1.GeneralDescriptionoftheDollanditsUsesTheNurseTrainingDollhasbeendevelopedasateachingmodel,especiallyfortrainingnurses.Thereisawiderangeofexercisespossibleincludingbothfirstaidandemergencysituations,andstandardnursingpractices.Themannequinisconstructedtosimulateasnearlyaspossiblethenormalpositionsandmove-mentsofthehumanbody.Thedollhasbeendevelopedwiththehelpofnursingteachers.Themannequinismadeofaveryresistantplasticandcarefullyselected,highlyresistantnonrustingmetallicunionsusedbothforthejointsandforthemoveablelinksinthetrunkandbetweenthetrunkandhead.Thisallowsthemannequintobeeasilycleanedandhighlyresistanttoanydamage.Themodelisconstructedtomakemovementsconformverycloselytothoseofthehumanpatient.Thedollwillsitinabedwithoutspecialsupports.Alllimbscanbemovedinwaysthatconformtothenormalmovementsofthehuman.Theheadandjawsalsomovenaturally.

Becauseofcarefulselectionofdurablematerialforthemanufactureofthedollitisnearlyimpossibletodamageitevenwiththemostadversetreatment.

Thehandsandfeetofthedollareconstructedofsoftplasticwiththefingersandtoesseperatedsothatcleaningtechniquescanbepracticedinanaturalway.Thescalpisalsoofsoftplasticbutcoversaharderplasticskulltobettersimulatethenaturalcondition.

Thestandarddollhasnoinnerorgansandisintendedforbasiccareprocedures.Thespecialdollhastheusualinternalorganssothatinadditiontothebasiccareproceduresawiderangeofadditionaltreatmentprocedurescanbepracticed.

Thedollmeasuresabout174cm(68.5“).Thespecialdollsweighsabout15.5kgs(34Ibs)andthestandarddollweighsabout14kgs(31Ibs).

Thetrunkofthedollismadeintwopartswhichcanbeseparated:a)anupperpartwhichcontainstheheart,lungs,andstomachinthespecialdoll.b)alowerpartwiththeexternalsexorgans.Thespecialdollincludestheorgansoftheabdomenand

pelvisincludingtheintestine,thebladder,andtheinternalsexorgans.

Thepartsofthetrunkarejointedatthewaistbyamoveableunionthatshouldnotbeloosenedunlessapartofthetrunkmustbereplaced.Thepairedupperandlowerextremitiesareattachedtothetrunkbyeasilyhandledunionsandcanbedetachedeasily.Thearmandforearmandthethighandlowerlegareconnectedbysimilarjoints.

Theheadisfixedtothetrunkinasimilarway.Boththelowerandupperjawsprovidedwithpartialden-turesthatcanberemovedandreplaced.Everydollhasanexchangeablemaleandfemalegenitalinsert.Thedollhasinjectionpadsinthearms,thighs,andinthebuttocks.

The3B-NurseTrainingDollTherectum,partoftheintestine,theurethraandbladder,theesophagus,andthestomacharelinkedbyspeciallydesignedunionsthatreleasewhenturnedclockwise.Theseunionsareverysecureandtheyareeasilyputtogether.Theinternalorganshavethefollowingcapacities:

1.Intestineabout1.200ml2.Bladder220ml3.Stomach360ml4.Lungs1.800mlEachdollhasaserialnumberinsideontheuppersectionofthebackandshouldbereferredtowhenorde-ringextrapartsorincaseofproblemswiththedoll.

2.AssemblyForshipment,theNurseTrainingDollisdisassembledandpackedinastrongcorrugatedbox.Thedolliseasilyassemblediftheinstructionsbelowarefollowedcarefully.Notoolsareneeded.Allpartsofthedollshouldbeunpackedandarrangedasshownonthepicturebelow:

Thefemaleinsertisinplaceinthedoll.Thetwopiecetrunkofthespecialdollcontainstheinternal,organs.Theseshouldberemovedfirsttosimplifytheattachmentofthelimbs.

2a.RemovaloftheInnerOrgans(SpecialDoll)andtheGenitalInsertPlacethetrunkonitsback.Takeoffthechestcoverandthenremovetheheart,stomach,andlungs.Removetheabdominalcoverexposingthebladder,partoftheintestinesandtheinternalgenitalorgans.Thebladderisconnectedtotheurethrabyaunionthatcanbeloosenedbyturningittotheleft.Thenpushasidetheuterusandovariesandunfastentheunionconnectingthelowerbowelandrectumbyturningcounterclockwise.

Afterremovingthispartoftheintestine,thefastenersofthefemalegenitalinsertmustbereleased.Pushasidethevaginaandrectumtoexposeaplasticbandthatisstretchedoverascrewheadinthemiddleofthesacralbone.Thisunionisreleasedbyremovingthestrapfromthescrewhead.Ashorterstrapconnectsthegenitalinserttothepubicboneandpartofthetrunk.Afterreleasingthesecondstrapthegenitalinsertmayberemoved.Inthecaseofthespecialdoll,theurethra,vagina,uterus,ovaries,andrectummayalsoberemoved.

2b.AssemblyAllboltsandnutsareinplaceonthemodel.Asthemodelisassembled,theboltsshouldberemovedandthenreplacedasassemblyiscompletedtoavoidanyerrors.

The3B-NurseTrainingDoll2h.InstallationoftheGenitalInsertEachofthegenitalinsertsisprovidedwithashortplasticstrapaboveandalongeronebelowwithwhichtheinsertcanbefastenedinplace.Placethegenitalinsertattheopeninginthepelvisandpushitinplace.Movetheshorterplasticstrapthroughtheslotatthepubisuntilthetopofthegenitalorgansfitproperly.Fastenthestraptothescrewheadabovetheslotinsidethebodycavity.Thenpushthelowerpartofthegenitalinsertfromtheoutsideintotheopeninguntilitfitsinplace.Finally,pullthelongerplasticstrapupinsidethemodelandfastenittothescrewheadinthemiddleofthesacralboneintheabdominalcavity.

2i.InstallationoftheInternalOrgans(SpecialDoll)Putthelowerintestineinplaceinstertingitsboltintotherectum(fixedtothegenitalinsert).Puttheboltinplaceandtighten.Insertthebladderbysrewingitintotheurethra.

The3B-NurseTrainingDoll2k.InstallationoftheAbdominalCoverTheabdominalcover(withanostomysite)maynowbeputinplace.Inthecaseofthespecialdoll,theostomysite(whichhasnoconnectiontotheintestine)mustbefastenedtotheinsideoftheabdominalcover.Thenursingdollisnowreadyforuse.

3.MaintenanceTheNurseTrainingDollisdesignedtofacilitatecleaningandmaintenanceprocedures.Regularcleaningandcarewillprolongtheusefullifeofthedoll.Allpartsofthedollaremadeofplasticsoallsurfacesmaybecleanedwithwaterandifnecessaryamildsoapsolution.Afterusingsoap,themodelshouldberinsedthoroughlywithcleanwater.Alwaystakecarethatallpartsarewipeddryandthenarelefttodryintheair.Thenthedollmaybereassembled.Asthedollisreassembleditissuggestedthatallarticulationsandareaswhereplasticsurfacesrubagainstoneanother,besprayedwiththeincludedlubricatingspray.Moderatesprayingwiththesamesprayissug-gestedtomaintaintheinnersurfacesofthenoseandthroat.Treatthebuiltinvalveswithvaselineatregularintervals.Whenthebladderisremovedonecanseeavalvemadeofasyntheticcloth;thisshouldbeimpregnatedwithvaseline.Thevalvesattheanusandrectumshouldalsobeimpregnatedwithvaselineperiodically.Occasionalbitsofpiasteronthesurfaceofthemodelcanberemovedwithgasolineforcleaningpurposes.Marksonthedollshouldbemadewithapen-cil.Theinksofballpointpansandfiberpointpenscannotberemoved.Onlywatershouldbeusedforexerciseswiththedoll.Otherfluidsmaydamagetheplastic.Avoidtheappli-cationofanyothermaterials.Aftereveryexerciseallremnantsofliquidsmustberemoved.Fordrainage,openingshavebeenprovidedinthehead(Iowerside),thelungs,theintestines,andthebladdersothattheseorgansmaybethoroughlydrained.Thestomachcanbeemptiedthroughtherelativelylargeaper-tureattheesophagealend.Usingthelubricatingsprayontheboltsatthearticulationsoccasionallyprovi-desproperlubrication.Specialproceduresforthemaintenanceofspecificpartsofthedollwillbefoundwiththeappropriateexercises.II.USESOFTHETRAININGDOLLThemajorexercisespossiblewiththenursetrainingdollarepresented.

A.BasicPatientCareFortheseriouslyillorhelplesspatient,nursesmustperformdailybodycare.Thenursingdollmaybeusedtopracticeproceduresforfastandthoroughbodycare.Inaddition,procedureswhichmakeiteasierforthenursetolift,carry,andputthepatienttobedmaybepracticed.Thestandardtrainingdoll(withoutinternalorgans)issufficientforpracticingallproceduresthatareconcernedwithbasicpatientcare.

I.Lifting,CarryingandMovingthePatientToavoidphysicalstrainwhenmovingpatients,nursesshouldlearnthepropertechniquesofpatienthand-ling.Suchtechniquescanbepracticedwiththetrainingdoll.

Allproceduresformovingapatientandliftinghimoutofthebedcanbepracticed.Also,proceduresforcarryingapatientandmovinghimintodifferentpositionscanbelearned.Assistingthepatientingettingupfromachairorsittingupinbedcanbepracticedsincethedollwillholdthesittingpositionwithoutanysupport.

2.BedCareofthePatientAlltechniqueswhichinvolvehelpingthepatientintobed,helpinghimchangeposition,aswellashelpingthepatientdressandundresscanbepracticedwiththetrainingdoll.

3.MovingthePatientAlltechniquesformovingthepatientmaybedemonstratedwiththetrainingdoll.

The3B-NurseTrainingDoll4.BathingthePatient(GenitalCareofMaleandFemale)Alloftheproceduresforbathingthebed-riddenpatientcanbepracticed.Theexchangeablemaleandfemalegenitalinsertsmakeitpossibletopracticegenitalcareforbothmenandwomen.

5.CareandCleaningoftheMouthCleaningthemouthandteethcanbedemonstrated.Itisalsopossibletodemonstratethecareofdenturessincethedollhasremovablepartialupperandlowerdentures.

6.CareandCleaningoftheEyesProceduresforcleaningtheeyescanbedemonstratedonthedollbecausetheeyesareinsertedintosocketsandtheeyelidsaremadeofsoftplastic.

7.CareandCleaningoftheEarsThedollhascarefullysimulatedexternalears;theearcanalisabout2cmlongandisclosedontheinnerend.Thisallowsnormalwashingofthewearcanalwithwater.

8.CareandCleaningoftheNoseAconnectionbetweenthepharynxandthenasalcavitywiththenostrilopeningsmakesitpossibletofol-lownormalcleaningproceduresclosely.GeneralMaintenanceAftereachoftheaboveprocedures(especiallywhenwaterisused)thedollmustbecleanedwithmildsoapsolutionandthenrinsedwithcleanwater.Allpartsmustbewipeddryandthenarelefttodryintheair.Thepartsofthedollwhichwereusedshouldbetakenapartcompletelytoassurethatallpartsaredry(seehintsforassembly).Beforereassembling,allpartsthatmayrubtogethershouldbesprayedwithalubrica-tingspray.

B.Medicalcare

1.PreventiveTechniquesPracticewiththenursetrainingdollwillpreparethenursingstudentthoroughlyforcarryingoutawiderangeofpreventivetechniques.

1a.PreventionofBedsores(Decubitus)Withbedriddenpatientstheweightofthebodyonthetissuesofthebackandlegscaneventuallyleadtotheproductionofskinulcers(decubitus).Measurestakentopreventdecubituscanbedemonstratedwiththenursingdoll.Movingthepatienttoseveraldifferentpositions(prone,supine,ontheside,etc.)canbedemonstrated.Thebestmethodsformovingandturningthepatientcanalsobeshown.

1b.Prevention01MusclesCrampsVariouspreventivemeasurescanbedemonstrated.Thehipandkneejointscanbeextensed180degrees;thiscanbealternatedwithflexionoverakneeroll.Inthesupinepositiontheshoulderjointcanbemovedinto30and90degreeabduction.Itisalsopossibletoshowfullextensionand90dregreeflexionoftheelbowjoint.Fortreatingalegspasmtheinternalandexternalrotationofthefootcanbedemonstrated.lnadditiontreatmentforaseverefootcontractioncanbeshown.

1c.PreventionofPneumoniaPneumoniaisadangerouscomplicationthatcanbeprevented.Asalreadymentioned,turningthepatientandgivinghimarubdowncanbedemonstratedwiththetrainingdoll.Onlyclearwater(noalcoholorsimilarfluids)shouldbeusedforarubdown.

1e.PreventionofThrushandParotitisThemostimportanttechniquesformouthcarecanbepracticedwiththenursetrainingdoll.Forallsuchprocedures,onlywatershouldbeused,neveranymedications.Thetrainingdollhasremovabledenturesinboththeupperandlowerjawssothatpropercareofsuchprosthesescanbedemonstrated.

2.PhysiotherapyWhendemonstratingthetechniquesofheattherapythedollshouldnotbeexposedtotemperaturesabove25°C(77°F).Thereare,however,nolimitstodrycoldapplications.Forintubationexercisesoneshouldusealinseedpapasalubricant.Thismaterialcanberemovedeasilyandwillnotcausediscolorationordamagetothedoll.

MaintenanceAfterusingwaterforanyoftheaboveexercisesthedollshouldbecleanedanddriedcompletely(seeI.3.)

2a.InhalationTherapyInhalationtherapyandtheuseofvariousinstrumentssuchasaerosols,vaporizers,andhumidifiersmaybedemonstrated.Properpositioningofthepatient,theproperinstallationoftheinstruments,andthecorrectapplicationoffacemasksandmouthpiecesmayallbedemonstratedwiththetrainingmodel.Useofthevariousinstrumentsshouldbedemonstratedwithoutturningthemon.

2b.InhalationofOxygenThetrainingdollcanbeusedtoshowtheseveralmethodsofadministeringoxygen.Forexample,onemaylearntechniquesfortheuseofanoxygenmask,anasalcatheter,oranoxygententorchamber.

3.ResuscitationThetrainingdollprovidesanexcellentmeansforpracticingseveralresuscitationtechniques.

3a.MouthtoNosePreparationofthepatientformouthtonoseresuscitationcanbepracticedonthedoll(positioningthepatient,clearingtheairpassages,andremovalofthedentalprothesis).Thetechniqueofmouthtonoseresuscitationcanbeshownwiththedoll,evenshowingtheexpansionofthelungsifthetechniqueispro-perlyexecuted.Forthisexercisethestomachmustbeconnectedtotheesophagusandthedrainapertureofthelungmustbeclosed.Afterpracticingresuscitationtechniquesthemodelshouldbecleanedwithaskindisinfectant.

3b.MouthtoMouthFormouthtomouthrespirationthetechniquesarethesameasabove.Insertionofatubeforrespirationcanalsobedemonstrated.

3c.UseofaVentilationBagAlltechniquesnecessaryforartificialrespirationwithaventilationbagcanbepracticedonthetrainingdoll.

3d.OtherMethodsCareofatracheostomymayalsobedemonstrated.ltisalsopossibletodemonstratetheplacementofatrachealcatheteraswellassuctionevacuationofthetrachea.Beforebeginningtheseexercises,allprobes,cathetersandcannulaeshouldbelightlysprayedwithalubricatingspray.

The3B-NurseTrainingDoll4.BandagesThedollmaybeusedtoteachawiderangeofbandagingtechniquesonallpartsofthebody.Thespecialdollisequippedwithanamputationstumpinsertionfortherightthightobeusedtoteachbandagingtechniquesfortheamputee.Substancessuchasgypsumorzincglueshouldnotbeusedwiththemodel.Remainsofpiastercaneasilyberemovedwithgasolineforcleaningpurposes.

5.IrrigationsandLavagesExerciseswiththedolltoshowbodytemperatureirrigations(withorwithoutmedications)shouldbecar-riedoutusingonlywater.

5a.IrrigationoftheEyesTheeyelidsofthenursingdollareelasticsothatallmanipulationsinvolvedwiththeirrigationoftheeyescanbecarriedout.Itisalsopossibletoshowhowtoapplyophthalmicointmentsandhowtoputdropsintotheeyes.Onlywatershouldbeusedfortheseexercises.

5b.IrrigationoftheEarsTheexternalauditorypassageiscloselysimulatedinthedollthoughitisclosedattheinnerend.Irrigationoftheearaswellastheuseofmedicationscanbepracticed.Again,onlywatershouldbeused.Theaudito-rypassagemustbedriedthoroughlywithcottonaftertheseexercises.

5c.Gastric(Stomach)LavageForpracticingagastriclavageuseadisposableplasticstomachtube(forexamplesizeCH18).Beforeinser-tion,thetubeshouldbesprayedwithalubricatingspraytoprovideproperlubrication.Thethroatandmouthofthedollcanalsobesprayedasneeded.Thestomachhasacapacityofabout360mlandiscon-nectedtotheesophagusbymeansofascrewjoint.

5d.IntestinalIrrigationForthisexerciseoneshoulduseadisposableplastictube(forexamplesizeCH28).BeforetheintestinaltubeisinserteditmustbelubricatedwithvaselinesothatitwillpasseasiIythroughtheplasticvalvesoftherectum.Laythedollonitssideandusejustenoughliquidtofilltheintestine(capacityisabout1.200ml).Withtheproperamountofliquidtheintestinaltubecanberemovedandreplacedwithoutlosingtheliquid.Whenliquidrunsoutoftheanusandtheintestinaltubehasnotbeeninserted,thenthebluecoloredscrewcapontherectummustbeopenedtofillthevalvewiththeVaselineinjection(ca.2ml).Thetotalvolumeoftherectalvalveis5ml.

5e.IrrigationoftheBladderForirrigationofthebladderoneshoulduseadisposableplasticcatheter(forexamplesizeeH16).Beforeinsertionthecathetermustbelightlysprayedwithalubricatingspraytoguaranteegoodlubricationandtofacilitatethepassageofthecatheterthraughtheplasticvalveattheentranceofthebladder.Thebladderhasacapacityofabout220ml.Fortheseexercisesthefillingapertureofthebladdershouldbeclosed.Pleasenotethatthesoftcathetersmadeofrubberarenotapprapriatefortheseexercisessincetheyaretooflexibleanddonotpasseasilythroughtheplasticvalveofthebladder.Medicallubricationsandanesthe-ticsshouldnotbeusedwiththemodelbecausetheydrytheplasticandwilleventuallyaffecttheoperationofthedoll.Whendoingthisexercise,eitherthemaleorthefemaleinsertscanbeused(seeI.2.forassem-blingprocedures).Thevalveinthebladder,whichisjustinsidethepointofattachment,mustbetreatedbothbeforeandaftereveryexercisewiththelubricatingsprayorwithvaseline.

The3B-NurseTrainingDoll5f.IrrigationoftheVaginaAplastictube(about15cmlong)attachedtothefemalegenitalinsertrepresentsthevaginaofthedoll.Itconnectstotheuterusatitsinnerend.Thevaginadoesnothavevalvessoitispossiblethatsomefluidmayleakintotheattachedfallopiantubes.Theremainingwaterinthevaginamustberemovedthroughthescrewcapofthegenitalinsertaftereveryexercise.

MaintenanceAftertheseexercises,cleananddrythedollandallofitspartsbeforereassemblingit.Removealltracesofliquidframtheorgansused.Theopeningusedforfillingthebladderandintestinecanbeusedfordraina-ge.Thelastremainsofliquidcanberemovedfromtheorgansbyshakingitout.

6.EnemasEnemascanbeperformedonthetrainingdoll.Aplasticvalverepresentstheanalsphincteronthedoll.Thisvalvepreventstheleakageofliquidwhentheintestinaltubeisinsertedorafteritsremovalfromtheanus.Theintestineisattachedtotheinnerendoftherectum(capacity1.200ml).Use1.200mlofwaterforallexercises.ThisisrecommendedsothattherewillbenobackflowofliquidwhenthedollisIyingonitsside(see11.B.5d.).Thedisposablesyntheticintestinal-tubesinthesizeCH/FG28,whichhavetobegreasedwithVaselinebeforefilling,shouldbeusedforEnemas.Aftertheexerciseunscrewtheintestinefromtherectumandreleaseanyremainingliquid.Whenallpartsarecompletelydrytheymaybereplacedinthedoll.Manytypesofintestinalirrigationsmaybedemonstratedwiththenursetrainingdoll.Theadmini-strationofretentionenemasispossible.Onlywater(withoutanyaddedmedications)shouldbeusedintheexercise.

7.CatheterizationThenursetrainingdollhasexchangeablemaleandfemalegenitalinserts.Urethralcatheterizationcanbepracticedforbothsexes.TheprocedurefortheinsertionofthegenitalorgansisdiscussedinsectionI2.

Assembly.Beforestartingthedemonstration,watercanbeputintothebladderwithasyringethroughthefillingaperture(capacityabout220ml).Forcatheterization,thefillingapertureshouldnotbeclosed.Whenthecatheterisproperlyplacedinthedollandtheliquiddoesnotflowoutofthebladder,onecanstarttheflowwithslightpressureonthebladder.Todothis,closethefillingaperturewiththefingerandpressonthebladder.Movingthedolltothesittingpositionmayalsostarttheflowofwater.

7a.CatheterizationofMenCatheterizationofthemalecanbepracticedafterthemaleorganshavebeenputinplace.

8a.HypodermicInjectionsThetechniqueforhypodermicinjectioncanbepracticedontheinjectionpadsinsertedintheupperarmsandthighs.Propercareoftheinjectionsitecanbedemonstratedonthedoll.

8b.IntramuscularInjectionsThetechniqueforintramuscularinjectionscanbepracticedusingthetwopadsprovidedonthebuttocks.

8c.HypodermicInfusionsThetechniqueofhypodermicinfusionscanbeshown.Besurethatthecapacityoftheinjectionpadsisnotexceeded.(Theliquidmaybepressedoutofthepadsfromtimetotime).

MaintenanceTheinjectionpadsconsistofaplasticsheathwhichcoversanaborbentpad.Thepadsmayberemovedforcleaningorforremovinginjectedfluids.Theabsorbentpadsareremovedthroughaslitonthelowersur-faceoftheplasticsheath.(Theshapeofthefoamrubberinsertindicatesitsproperpositionintheplasticcover).Aftertheseexercises,allinjectionpadsmustbecleanedthoroughly.Removethepadsfromthepla-sticcoveranddrythemseperately.Beforereinsertingthemintothecovers,alittletalcumpowdermaybeusedforlubrication.Sinceinjectionprocedureswilleventuallywearoutthepadstheyshouldbereplaced.Suchreplacementpadsareavailable.

9.OstomyTheabdominalcoverofthenursetrainingdollisprovidedwithanostomysite.Asmallplasticintestinalextension(about8cmlong)canbefastenedtotheinsideoftheabdominalcover.Itisclosedatitsinnerendandthushasnoconnectiontotheintestineitself.Alltechniquesconcerningthemaintenanceoftheartificialintestinalopeningcanbepracticedonthedoll.
