RX for Senior Health

Caring for an aging loved one can pose many challenges. One of these challenges is all too common and described as Polypharmacy. It’s a growing phenomenon in our elderly population and avoidable if caregivers see a sign of the times.

What is polypharmacy? Polypharmacy is the use of multiple medications by a patient, generally older adults (those aged over 65 years). More specifically, it is often defined as use of five (5) or more regular medications. It sometimes alternatively refers to purportedly excessive or unnecessary prescriptions...Polypharmacy is most common in the elderly, affecting about 40% of older adults living in their own homes. (sourced from Wikipedia)

According to Peterson Home Care staff, a patient may take between 8-13 medications on a daily basis. Patients and family members seek help in managing a loved one’s medications in the home. Concerns about numerous medications include adverse drug reactions and the mismanagement of medications which can ultimately be the cause of unnecessary hospitalizations in the elderly.

Peterson Home Care offers tips to help families spot the signs of mismanagement of prescribed medications? Some obvious signs may include:

• Falls• Cognitive changes• Missing medications• Frequent refill attempts

Under our supervision, Peterson Home Care can serve as the starting point in determining what type of medications mismanagement your loved one is experiencing. With a physician’s order/referral, Peterson Home Care’s skilled nurses can review medication regiments and create a set schedule of reminders. Peterson’s skilled nurses can teach each patient and caregiver how to use medications, manage or identify side effects, outline proper dosing requirements as determined by the ordering physician.

Medication management through Peterson home health can prevent unnecessary hospitalizations, assisting in communication with your physician, as well as educate the entire family on the disease management.

Understanding your medications is the first step in maintaining independence in the home. Peterson Home Health offers all the services you or your loved one needs in maintaining that independence and functional ability.

To learn more about managing your medications or for someone you love, contact Peterson Home Care at (830) 258-7400 or visit the website at www.petersonrmc.com. Select Home Health.
