2.2 Essex Cares - logoWherever possible, the standard version with the strapline must be used. The preference is to use the logo on a white background.
Occasional use on dark backgrounds is expected, for this the logo is also available with a white wordmark. The black-and-white versions must only be used where printing in colour is not possible for technical or cost reasons. Please note that (except in the black-and-white version for dark backgrounds) the heart within the symbol appears white – not transparent.
Artwork is available as EPS files in 4-colour (CMYK) and spot colour (Pantone) versions. JPEG files are also available in colour (RGB) and black and white. Please don’t attempt to re-create or alter the logo in any way.

For the Essex Cares logo, the exclusion zone is the same as the height of the letter ‘e’ in the word ‘essex’.

2.4 Essex Cares - logo using the symbol on its ownGenerally, the symbol and the wordmark that form the logo must never be separated and the logos should always appear as described on the previous pages of these guidelines.
However, there are certain applications, such as vehicle livery, literature, bullet points and advertising campaigns, where the symbol can be used on its own, in addition to the full version of the logo. In these circumstances the ‘e’ can be rotated slightly but never more than 45 degrees. As displayed below the ‘e’ symbol can also have a transparency effect added to it.
Artwork files for the symbol are available from the Essex Cares’ Marketing and Communications Department.

2.8 Essex Cares - design elements in use
Community based support to prevent hospital admissions
Mobile Assessment and Enablement ServiceEssex Cares Reablement service has developed a ‘mobile assessor service’ which combines the Reable-ment assessment with the ability to assess for and deliver a range of equipment which supports inde-pendent living.
The majority of our customers benefit from the provision of equipment which enables them to progress through the Reablement programme and gain optimum function and increased independence. By of-fering the assessment and equipment delivery within a single visit we are able to provide ‘first contact resolution’ to our customers and an immediate start to the Reablement programme.
Supporting best practice in End of Life Care through Enabling ChoiceEssex Cares is piloting an End of Life Reablement service which will model national best practice guid-ance as per the Department of Health End of Life Care Strategy.
National statistics highlight that 80% of people die in a setting not of their choosing. Essex Cares service solution will seek to address this national statistic by enabling more people to be able to die in a setting of their choosing, with dignity and respect.
HomeSafe – helping you live independentlyHomeSafe is a range of products and services offered by Essex Cares which enables individuals to re-main living independently in their own home and enjoy being active members of their local community. Our dedicated call centre monitors quality and offers customers ongoing support. Products and services currently available through HomeSafe include:
• Home Support – services that are designed to support independence including help and support with personal care, meal preparation, domestic tasks, mobility and help to attend appointments.
• Active Life – support to access the local community, meet new people and discover new hobbies and interests.
• Home Help – tasks completed such as gardening, shopping and cleaning that a service user may no longer be able to manage to do themselves.
• Aids and Adaptations – assessment, delivery and installation of aids that help service users feel more safe and secure in their own home
This service is available to both private funders and people in receipt of personal budgets, offering a flexible and personalised alternative to traditional state provision.
‘I have regained 90% of my previous abilities and this is thanks to the encouragement, patience and support I have received from the HomeSafe team, without them my progress wouldn’t have been so rapid’
We can Assess, Coordinate and Deliver a whole range of services through a single point of contact in an efficient and streamlined way.
Ward led hospital discharge to ReablementThis discharge pathway provided by Essex Cares has been proven to significantly re-duce re-admission to hospital within the first 30 days of discharge and to increase the number of people requiring no further ongoing support at the end of the Reablement programme.
Essex Cares perform bedside Reablement assessments on wards when a referral is re-ceived directly from the ward nursing staff and OT teams. Patients are discharged directly to the Essex Cares Reablement team who then provide up to 6 weeks of support to max-imise independence at home. Support includes the provision of aids and adaptations as well as assistive technologies.
Crisis Response ServiceEssex Cares’ Crisis Response assessment & support commences within 2 hours of initial referral. Identified equipment is delivered & installed within 24 hours of our assessment. This means that 80% of individuals referred to our Crisis Response service avoid unneces-sary admission to hospital.
Essex Cares Crisis Response service provides equipment and support to individuals who urgently need assistance in order to avoid unnecessary emergency admission to hospi-tal.
The service provides short term care and support needed to help individuals regain their strength or confidence to be independent at home again. Referrals to the service are made by Health (including Community Nurses, A&E, GPs) and Social Care teams.
We provide support with:
• personal care
• dressing/undressing
• toilet assistance
• meal preparation
• shopping
• laundry
• assistance with medication and equipment
• devising an individual, outcome focused support programme
We work closely with the Community Nurse and Primary Care Trusts to ensure that the required health support is provided at home, to allow a complete, integrated program.
Essex Cares’ provision has demonstrated that it:
• Speeds up the hospital discharge process
• Reduces the number of assessments and hand offs between health and social care teams
• Reduces discharge delays
• Improves the patients experience
• Avoids duplication of social care assessments.
‘…..We have used the pilot service and have found it extremely good for those patients they have had referred needing both Health and social input rapidly and has prevented admission to hospital. It is a very good responsive service’.Mid Essex Community Health team
80% of individuals referred to our Crisis Response service avoid

2.9 Essex Cares - photographyPhotography plays an important role in our communications. It is much easier to communicate a message through an image than with words. Therefore we have to make sure we always select appropriate and relevant photos for everything we publish. Our pictures should be positive, uplifting and always represent at least one aspect of what we do.
Please make sure that photography is of high quality and that you have the necessary rights to use the images for the intended application including consent from those pictured. This page shows examples from our image library.

Essex Cares provides this service if you need help and support urgently. With our support you can avoid the need to be admitted into hospital and instead have all the care you need to get back on your feet, at home. For example you might need this support for a short time while you are recovering from an illness and need to regain your strength or confidence before you can be fully independent at home again.
Normally it will be the Community Nurse who will contact us to explain your situation and agree which type of support will help you get better at home. We will then arrange with you, your family and the Nurse to begin supporting you at home within four hours of receiving your information.
Our HomeSafe services are specially designed to help you live as independently as possible with peace of mind. We have four services which can be provided either separately or as a combination and we will work with you to make sure you get the service you need, when you need it.

I am really pleased that we are now at the point where Dee and I will be able to work
together to ensure that your transition from West Sussex County Council to Essex Cares is
managed as efficiently as possible. One of my key objectives will be to ensure the continued
wellbeing of staff and customers alike. I want to reassure you that I will be around in West Sussex for
the majority of the working week as I feel that it is important that we
have the opportunity to get to know each other and work
together to develop the service going forward.
Nina Worley (Consultant Project Manager) and I are planning
to visit each team mid-July following the consultation road
shows. I hope that we will all enjoy a very positive working
relationship.

Hello everyone,
I would just like to take this opportunity to
introduce Essex Cares HR services. I was TUPE’d
from Essex County Council into Essex Cares in July
2009 so I understand and appreciate some of the
concerns and anxieties you may have about a
transfer situation. It’s our job over the next few
weeks and months to work alongside West Sussex
HR and your managers to support you during this
transfer. This is something that is not new to us,
as the Essex Cares HR team have a broad
experience in supporting staff during change.
I’ll be attending the road shows and I am looking
forward to meeting you all and taking you
through the steps from now until the transition
process is complete. There will be plenty of
chances for you to ask questions when we meet
in July however if you prefer to email your
questions, at any time, then we have set up a
contact email address [email protected].
Essex Cares is a friendly and reputable employer,
all our employees work to our values and
behaviours, which means that we all share the
same hard working approach and are committed
to delivering quality support to our customers.
See you all very soon!
