How to fill out the Questionnaire form

We have prepared the Questionnaire form for you to fill out and submit to us before your first

appointment. You can choose to fill out the form after downloading it from our website

[http://www.yapspecialist.com.au] or you can ring our receptionist to obtain a copy sent out to you via

email.

You can choose one of three ways to fill out the Questionnaire form:

Handwritten Print-out

PDF Fill & Sign

Microsoft Words – Type & Save

Just press the Ctrl key before clicking your cursor arrow on your preferred choice. This will take you to a

simple step-by-step guide on how to go about filling out and submitting the form.

Handwritten Print-out

Step 1: Open the file and Print it out

Print out the Questionnaire form without changing the format or page layout of the form.

If you open the Pdf document, go to the top left corner of the page and click on the printer icon

If you open the Microsoft Word document, go to the top left corner of the page and click on File tab

to go to the File page, then click Print to go to the Print page to print out the document. Follow the

red arrows below:

Step 2: Fill out the form

Fill out the form with a black pen.

Any question for which you don’t have the answer, please write either ‘N/A’ (for not applicable) or ‘I

don’t know’.

Step 3: Return the form

You can choose one of the following options to return your form.

(a) scan the form in pdf format, then email it back to booking@yapspecialist.com.au as an

attachment

(b) fax the form to us at 08 8297 5823

(c) mail the form to PO box 225, Edwardstown, SA 5039

(d) hand in the form to our office at 680 South Road, Glandore SA 5037 during office hour

Step 4: Confirmation of your appointment

Once we receive your filled-out questionnaire form, we will contact you by email or phone to

confirm your appointment date and time which you have tentatively booked with us.

PDF Fill & Sign

Step 1: Open the file

Open the Pdf document using Adobe Acrobat Reader DC.

If you don’t have the latest version, you can download it online

Step 2: Fill out the form

On the right panel, click on this

Then, on the top panel, you will see this row of icons, and choose one of them

Step 3: Return the form

You can choose one of the following options to return your form.

(a) save the form in pdf format, then email it back to booking@yapspecialist.com.au as an

attachment

(b) print & fax the form to us at 08 8297 5823

(c) print & mail the form to PO box 225, Edwardstown, SA 5039

(d) hand in the form to our office at 680 South Road, Glandore SA 5037 during office hour

Microsoft Words

Step 1: Open the file

Ensure you have Microsoft Word installed, preferably 2013 or later. If your version is 2003 or earlier,

you may need to install the latest updates at http://support.microsoft.com/kb/2460049 .

Enable your Microsoft Word to open protected file downloaded from the internet or email. This is an

important step you should complete before you open the downloaded form.

Open the Microsoft Word program by clicking

Go to File > Options > Trust Center > Trust Centre settings > Protected view to uncheck the 3 option

boxes. Follow the diagrams below:

Then click OK to return to the main page.

You may download the most appropriate Questionnaire form from the Website

http://www.yapspecialist.com.au . The Form page is located under Contact tab > Form

Step 2: Fill out & Save the form

Once you have successfully open the questionnaire form, you can start filling out the form

electronically.

Always keep the file as docx format and remember to save it regularly.

Step 3: Return the form

Once you have completed the form, choose one of the following options to return your form:

(a) save the form in docx format, then email it back to booking@yapspecialist.com.au as an

attachment

(b) print & fax the form to us at 08 8297 5823

(c) print & mail the form to PO box 225, Edwardstown, SA 5039

(d) hand in the form to our office at 680 South Road, Glandore SA 5037 during office hour

If you choose to return the form by email, please put your name on the email subject title, and

remember to attach the form to your email before sending the email.
