    The ubiquitous buzzword in todays culture instressand, unfortunately, it is not going anywhere. Thequestion is how to deal with it and keep it in check.

    Stress is a fact of life, but being stressed out is not.We dont always have control over what happens to us,says Allen Elkin, Ph.D., director of the Stress ManagementCounseling Center in New York City, in a Readers Digestarticle. Yet that doesnt mean we have to react to a dif-ficult, challenging situation by becoming frazzled or feel-ing overwhelmed or distraught. Being overly anxious isnot just a mental hazard; its a physical one too. The morestressed out we are the more vulnerable we are to colds,

    flu, and a host of chronic or life-threatening illnesses. Andthe less open we are to the beauty and pleasure of life.Donna Bennett is a recreational therapist in the inpa-

    tient unit in Mental Health Services at Froedtert & theMedical College of Wisconsin Community MemorialHospital. She has spent 22 years in the field of mentalhealth and she has a lot to say on the subject of stress.

    Stress is growing (among people in todays socie-ty), she said in a phone interview with the Express News.We want to do more, there are more options on thethings we can do and we never relax anymore. We arealways multi-tasking.

    Multi-tasking, however, is by its very definition aflawed approach, Bennett pointed out.

    Research shows that multi-tasking is not really effec-tive. You are not in the moment attending to the task. Youare more forgetful and it takes longer to accomplish thetask, she said.

    Dear Dr. Winter, I need implants in my upper and lowerarches. I have had several consultations and opinions all overthe board! Some of the consults said I only needed 4 implantsfor the upper and 4 for the lower and others have said I need

    just two for the lower. I did tell them I wanted to be able tohave teeth that didnt come out and I dont have a lot of bonein either arch. What are your thoughts? Thanks, Jack J.

    Dear Jack, there are several good questions here. I will tryand break them down and I encourage you and the readers toread my articles on Upgradeable Dentistry and AdvancedTreatment Planning under publications at:

    www.hamptondentalas sociates. com where I have writtenarticles about these very questions. I also outline in variouspublications the differences between removable overdenturesand fixed implant bridges and show examples of each type ofimplant treatment.

    First of all, without a consultation and x-rays I cannotcomment on your specific situation. The number of implantsyou require is dependent upon the quality and quantity ofyour remaining bone, your budget and the type of prosthesisyou require. Whether the implant restoration should befixed or non-removable by the patient or whether it isacceptable to remove the implant supported denture and takeit out at night is a personal decision and both have advantagesand disadvantages. Most people that lose their teeth wantfixed teeth. In addition, the patients health, medication(s)and anatomy all play a role in deciding upon the ideal implantrestoration. It is important to note that sometimes we can startwith a removable implant denture due to budgetaryconstraints and add implants over a year or two and Upgradeto a fixed restoration or bridge when finances allow!

    Next, we can discuss the number of required implants. Inthe upper arch most people have already been wearing adenture or partial and they have already undergone bone loss.As bone is lost in the upper jaw the sinuses expand and in

    order to place implants in this area there is a need to growthe bone by doing a sinus grafting procedure and possibly ablock grafting procedure. This sinus augmentation is aprocedure that will allow implants of normal length to beplaced in the posterior upper jaw. A way to bypass this is azygomatic or long implant that is used in an all-on-fourprocedure. This procedure will place 4 implants at angles thatare used to support a full arch of denture teeth or a hybridprosthesis. This may be a predictable procedure but replacing14 teeth (24 roots) with 4 implants doesnt provide as large amargin of safety as using 7-9 implants per arch. This is amatter of philosophy and the treating implant dentist andsurgeon will be able to explain their philosophy based upon

    their education, skill and judgment.As far as the two-implant option given to yo

    lower arch, this is strictly to support a lower ovwhich is removable. In order to have a fixed (non-removable) you would require a minimmaxillary and 5 mandibular implants in my pracbeing said if you are able to place more implantsengineer the situation you are always better offextra implants will redistribute chewing forceimplants, instead of just four.

Thebig stressors are exactly what one wouldthinkfamily, work, communication issues inboth of these areas, finances, she listed.There is also the worry of how to get thingsdone with both the husband and wife workingand the kids in various activities.

    Make a plan: Once you know what willtrigger your stress, be proactive in dealing with

    it, said Bennett. Make a plan for how to cope.Look at what your options are and have a strat-egy in place, she said.

    Engage in physical activity: One way tohandle stress better is to literally work it out. Dosomething physical, Bennett recommended.You dont have to go running, but do some-thinggo for a walk, stretch, let some energyout.

    Quiet your mind: Two easy ways toaccomplish this are reading and meditating.Meditationcalming the mind, focusing ondeep, slow breaths and counting to five foreach inhale and exhaleis very helpful whendone right, said Bennett. "You can do it any-time, anywhere you need to. It is not somethingwe are used to doing, but it really helps. Whena person is stressed they tend to take shallow,what I call anxious breaths in the upper partof the chest.

    Make a spiritual connection: People withno spiritual base tend to more stressed out,according to the experts. It doesnt have to bea specific religion or church, or even a churchat all, explained Bennett. Just having a con-

    nection to nature can help. Cognitively engage: The mental health

    professional urges individuals to use theirbrain. Take part in an activity in which youhave to focus to be successfula puzzle, aword search Sudoku, she said. Even someelectronic games work for this.

    However, sometimes its necessary to... Pull the plug: Take a break from elec-

    tronics, especially at night, said Bennett. Theystimulate your brain and it is hard to fall asleep

    which contributes to stress. Have a support system: You need yourpeople, Bennett stated simply. You cantmake it on this earth without your people. Youneed your family and friends and if you donthave friends, make some through volunteeringand taking recreation classes and things likethat.

    Most of the time stress can be handle in-house, but sometimes the situation is more direand in those cases...

    Make a call: There are hotlines andwarmlines available in each county, Bennettsaid. Hotlines are for when you are in crisisand a warmline is for when you just need to talkabout your stressors.

    Too many people sweep their stress underthe carpet and hope it will vanish. It will not,stated Bennett.

    If you dont do anything and you're not

    willing to make changes in your lifestyle andbehaviors, things are not going to get better,she said. There are so many effects ofstress...concentration problems, cardiac issues,back and neck pain...theres even correlation to

    cancer. At a certain point our body fights backand says, Notice me! Notice me!

    It is all within our power to make thechange, Bennett said.

    We make choices, she said. Do wehave to have our kids in every (activity)? Whatmessage are we sending to our kids when welive like this? Everything is a choice.
