Coaching requests must be submitted through the Coaching Request Form online (shown to the right). Link provided on Expanding Access program page.
Option #1: MacColl Center Team development - internal facilitator training
• Each organization can nominate 1 staff member to become
an internal facilitator
• Training will provide more guidance in using the LEAP Team
Guide interventions
• Training format willl consist of 2-3 webinars over six months
• Your facilitator becomes in-house resource for team
development and culture change
• Deadline for facilitator nominations will be announced soon
+ Option #2: Coleman Associates Coaching for Access Improvement
Coaching tailored to your clinic’s current
performance and specific improvement goals
10 hours of contact (phone and email)
Tactical support for testing of several changes
Start now or any time before March 1
Option #3: Carolyn Shepherd, MD Senior leadership consultation
• Exec-level and team advice from safety net medical director responsible for testing, implementation, spread & sustaining innovations to grow clinic, improve access, health outcomes, patient experience, staff satisfaction
• Coach for team leaders and/or senior clinic leaders to address challenges of transformation in safety net clinics
• Help with leadership vision, strategy, and tactics for integrating, sustaining and spreading changes
• Solutions, tools, lessons learned from Clinica’s processes
• Request specific scope of work, or ask as needs arise
• Start now or any time before March 1.

PROJECT TEAM PLANNING FORM
Jumpstart your efforts to improve access and develop your care teams. As a team, document your priorities and next steps for the upcoming two months using the team development and appointment access strategies taught today. Focus on what is tangible, realistic, and best addresses your clinic’s specific improvement priorities.
