How to Access, Save & Submit Grant Applications

• Enter your Username and Password to login

• Once you are logged in, you will be taken to the application.

• The Community Health Grant Application has three tabs:

• Tab 1: Application• Tab 2: Attachments• Tab 3: Review & Submit

Fill in the following sections:

• Organization Information

• Executive Director or CEO Contact Information

Note: Fields in RED are required.

• Fill in the following fields:• Organization

Information• Project Information

• Click “Save”

Review the information you entered, select:

• “Continue” to move onto the next tab, “Attachments.”

OR• “Edit” to change the

information.

• The “ Attachments” tab has four templates that are required for your application and shown in red.

• Additional information may be uploaded in the “Attachment (optional)” section.

• See the next slide for instructions on how attach templates or additional information to your applications.

• Once all Attachments are uploaded, click “Continue.”

For Attachments that require the use of templates:

• Click on the hyperlink to view the template.

For Attachments that require use of templates:

• On the bottom left of the screen click “Download."

• Once downloaded, click to open the document and save it to the desired location.

• Fill out the template and save to your computer.

For Attachments that require the use of templates:

• To return to your application, click on the “Greater Rochester” tab at the top of the screen.

For Attachments that require the use of templates:

• Once completed, go back to the Attachments page and click ”Select/browse.”

For Attachments that require the use of templates:

• Search your files to select the completed template file.

• Double click on the name of the file.

For Attachments that require the use of templates:

• The file name will be above the upload button.

• Click “Upload”.• If you uploaded the wrong

file you can fix that by clicking on the “Replace or Remove” link in the lower left corner to try again.

• Review your application. Once you submit your application, you will not be able to change it.

•“Submit” or “Print’’ your Grant Application.

•Once your Grant Application is submitted, you will receive a confirmation email.

• Applications that have not been completed or have been submitted will be saved under “My Submissions” for your registered user account.
