the highest-quality care. Safety is the foundation of quality care. Each of us, in our

own way, plays a vital part in keeping patients safe. Our leadership team plays a

critical role by providing and communicating a clear vision and making sure we have

the knowledge, resources and systems necessary for peak performance. Each of us

shares that vision and embraces it as our own. By making patient safety the focus

of our individual roles, we create a hospital-wide culture of safety that is manifested

in everything we do, every single day. The result is an entire hospital community

lives—depends on properly functioning medical devices. I help protect patients by

making sure that all clinical infusion devices are in excellent working order. I inspect

the devices, perform regular preventive maintenance and handle any repairs necessary.

Part of my job is providing technical expertise to clinical staff and other technicians

and educating them in proper use of this sophisticated equipment. Today, more than

ever, medical devices are a vital component of good, safe patient care.

care environment is safe. One of the ways Engineering ensures a safe environment

is through weekly rounds. I go to one of the nursing units, where a nurse and I do

a detailed inspection of the physical environment. If I find anything that could

be a safety hazard I address it right away. The nurses know they can count on me

to get things done. Helping people is my thing. That’s why I’m also a palliative care

volunteer here at the hospital.

to clean rooms and beds between patients, and I clean as if the next patient in that

room was going to be my mom. I put on gloves before I go in, strip the room, wash

everything down and set it up for the next patient. Before I leave the room, I take off

my gloves so germs don’t spread, then I wash my hands with Purell®. I’ve been at

Hartford Hospital for 30 years, and I sometimes help train new people. I work

alongside them as they learn to properly and completely clean a room and get it

ready for the next patient.

take my work seriously, because

good nutrition is essential to the healing process. I inspect deliveries to make sure

foods are wholesome and fresh on arrival, then I work closely with our staff of 35 to

see that food is handled safely and prepared according to exacting standards. We’re

always testing new recipes. And we sample test trays to ensure that patients receive

high-quality meals. Our kitchen serves roughly 5,800 meals a day, and I want every

one of them to be both healthful and appealing.

’ve been here more than 40 years,

and I love my job. I work with the clinical departments to make sure they have the

equipment and supplies they need to care for patients and keep them safe. We

research and try out products, stock approximately 2,000 items in our storeroom,

and keep all the units supplied. That patient on the unit—it could be me, you or a

family member. And that patient is depending on me to do my job well so they’re

protected and can get the treatment they need.

meant to do. I love it. And every aspect of nursing involves patient safety—infection

control, medication administration, documentation and more. I initiated an allergy

bracelet program here. If a patient being admitted has any medication or food

allergies, we create a red bracelet with the allergies listed right on it. The patient

wears it throughout his stay. The allergy information is also in the electronic record,

but the bracelet is an added check that further protects the patient—and protecting

the patient is our top priority.

want to be treated. If a patient is at risk of falling, I make sure bed and chair alarms are

on and that the patient has everything within reach. I also check on that patient more

often. When patients are on contact precautions to prevent the spread of infection,

I help visitors understand the precautions and make sure staff and visitors follow them.

Plus, I wash my hands all the time and remind others to do it, as well.

hen I’m called to lift a patient,

I ask the nurse about any special needs, so I can give the patient a safe lift. We’re

always thinking about keeping the patient safe during transport. I make sure that

the side rails on the stretcher are up or that the wheelchair wheels are locked befor

prevent pressure ulcers. I’m a people person—I try to give every patient the same

care I’d like for myself.

minimal side effects—that’s what we’re all about. Pharmacists review all medication

orders and double-check that the drug and the dosage are safe for that patient. If we

note any allergies or drug interactions, we’ll recommend an alternative medication.

If the patient’s kidney or liver function isn’t normal, we’ll suggest a different dose. With

some medications—antibiotics and blood thinners, for example—dosing is especially

crucial, so prescribers often work with us to identify the best dose and arrange for

proper monitoring.

today’s fast-paced environment, we physicians have to consciously slow down, focus

intently on the task at hand, and listen. We have to listen carefully to our patients

and consider what their complaints may indicate. We have to stop and listen to our

colleagues—nurses, pharmacists and others—when they raise concerns about a

medication or procedure. Historically, people were reluctant to question doctors.

I’m glad that, with the culture of safety spreading through the hospital, people feel

free to speak up if they have concerns.

Ensuring patient safety is a challengein today’s health care environment. Twenty-first century medicine is complicated. Hospitalized patients are sicker than ever. Manyhave multiple illnesses and conditions requiring complex care.Recognizing this challenge, Hartford Hospital has launched a bold,comprehensive and transformative initiative to ensure the safety of every patient now and in the future. We are committed tobecoming the best in the country for patient safety, and we arepursuing that goal by taking innovative action and creating a culture of safety at every level of our organization.

P A R T N E R I N G F O R P A T I E N T S A F E T Y

The core of the hospital’s patient-safety initiative is the Patient SafetyAction Group. Launched in January 2008, PSAG (commonly called“PEA-sag”) is a multidisciplinary group comprising directors andmanagers from all areas of the hospital. Jamie Roche, MD, VicePresident for Patient Safety and Quality, and Linda Spivack, RN,MSN, Vice President of Patient Care Services, co-chair the group.

PSAG meets every weekday from 7:30 to 9 a.m. Its roughly 30participants include nurses, doctors, administrators, environmentalservices staff, biomedical engineers, pharmacists, purchasing officers

and others. People throughout the hospital are encouraged to bringsafety concerns to the attention of the group, and participantsdiscuss these concerns at the daily meeting.

As the name implies, PSAG focuses on taking fast action toresolve potential safety hazards. Because every department is represented at the meeting, solutions to many problems can bedecided upon on the spot, eliminating risks almost as soon asthey’re identified. Smaller action groups are deployed for issuesrequiring additional or ongoing work, and action groups reportback at subsequent meetings.

This year we took a variety of actions toward our goals. Weinstituted a hospital-wide patient-safety and quality-improvementinitiative. We proposed to the University of Connecticut HealthCenter a partnership that would strengthen medical education andresearch in the state. We took steps to better integrate the otherhospitals and health care providers in our regional network in orderto provide patients with more seamless continuity of care.

The Board of Directors plays a key role in moving HartfordHospital forward. Recognizing that advancement demands investment, the Board this year authorized expenditures to accelerate progress toward a completely electronic medical record,acquire our third da Vinci® Surgical System, and purchase a newhelicopter and helipad for our LIFE STAR emergency air transportsystem, the only program of its kind in the state.

All of us who are members of the Hartford Hospital communitytoday have inherited a proud tradition.
