LIST OF DOCUMENTS FOR ADOPTION DEPARTMENT 

- Application for registration of candidates for adoption, certified by a notary; - Certificate of employment and salary for the last six months, or a copy of income tax returns for the previous calendar year, certificated by authority that

issued it or a notary; - Notarized copy of marriage certificate registered by the competent authority (in duplicate);- Health Certificate of each applicant; - Notarized written consent to the adoption of the other spouse (if the child is adopted by one of the spouse), - A certificate of criminal record for each applicant issued by the competent authority in the country of residence; - Approval of the competent authority of country of residence that confirms the possibility of applicants to be adoptive parents. If the opinion issued by

non-governmental body, it must include a copy of the license of this agency for activities related to the adoption; (in duplicate); - Permission of the competent authority of residence country to adopted child to enter and reside permanently, - Copy of passport or other document identifying the person, in triplicate; - Notarized copy of document certifying the ownership or use of residential premises, indicating the total and living area and number of bedrooms; - Notarized obligation of the applicant's (in duplicate):

- put a child registered in the Consulate or diplomatic mission of Ukraine (indicating name of institution, establishment, its address) within one month after entry into the country of residence); - ensure the right of the adopted child to keep the citizenship of Ukraine to the age of 18; - provide consular post or diplomatic mission in Ukraine at least once a year during the first three years after adoption and subsequently every three years until the child reaches eighteen years a report on living conditions and health of the adopted child; - provide an opportunity for the representative of the consular post or diplomatic mission of Ukraine to communicate with the child;

- Inform the consular post or diplomatic mission of Ukraine on change of residence of the adopted child. - Notarized written consent of the applicant who is a foreigner, to obtain information about him at the General Secretariat of Interpol and law enforcement

agencies of the state of nationality of the foreigner, and the State in whose territory he resides (in duplicate).
