This year’s flu vaccine program

If you get an annual TB test:• You are required to either get the flu

vaccine or wear a surgical or isolation mask during flu season for all face-to-face patient interactions or to enter a patient’s room for any reason.

• If you decline the flu vaccine, you must sign a declination form and give the reason why (medical/religious/other specific reason).
