All parts of this document apply to me and each of the children or legal wards listed above. [We will collectively call ourselves "I," "me," or "my" throughout this document.]

IT IS AGREED AS FOLLOWS:

1. Consideration. I have voluntarily requested to engage in any or all of The Activities and I am signing this document inconsideration for being allowed to engage in any or all of The Activities now and in the future. I am NOT relying on PHBA to list all possible equine-related risks in this document orany time, now or in the future.I understand that anyone riding, driving, handling, working with, or even near an equine at any location can suffer bodily and other injuries. Among other things, equines are unpredictable by nature. For example, when frightened, angry, or under stress, the natural instincts of an equine are to jump forward or sideways, back up quickly, or run away from real or perceived danger by trotting or galloping. Equines also have the ability to kick, buck, rear up, spin around, strike, or bite. I know that equines can do these and other things without warning. and dangers that are inherent in equine and livestock-related activities, and I agree to assu,ne all of them. I also INITIAL HERE: ___ 3. Assumption of Risk and Waiver of Liability relating to

Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health

Organization. COVID-19 is extremely contagious and is believe to spread mainly from person-to-person

contact. As a results, state and local governments and federal and state health agencies recommend social

distancing and have, in many locations, prohibited the congregation of groups of people.

Palomino Horse Breeders of America (“the Association”) has put in place preventative measures to

reduce the spread of COVID-19; however the Association cannot guarantee that you or your minor

child(ren) will not become infected with COVID-19. Further, attending the PHBA World Championship

Show could increase your risk and your minor child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily

assume the risk that my minor child(ren) and I may be exposed to or infected by COVID-19 by attending

the PHBA World Championship Show and that such exposure or infection may result in personal injury,

illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by

COVID-19 at the PHBA World Championship Show may result from the actions, omissions, or negligence

of myself and others, including, but not limited to, Associations employees, volunteers, and other

attendees and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury

to my minor child(ren) or myself including, but not limited to, personal injury, disability, and death, illness,

damage, loss, claim, liability, or expense of any kind that I or my minor child(ren) may experience or incur

in connection with my minor child(ren)’s attending at the PHBA World Championship Show or

participation at the PHBA World Championship Show (”Claims”). On my behalf, and on behalf of my minor

children, I release, covenant not to sue, discharge, and hold harmless the Association and the Illinois State

Fairgrounds and/or their directors, officers, employees, agents, and representatives, of and from the

Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or

relating thereto. I understand and agree that this release includes any Claims based on the actions,

omissions, or negligence of the Association and the Illinois State Fairgrounds, and/or their directors,

officers, employees, agents and representatives, whether a COVID-19 infection occurs before, during or

after participation at the PHBA World Championship Show or event held at the World Show.
