Recommended Goal. Courses run from between 2 and 12 weeks. Upon completion of this tool, your activity will be submitted for point's calculation. Points will be reflected on your Vitality Statement within 48 hours of completion.

The more you do to stay healthy, the more Vitality Points you can earn. When you have other members of your household engage with Vitality, their healthy activities can count toward VitalityPoints, too. The more VitalityPoints you earn, the higher your Vitality status, giving you more Vitality Bucks to spend at the Humana Vitality Mall and discounts up to 40 percent on your purchases.

Examples of ways to reach Silver Status (Single)
• Take the health assessment and complete a Wellness Option by May 31, 2015
(Employee and covered spouse if applicable)
• Qualify for Preferred Benefit – NationalONE 200-25 plan: $20 differential of the office visit co-pay
– High Deductible Plans: $250 single/$500 family deductible differential (NationalONE VEBA, $2000-100%, $1500-100%, and HSA $2500-100%)
For example, if an employee and spouse on the NationalONE 200-25 plan choose not to participate, then starting July 1, 2014 the employee and spouse (and dependents on the plan) will have a office visit copay $20 greater then those that completed the program.
