HOURS OF OPERATION Our regular hours of operation are Monday through Friday, 8 a.m. to 4 p.m. We are closed for lunch from 11:15 to 12:15 p.m. For emergency after-hours care, the on-call physician can be reached at (317) 745-6160. A physician is always available after-hours for urgent needs. If your call is not answered within 30 minutes, please call back. Routine questions and prescription refills should be handled during regular office hours. APPOINTMENTS Appointments can be made by calling us at (317) 718-4000. To best meet your health care needs, we ask that patients arrive on time for scheduled appointments. If you are going to be more than 15 minutes late, please call the office as we may need you to reschedule. If you need to cancel an appointment, please do so no less than 24 hours in advance of your appointment time. Although we make every effort to see you at scheduled time, emergencies may sometimes delay us. In such cases, we will attempt to contact you as early as possible to notify you of the delay or to reschedule your appointment. If you are a new patient, please arrive 15 minutes before your scheduled appointment. This will allow time for you to complete any necessary paperwork. WHO SHOULD SEEK CARE

We specialize in treating patients with diseasesof the chest and lungs, such as emphysema, asthma, pneumonia, lung cancer and respiratory failure. Dr. DeWeese has special training in sleep disorders and often treats patients who su� er from insomnia, sleep apnea, loud snoring, di� culty falling/staying asleep, nighttime or morning headaches, nightmares or hallucinations when falling asleep, and restless leg syndrome.

SLEEP DISORDERS CENTER

Sleeping well is important to good health. Sleep disorders have been linked to depression, substance abuse, weight gain, diabetes, heart disease, and problems with learning and memory. When sleep problems a� ect your everyday life, it’s time to takea close look at your symptoms. With a complete and accurate diagnosis, most sleep disorders can bee� ectively treated or managed.

The Hendricks Regional Health Sleep Disorders Center o� ers a full-service department led by board-certifi ed sleep medicine specialists who diagnose and treat a wide variety of sleep disorders. Our Center is accredited by the American Academy of

Sleep Medicine and o� ers several di� erent types of sleep studies to achieve an accurate diagnosis. The standards we have met, and exceeded, to receive accreditation ensure that you will receive the highest quality medical care related to sleep or daytime alertness. TEST RESULTS

Our sta� will make every e� ort to get lab, radiologyand pathology results to you as soon as possible. Please keep in mind that pathology reports routinely take two weeks, and other results are not typically available in our o� ce for two to three days.

PRESCRIPTIONS AND RENEWALS

We want our patients to take an active role as a part of the health care team. One way to do this is by keeping an accurate record of the prescriptions and over-the-counter medications you are taking. Be sure to take your medication record to every appointment to update your physician. Medication record forms are available by request in our o� ce or online at HENDRICKS.ORG/ MEDICATION-RECORD.

Our o� ce sta� will be happy to call in prescription refi lls to your pharmacy. We ask that you allow us 24 hours

to call the pharmacy. For a prescription refi ll, call our o� ce at (317) 718-4000. Please have the phone number and address of your pharmacy on hand.

HOSPITAL ADMISSIONS

Patients of Hendricks Pulmonary & Sleep Medicinethat require hospitalization will be overseen by Dr. DeWeese and board-certifi ed hospitalists (doctors who exclusively provide inpatient care) at Hendricks Regional Health in Danville.

OUR PHYSICIAN

Eric DeWeese, MDDr. DeWeese is a fellowship-trained graduate of the Indiana University School of Medicine. He is board-certifi ed in Pulmonary Medicine, Critical Care Medicine, Sleep Medicine and InternalMedicine. He is the medical director

for the Hendricks Regional Health Sleep Disorders Center and often treats patients for sleep disorders. He is a member of the American College of Chest Physicians, the American Thoracic Society, and the American Association of Sleep Medicine.
