These questions and consent are considered necessary to ensure the school can undertake its administrative and care responsibilities. Sections of the form not marked (*) are optional. However, failure to complete these sections may result in the school not being eligible for important Federal and State Government funding. Parent’s occupation and education: All parents across Australia, no matter which school their child attends, are being asked to provide information about family background (answering this question is optional). The main purpose of collecting this information is to promote an education system which is fair for all Australian students regardless of background. Introduction to the State School Consent Form (attached) for Mansfield State High School

This letter is to inform you about how we will use your child’s personal information and student

materials. It outlines:

what information we record

how we will use student materials created during your child’s enrolment.

Examples of personal information which may be used and disclosed (subject to consent) include part of a person’s name, image/photograph, voice/video recording or year level.

Your child’s student materials:

are created by your child whether as an individual or part of a team

may identify each person who contributed to the creation

may represent Indigenous knowledge or culture.

Purpose of the consent

It is the school’s usual practice to take photographs or record images of students and occasionally to

publish limited personal information and student materials for the purpose of celebrating student

achievement and promoting the school and more broadly celebrating Queensland education.

To achieve this, the school may use newsletters, its website, traditional media, social media or other

new media as listed in the ‘Media Sources’ section below.

The State School Consent Form may, at your discretion, provide consent for personal information

and a licence for the student materials to be published online or in other public forums. It also allows

your child’s personal information and student materials to be presented in part or alongside other

students’ achievements.

The school needs to receive consent in writing before it uses or discloses your child’s personal

information or student materials in a public forum. The attached form is a record of the consent

provided.

It should be noted that in some instances the school may be required by the Education (General

Provisions) Act 2006 or by law to record, use or disclose the student’s personal information or

materials without consent (e.g. assessment of student materials does not require further consent).

Voluntary

There will not be any negative repercussions for not completing the State School Consent Form or

for giving limited consent. All students will continue to receive their education regardless of whether

consent is given or not.

Consent may be limited or withdrawn

Consent may be limited or withdrawn at any time by you.

If you wish to limit or withdraw consent please notify the school in writing (by email or letter). The

school will confirm the receipt of your request via email if you provide an email address.

Page 14: Enrolment Application Pack - mansfieldshs.eq.edu.au · Copy of rental bond receipt Copy of latest electricity Account or copy of ... Bachelor degree or above No non-school qualification

If in doubt, the school may treat a notice to limit consent as a comprehensive withdrawal of consent

until the limit is clarified to the school’s satisfaction.

Due to the nature of the internet and social media (which distributes and copies information), it may

not be possible for all copies of information (including images of student materials) once published

by consent, to be deleted or restricted from use.

The school may take down content that is under its direct control, however, published information

and materials cannot be deleted and the school is under no obligation to communicate changes to

consent with other entities/ third parties.

The information will only be accessed by school employees administering the Scheme. However, if required, some of this information may be shared with Education Department employees for the purpose of debt recovery. Definitions 1. Reference to a “parent” is in accordance with the definition in the Education (General Provisions) Act 2006 and refers equally to an independent student. The State Education Fees Scheme (SEFS) 2. The cost of providing instruction, administration and facilities for the education of students enrolled at state schools is met by the State. 3. The school is not responsible for providing textbooks, in-class consumables and personal resources for students. 4. The SEFS enables parents to access certain resources, as detailed by the school, for an annual fee. 5. Participation in the SEFS is optional: parents are under no obligation to participate. Textbook and Resources Allowance (TRA) 6. The State provides the TRA annually to parents of eligible secondary students to assist with the costs of education. 7. The TRA is paid to the school based on enrolment data and will reduce the cost of participating in the SEFS. 8. Parents who choose NOT to participate in the SEFS and whose children are eligible for the TRA have the option to receive the TRA as a credit to their child’s school account or directly from the school. Parents and Citizens’ (P&C) Association support of the SEFS 9. All parents are invited to participate in the annual meeting of the P&C Association at which the income and expenditure report is presented and the proposed SEFS inclusions and fees are tabled for approval. Parents experiencing financial hardship 10. Principals may vary payment options and /or waive all or part of the SEFS for parents experiencing financial hardship. 11. Parents experiencing financial hardship who wish to participate in the SEFS should contact the Principal to discuss options. 12. The onus of proof of financial hardship is on the parent. 13. The school may require annual proof of continuing financial hardship. 14. All discussions will be held in the strictest confidence. Parents participating in the SEFS 15. Parents indicate whether or not they wish to participate in the SEFS by completing this Acknowledgement of Subject Fees Form. 16. Invoices for the amount of the annual participation fee will be issued to the parent providing consent to participate in the SEFS 17. This agreement is for the duration of the student’s enrolment at the school, unless a new Acknowledgement of Subject Fees Form is completed. 18. Parents must notify the school before the invoice due date if they intend to cease their participation in the SEFS in any year. 19. Where a parent chooses to participate, they are agreeing to pay the annual participation fee. 20. All SEFS resources provided for the student’s temporary use remain the property of the school and must be returned when requested by the school or if the student leaves. 21. Parents are responsible for ensuring that any SEFS resources provided for the students temporary use are kept in useable condition. Where an issued item is lost, not returned, or negligently damaged, parents will be responsible for payment to the school of the value of the item or its repair. 22. Parents must inform the school if items on the list of resources are not received. 23. Non-payment of the participation fee by designated payment dates may result in debt recovery action in accordance with the Department’s Debt Management Procedure. In such cases, the principal may:

• Withdraw the student’s participation in the SEFS • Require the return of items provided by the SEFS • Withhold the provision of any further items under the SEFS • Refuse participation in the SEFS where participation fees are

overdue from the previous year/s, • Exclude the student from optional, non-curricular activities,

and/or • Imitate debt recovery action.

Parents NOT participating in the SEFS 24. Parents indicate whether or not they wish to participate in the SEFS by completing this Acknowledgement of Subject Fees Form. 25. This agreement is for the duration of the student’s enrolment at the school, unless a new Acknowledgement of Subject Fees Form is completed. 26. Where a parent has previously indicated that they will participate in the SEFS, but wishes not to participate in any year/s, the parent must notify the school prior to the invoice due date of their intention to cease participation. 27. Parents who choose not to participate in the SEFS are responsible for providing their child with all items that would otherwise be provided by the SEFS. School-produced resources (e.g. Student Diary) will be available for separate purchase from the school for non-participants. Operating the SEFS 28. Schools will provide all parents with a list of resources provided by the SEFS to enable parents to assess the cost effectiveness of participation. 29. Schools will also provide non-participating parents with detailed lists of resources to supply for their child. 30. All items included in the SEFS must be able to be independently sourced by parents who choose not to participate in the SEFS, either from a third party supplier or by purchasing a school-specific-resource (such as a diary or workbook) from the school. 31. Schools may distribute information and invoices to parents in the year preceding the SEFS but invoices must not be due before the end of the first week of term 1 of the SEFS year. All invoices must have a minimum of 30 days for payment. 32. Parents must be given the option annually to choose not to participate in the SEFS. 33. In return for payment of the participation fee, the SEFS will provide participating students with the resources and materials as outlined in the school SEFS documents. 34. SEFS monies received are to be expended only on student resources outlined in the school’s SEFS and must not be expended on other items or used to raise funds for other purposes. 35. As the SEFS operates for the benefit of participating parents and is funded primarily from participation fees, the school retains control of the resources and determines the rules around their availability to non-participants. 36. A student entering the school during the school year is entitled to pay a pro-rata fee to participate. 37. Where participation fees have been paid and a student leaves the school during the year, schools must determine if parents will be eligible for a pro-rata refund. This calculation will also take into account any pro-rata allocation of the TRA (if eligible) and any outstanding SEFS debts (including any debts from damaged or non-returned items). Where the cost of outstanding debts is higher than the calculator refund, parents will be liable to pay this balance of funds. 38. The replacement cost of any resource will be up to the maximum value (subject to depreciation where appropriate) of the acquisition cost to the school, as detailed on the annual SEFS information provided to parents. 39. Schools may initiate debt recovery action in accordance with the debt management procedure where a parent has an outstanding debt.

Docs2Me Consent Form Dear Parents and Caregivers,

The purpose of this form is to request your permission to use the third party site Docs2Me to collect and pass on personal information of your student. At Mansfield SHS we use electronic processes for disseminating and collecting information required for school activities. The school utilises an online forms service through a third-party web-based provider to streamline the delivery and collection of information. This service is used to replace a range of forms that are usually disseminated and collected by a school, such as excursion and permission forms. Additionally, the use of this service ensures an increased speed and accuracy in which we can process information.

We can ensure that the legal and safety requirements in the handling of personal information of parents, guardians and students are safe, acceptable and supported by the Department of Education. For you to use the service, student personal information needs to be disclosed to the provider of the service. They are a private company that is hosted onshore in Australia. The data collected is passed onto the school and then destroyed. As a parent or caregiver, we require your permission for the use of this site to collect and pass on student personal information.

Please be assured that there are a number of safeguards to ensure the security of the information shared with this provider. To disseminate and collect information from parents, the school will send an email to the verified parent email address with a unique security token and another email with a link to the appropriate web form. Parents will use the security token to validate the information on the web form and submit the details online.

It is important that you understand the reasons that these websites collect this information, what will be done with it, who may have access to it and where the data is stored. This information can be found in the hyperlinks below referring to the website’s terms and conditions and/or privacy policy. If you have any queries around the storage of student’s information, please feel free to contact Mark Casey (Head of Information Technology) - [email protected]. The information collected will be handled in accordance with the Information Privacy Act 2009.

Terms of Participation Although it is preferable for students to join the Mansfield Laptop Program, we understand that this is not suitable for all families. For this reason, Mansfield SHS will allow students to bring their own device. This is known as the BYOD program. There are a number of considerations for families wanting to “BYOD” which will be outlined in this document.

1. Period of Participation 1.1 The period of participation is the current school year. Families will need to sign a new agreement each year. 1.2 The School agrees to provide access to the school network through a mobile device management system.

2. Fee for Participation 2.1 Fees for the BYO Program are incorporated in the school levy. There is no extra payment for BYOD.

2.2 If the school levy is not paid then access to the school network may be reviewed.

3. Minimum Specifications 3.1 All Devices must meet Mansfield’s BYOD Minimum Requirements; devices that do not meet the requirements will not be

connected to the school’s resources.

Minimum Hardware Requirements

Platform PC, Tablet PC or Mac (No phones, Tablets or ChromeBooks)

Screen Size 11” Screen or above

Processor Minimum Pentium. Core i5 recommended minimum for graphics, media and IT students

Operating System Windows 10 or Mac OS High Sierra and above. (iOS, Android, Windows RT or Linux not supported)

Wireless Must be 5GHz capable

Battery Life 8+ hours

Minimum Software requirements

Antivirus Up-to-date version of trusted antivirus software suite

PDF Viewer e.g. Adobe Acrobat Reader

Office Suite e.g. Microsoft Office (available for free to Qld school students)

Internet Browser e.g. Google Chrome, Mozilla Firefox

4. Connection to the Internet 4.1 The school provides secured, filtered access to the Internet via its high speed wireless network. Student access to the Internet

is governed by the School’s ICT Policy and Responsible Behaviour Plan. 4.2 The school uses the 5GHz wireless band as it provides a more stable connection and less interference. All BYO Devices must be

capable of connecting to a 5GHz Wireless Network, the school will be unable to connect devices that do not support 5GHz. If you are unsure consult the manufacturer’s website. (Some lower cost laptops do not support 5GHz Wireless.)

4.3 The school provides access to the school network via a mobile device management system (MDM). Enrolment in the system is best completed at home after attending school for the first time. All students are required to have Administrative Access to their BYO device to allow connection and installation of school resources.

4.4 The school reminds the parent/guardian of their obligations under this agreement. 3/4G ability e.g. USB Dongles, should be disabled or not brought to school as this function when activated allows students to bypass the EQ internet security filters. The school will take no responsibility for the content accessed by students using 3/4G facility on their personally owned devices.

4.5 At school, the carriage service and connectivity to the internet is governed by the School’s Student Network/Internet Access Agreement and the School’s Internet Usage Policy

4.6 When students use their devices at home the filtering is the responsibility of the family. Please note that some filtering solutions may interfere with network access at school. Care should be taken when selecting a filtering product to ensure school access can be maintained.

5. Improper use 5.1 The parent/guardian must ensure that the laptop is not tampered with in order to connect to internet services outside the

school and that the laptop is not used:

For any illegal, pornographic, fraudulent or defamatory purposes;

For bulk transmission of unsolicited electronic mail;

To send or cause to be sent any computer worms, viruses or other similar programs;

To menace or harass another person (or used in a way that would be regarded by a reasonable person to be offensive);

To transmit any harassing, obscene, indecent, offensive, or threatening material or emails;

To reproduce, distribute, transmit, publish, copy or exploit any material that constitutes an infringement of any intellectual property rights (such as copyright) of a third party; or

In a way that violates any laws, such as privacy laws.

BYOD Participation Agreement 2020

6. Software 6.1 Only licensed software authorised by the School can be stored or otherwise loaded on to the laptop. The parent/guardian must

ensure that any other software is not loaded onto the laptop. 6.2 The software loaded on the laptop is licensed to the Department of Education and Training or the School. The parent/guardian

must ensure that the software is not copied, deleted or transferred, for any reason at all, without prior written consent from the School. Unauthorised use may breach copyright laws and the parent/guardian may be held liable for any damages incurred.

7. Virus protection 7.1 Viruses have the potential to severely damage and disrupt operations within the School and the Department’s network. They

can also be costly to restore the network, infected hardware or software to its previous state and operability. 7.2 Viruses can enter laptop computers through: removable media such as CDs, DVDs, floppy disks and USB memory sticks

Emails/Phishing attempts (emails linking to malicious websites) The internet (including web browsing, FTP programs and chat rooms) file download Network file shares, such as servers and shared folders.

7.3 Students have the right to use their laptops at home for personal use. If accessing the Internet from home via cable or wireless, they should take all steps to protect the school-owned laptop and the department’s computer network from virus attacks.

7.4 The parent/guardian must take reasonably necessary steps to prevent a virus from infecting the laptop, including monitoring

Any data that is uploaded onto the Laptop from any device.

Virus checking any USB drives in the Laptop.

8. Repair and maintenance 8.1 All maintenance for the BYO laptop, operating system, software and/or apps purchased by the family are the responsibility of

the family. Families should ensure quick maintenance turnaround for student devices. 8.2 A limited number of Loan laptops may be available from the IT office for use while repairs are occurring; these will only be

available for use at school. The school provides minimal IT Support for BYO Devices. 8.3 If you run into a problem, students are advised to see the Mansfield IT staff who will attempt to diagnose the fault. If the

problem is not able to be resolved by IT Staff, they may recommend a course of action for repair (e.g. Re-install windows, warranty claim, insurance claim etc.)

9. Loss or damage 9.1 It is recommended that your device is covered by an extended warranty, statistically a computer will require, on average, 2.5

repairs during its 3-year life cycle. 9.2 Students are responsible for ensuring laptops are secure at all times, it is recommended that students do not leave their

devices unattended in public places. The school is not responsible for any damage to student devices and will not be accountable for any lost or stolen student devices or property.

9.3 When purchasing your laptop please look at options to purchase accidental damage protection for your IT device. ADP covers your device with accidental damage on and off the school campus. Fire, theft and acts of God are usually not covered under these programs and we request you to include it in your personal or home insurance. The insurance can be purchased with your computer vendor or any insurance company. All insurance claims must be settled between you and the insurance company. Statistically, 60% of repairs at the school are considered non-warranty. E.g. repairing a cracked screen from a drop. Purchasing insurance for your BYOD is a personal choice.

Laptop Rules for Students 1. The Laptop must be used for educational purposes while at school.

2. If you do not comply with these Laptop Rules for Students, you are not allowed to connect the laptop to the network. There may be other disciplinary consequences under your School’s Responsible Behaviour Plan and the School’s Student ICT Network Access Agreement and Usage Agreement also apply to your use of the network / internet when you are accessing the internet using the laptop. You are reminded of your obligations under that agreement and policy.

3. You accept responsibility for the security and care of the laptop. You must not tell anyone else your account password.

4. You are responsible for backing-up all necessary data. The School is not responsible for any data loss. Therefore, please ensure all your school work and important documents are backed up onto external storage devices or OneDrive.

5. You must take all reasonable steps to prevent a virus from infecting the laptop, including monitoring any data that is downloaded or uploaded onto the laptop from the Internet or any device and virus checking any USB drives in the laptop.

6. When not in use, the laptop must be stored in a carry case and either kept in your possession or in a secure place. 7. Images or sound captured by personal technology devices on the school premises or elsewhere must not be disseminated to

others using the laptop, for the purpose of causing embarrassment to individuals or the School for the purpose of bullying or harassment, or where without such intent a reasonable person would conclude that such outcomes may occur. The School has the right to invoke appropriate disciplinary processes to deal with such behaviour by a student.

8. You must not intentionally use the laptop or internet services to which it may be connected: For any illegal, pornographic, fraudulent or defamatory purposes;

For bulk transmission of unsolicited electronic mail;

To send or cause to be sent any computer worms, viruses or other similar programs;

To menace or harass another person (or use in a way that would be regarded by a reasonable person to be offensive);

To transmit any harassing, obscene, indecent, offensive, or threatening material or emails;

To reproduce, distribute, transmit, publish, copy or exploit any material that constitutes an infringement of any intellectual property rights (such as copyright) of a third party; or

In a way that violates any laws, such as privacy laws.

9. In particular you must not use the laptop (or any internet services to which it may be connected) to bully, harass or be unkind to other persons.

Use and care of the laptop computer

Usage

• Don’t use technology devices on soft surfaces (e.g. sofa, bed or carpet) because it can restrict airflow and cause overheating.

• Avoid dropping or bumping technology devices.

• Don’t place technology devices in areas that may get very hot.

• Don’t get technology devices wet, even though they will dry and appear to operate normally, the circuitry could slowly corrode and pose a safety hazard.

• Follow all instructions given by staff.

• Login correctly and logoff when finished.

• Always shut down computers through the ‘Start – Shutdown’ mechanism.

• Always package, carry and store technology devices in appropriate and secure carry cases for transporting.

• Personalise technology devices with methods approved by the school, to ensure students do not get the devices mixed-up.

• Don’t place objects on top of your laptop and never carry it around while it is turned on.

• Avoid exposing your laptop computer to direct sunlight or sources of heat such as desk lamps; dust, dirt, rain, liquids or moisture; heavy shock or vibration

Handling your laptop computer • Try to avoid moving your laptop around when it is on. Before switching on, gently place your laptop on a stable surface and

then switch on.

• You need to be careful with your laptop while it is in the bag. Do not drop the bag from your shoulder. Always place the laptop bag gently down.

• Be careful when putting the laptop in the car that no other items are on top of it and nothing will roll onto the laptop bag.

• Laptops should be switched off or hibernated before being placed into the bag.

• The bag should be fully zipped up before being carried

• The bag should be fully unzipped before removing the laptop to avoid non-warranty bag damage. Packing away your laptop computer

• Always store your laptop bottom down and with the LCD facing away from the front of the backpack.

• Do not wrap the cord too tightly around the power adapter or the cord will become damaged.

LCD screen • LCD screens are delicate - they don’t like being poked, prodded, pushed or slammed. Never pick up your laptop by its

screen. Don’t slam the screen closed and always be gentle when putting your laptop down.

• To clean your LCD screen:

Switch off your laptop computer.

Lightly dampen a non-abrasive cloth with water and gently wipe screen in a circular motion.

Do not directly apply water or cleaner to the screen.

Avoid applying pressure to the screen.

AC adapter • Connect your adapter only to your laptop computer.

• Do not step on your power cord or place heavy objects on top of it. Keep your cord away from heavy traffic areas.

• When unplugging the power cord, pull on the plug itself, rather than the cord.

• Do not wrap your cord tightly around the adapter box.

Keyboard • Gently brush your keyboard with a clean soft bristled paint brush or similar to remove dirt.

• If any key tops are missing or keys are in a damaged state, take your laptop to the IT department to be repaired immediately. A single key top can easily be replaced but continuing to use the keyboard with a missing key top can result in having to replace the entire keyboard which will treated as a non-warranty claim.

Case cleaning • Wipe with a dampened non-abrasive cloth. Do not spray any cleaners directly on to the casing.

• Gently rub your laptop casing with the moistened cloth to remove any dirty marks.

Security • Report any technology device fault or suspected virus activity to the nearest staff member.

• Undertake virus scans of computers after home usage and prior to reconnecting to the school’s ICT network.

• Make regular backups of your saved work.

• Keep your login and password confidential.

• Don’t tamper either physically or electronically with either hardware or software settings.

• Don’t attempt or undertake any malicious behaviour towards the School’s ICT resources.

• Don’t attempt to make unauthorised access to ICT resources or entities.

• Don’t have food or drink near the technology device.

• A good idea is to attach a fairly large name tag in a bright colour to the case or bag so it is easy to identify. Remember, over the life of the program this laptop may not be with the same student the whole time, and it may need to be returned for servicing at any time.

Software • Don’t copy any software from the School’s ICT network or system.

• All technology equipment should only have operating systems loaded that are compliant with departmental standards.

• Keep your virus check software up-to-date. If your virus check software detects virus activity then carefully follow the instructions for removal and advise the nearest staff member. If unsure, quarantine your computer and disks and immediately consult with the IT staff.

• Always adhere to licensing and copying agreements.

• Never use technology devices to engage in illegal activity, including violation of copyright or other contracts.

Batteries • Don’t use incompatible computer batteries and chargers.

• Computer batteries can get hot during use. Do not use your computer on your lap.

• Have fully charged battery/batteries at the start of each school day. All charging should be undertaken at home, as the school will not have the infrastructure or resources available to charge batteries for every student.

• Don’t permit a loose battery to come in contact with metal objects, such as coins, keys or jewellery.

• Don’t crush, puncture or put a high degree of pressure on the battery as this can cause an internal short-circuit, resulting in overheating.

• Don’t get your battery wet, even though it will dry and appear to operate normally, the circuitry could slowly corrode and pose a safety hazard.

• Follow battery usage, storage and charging guidelines found in the computer’s user guide.

Wet weather • Particular care needs to be taken during wet weather, whether at school or while travelling to/from school or at home

• Never drop your bag into a puddle, leave it out in the rain or where water might run, or have it otherwise unprotected from rain – if your school bag gets wet, your laptop might also.

Technical Support • Students can access the School's IT Department before school, during lunch times and after school to log a fault with their

laptop or seek technical advice from the School's technicians.

The goal is to ensure the safe and responsible use of services available to students through the provision of clear rules and guidelines. The student must understand that the right to use a device at school is conditional on the student abiding by the Charter. The device is primarily provided for an educational purpose. It is not provided for a recreational purpose. Failure to use the device in a responsible way may result in the student losing the device or losing the right to use the school network. When at school the student’s use of their device, in or out of class, is determined by a teacher. At all times the student is obliged to follow a teacher’s instructions regarding the student’s use of their device. Responsible use requires that the student follows the rules for the use of the device in the Mansfield State High School Handbook.

Breach of responsible use

Students will be held responsible for any actions caused by other persons using their account with the student’s knowledge. The school reserves the right to limit access of devices to the intranet, internet, email or other network facilities to ensure the security of the network and to provide a safe environment for all. The misuse of devices may result in disciplinary action which is not limited to the withdrawal of access to school services.
