GRIEF & GRIEVING 4CHECKLISTGrief is a natural reaction to a major loss of someone or something. This checklist is not intended to diagnose, but to bring public awareness to a widespread and important issue affecting our society.

CAUSES: Grief may be activated by the death of a loved one, an illness for which there is no cure, or a chronic condition that affects quality of life. The end of a significant relationship may also cause grieving. Everyone feels grief in their own way. People’s responses to grief will be different, depending on the circumstances of the death. If the death was accidental or violent, coming to accept the loss might take longer.

SYMPTOMS: People who are grieving may have: Inability to Sleep, Crying Spells, Grief Bursts, Depressed mood, Deep Sadness, Loss of appetite, Lack of productivity at work, Loss of Interest in things or activities they once loved. It is important to remember that these are normal, and to be expected.

TREATMENT: Family and friends can offer emotional support during the grieving process. Sometimes outside factors can affect the normal grieving process, and people might need help from: Clergy, Pastor, Deacon, Elder, Bishop, Mental health specialists, Social workers and Support groups. The acute phase of grief usually lasts up to 2 months. Milder symptoms may last for a year or longer. Psychological counseling may help a person who is unable to face the loss (absent grief reaction), or who has depression with grieving. Joining a support group where members share common experiences and problems and help relieve the stress from grieving especially if you have lost a child, spouse, prolonged illness or loss due to suicide.

WHEN TO CONTACT A MEDICAL PROFESSIONAL: Call your health care provider if: You can’t deal with grief. You are using excessive amounts of drugs, prescriptions, or alcohol. You become very depressed. You have long-term depression that interferes with your daily life. You have thoughts of suicide, hurting yourself or others.

PREVENTION: Grief should not be prevented because it is a healthy response to loss. Instead, it should be respected. Those who are grieving should have support to help them through the process. Your experience of grief is likely to be different from another person’s. Similarly, you will probably grieve somewhat differently each time you experience a significant loss. Your reaction to loss is influenced by the relationship you had with the person you have lost and by your general coping style, personality, and life experiences. How you express grief is influenced in part by the cultural, religious, and social rules of your community.

GRIEF IS EXPRESSED PHYSICALLY, EMOTIONALLY, SOCIALLY, AND SPIRITUALLY: Physical expressions of grief often include: Crying and sighing, headaches, Loss of appetite, difficulty sleeping, weakness, fatigue, feelings of heaviness, aches, pains, and other stress-related ailments. Emotional expressions of grief include: Feelings of sadness and yearning. But feelings of worry, anxiety, frustration, anger, or guilt are also normal. Social expressions of grief may include: Feeling detached from others, Isolating yourself from social contact, Behaving in ways that are not normal for you. Spiritual expressions of grief may include: Questioning the reason for your loss, The purpose of pain and suffering, The purpose of life, and the meaning of death. After a death, your grieving process is influenced by how you view death. Grief can cause prolonged and serious symptoms, including depression, anxiety, suicidal thoughts and actions, physical illness, and post-traumatic stress disorder. Intense grief can bring on unusual experiences. After a death, you may have vivid dreams about your loved one, develop his or her behaviors or mannerisms, or see or hear your loved one. If you feel fearful or stressed by any of these experiences, talk to your doctor and a mental health professional or clergy person experienced in grief counseling. Age and emotional development influence the way a person grieves a death. This list is not intended to diagnose, but to bring public awareness to a widespread & important issue affecting our society.

Here are some questions to help you compare options and find the best plan for you. Open enrollment is the one time each year when you can make changes to your Medicare coverage. ENROLLED IN MEDICARE PART A AND PART Bq “I am already receiving Social Security benefits” – you will be automatically enrolled in Medicare Part Aand Part B and receive your Medicare card 3 months before your 65th birthday.q “I am turning 65 and I am not receiving Social Security benefits.” You need to contact Social Security toenroll in Medicare. You have 3 months before your birthday, your birthday month and three months after toenroll.q “I or my spouse will continue to work and have employer health coverage through the working spouse’semployer.” You will want to check with Social Security about delaying Medicare Part B. In additon to the following questions, ask the facility to specifically describe how it will meet your loved ones known care needs. When asking your questions, remember to take into consideration that your loved one may need more care in the future.

PERSONAL CARE NEEDS: How will the facility determine if it is the appropriate facility for my loved one’s needs, or if another type facility, e.g., assisted living would be more appropriate? How will the facility evaluate my loved one’s needs? Who will do the assessment? Whatarehis/herqualifications?Howoftenarereassessmentsdone?How does the facility help residents maintain their ability to care for themselves, especially toileting, dressing and eating?If a patient requires more physical assistance, increases in confusion, or becomes incoherent, how does the facility address these changes? For patients who develop confusion, how does the facility help deterime whether it is due to problems with medications or if it’s the onset of Alzheimers’ or dementia? How often are residents’ whereabouts and well-being checked? What steps are taken if a resident’s behaviour changes and becomes verbally or physically abusive?

THE PLAN OF CARE:Who develops and implements the pland of care? How will my loved one, my family and I be involved? How does the facility involve a confused resident in their own care plan? What if I don’t agree with the plan of care? Does the plan focus on maintaining independence? Does it cover every aspect of a resident’s life...physical, psychological, social and medical?How often does the staff review the plan?

PROVIDING THE REQUIRED CARE:How will the facility monitor my loved one’s health? Is each resident’s weight routinely monitored? Are there water pitchers and glasses in the rooms? Are residents encouraged to drink liquids?Does the staff check each resident thoroughly from head to toe to avoid occurence of bed sores? If a resident doesn’t feel well, how quickly do they require medical attention? Does the staff respond quickly when residents push their call buttons? What health services are available at the facility: physical therapy? Wound care? Hospice Care? Will the staff set up medical appointments for my loved one? Under what circumstances does the facility call the family or current doctor? Does the facility use physical or chemical restraints?

MEDICATIONS:Are there any limitations on how the staff will handle your loved one’s medications? What safeguards are in place to ensure that your loved one gets the appropriate medications on time and in the correct dosage? Howareprescriptionsfilled?Mustaresidentusethefacilitypharmacy?Whatarethecosts? Who gives out medications? Can we bring medications from home? If not a nurse, how are staff trained and supervised? Who reviews medication procedures and how frequently?

EMERGENCIES:Who decides when to call 911? Are there written policies on how the decision is made? What kind of emergencies are the staff expected to handle and how are they trained for them? Is there an alert button in each room a resident can use to summon help? Does each resident carry an alert button with them at all times?
