Letter from President & CEO and Board Chair

Dear Friends,

Over the past year Eliza Bryant Village has continued to thrive through five key initiatives focused on managed care, higher acuity residents, aging-in-place, embracing technology and renal dialysis.

• We actively participated in AgeNet, a collaboration of Northeast Ohio’s most prestigious nonprofit long term care facilities to address Ohio’s mandate of managed care for Medicaid and Medicare recipients, “Dual Eligibles”. The collaborative efforts of this group have led to success in obtaining managed care contracts, developing quality metrics and identifying AgeNet facilities as industry leaders.

• We addressed the need of higher acuity by relocating skilled care residents to larger rooms in the renovated Rehab Center that also offers aviaries, an aquarium and specialized programming. Our expanded rehabilitation team offered many new therapies to facilitate a return home. Lastly, the Adult Day program completed a major renovation to support the transition from a social to a medical model of care.

• We promoted aging in place by initiating a pilot home care / chore services program for housing tenants. We recognize seniors want to remain living independently and for many housing tenants this requires a modest amount of assistance.

• We provided seniors and their visitors with free access to computers in three different locations throughout our 17 acre campus which is Wi-Fi capable. Nursing administration and therapy staff initiated use of tablets.

• We welcomed the Centers for Dialysis Care to our campus to address a growing community need for renal dialysis. This outstanding nonprofit organization has completed construction of a beautiful 20 station, 10,000 square foot facility which will open in the spring of 2015. With a substantially higher risk of renal failure in the minority community, this new renal dialysis facility will provide quality care, close to home for many individuals.

As Eliza Bryant Village celebrates its 118th year of service, it reaffirms its mission “to provide quality services, outreach programs and a dignified, compassionate and secure environment for seniors”.

The Village Highlights

This was an engaging and exciting year for Eliza Bryant Village. In September, we celebrated 118 years of serving Cleveland’s seniors as a premier provider of outstanding healthcare, programs and services along the continuum of care.

Highlights from the year include:

CENTER FOR DIALYSIS CARE AT ELIZA BRYANT VILLAGE: A groundbreaking ceremony was held on May 5, 2014 for a new state-of-the-art renal dialysis center on our campus. This is a collaborative effort between Eliza Bryant Village and The Centers for Dialysis Care. Open to the community, it is a benefit to residents and housing tenants who will no longer need to be transported for renal treatments. The new 10,000 square foot building is scheduled to open in Spring 2015 and will serve 120 individuals daily.

NURSING UNIT ROTATION: Affectionately called “The Big Move”, all four of Eliza Bryant Village’s nursing units were rotated on March 26, 2014. The move was undertaken to prepare for changes occurring with Ohio’s move into a managed care environment and to effectively build skilled care census. The relocation of the skilled care unit has helped to attract more short-term patients to The Rehab Center at Eliza Bryant Village.

MANAGED CARE - COMMUNITY TRANSITION PROGRAM: After numerous delays, MyCare Ohio for dual eligible individuals was implemented on May 1, 2014. Working in a managed care environment has presented challenges that have impacted reimbursements, nursing home admissions, length of stay, and all areas of care coordination. With support from The Cleveland Foundation, Eliza Bryant Village is building its organizational capacity to better navigate the transition into Ohio’s managed care environment.

AGENET - PARTNERS IN SENIOR CARE: Eliza Bryant Village has joined with twelve other Northeast Ohio nonprofit nursing homes, representing over 3,000 nursing home beds, senior housing and extensive outreach programs to proactively negotiate contracts with the three managed care companies.

SENIOR OUTREACH & WELLNESS SERVICES: We served 192 seniors. The Adult Day Program continues to transform from a social to a medical model to better meet the needs of a more frail and vulnerable older adult population. Programs and services were added to help adult day participants “age in place” and remain living in the community. The Abington Foundation provided needed funding to hire a nurse to monitor and address the medical needs of seniors attending the adult day program.

USDA ADULT CARE NUTRITION PROGRAM: We provided 40,434 hot nutritious meals to seniors in the Adult Day Program, tenants living in the Manor and community seniors. In October, the hot meal program was expanded to serve tenants in the Amasa Ford Lodge. The Higley Fund at The Cleveland Foundation provided funding to purchase a commercial grade food steamer to support the expansion of the nutrition program. All the meals are prepared in our dietary department. The Thatcher Family Fund provided scholarships to expand the hot nutritious meal program to additional tenants and seniors living in the community.

ADULT DAY COMMUNITY TRANSPORTATION PROGRAM: Eliza Bryant Village’s fleet of five vehicles provided services for 1,084 seniors, including 21,568 one-way trips and 1,654 wheelchair trips. The Community Demand Transit program transported seniors to medical appointments, social service agencies, pharmacies, banks and community organizations. Grants received helped to purchase a new 15 passenger wheelchair accessible vehicle and a light transit van for the adult day transportation program.

COMMUNITY TRANSITION PROGRAM: We continue to be successful in helping residents, adult day participants and housing tenants avoid preventable re-hospitalizations thirty days after discharge from a hospital or the nursing home. The Bruening Foundation provided funding to support the expansion of the program into the community.

GENERATOR FOR HOUSING: The Amasa Ford Lodge installed an emergency generator after power outages left tenants without electricity for several days over the past few years.

HOUSING HEALTH ASSESSMENTS: All 149 housing tenants received a comprehensive health assessment. Services were then put in place to assist individuals in helping them remain living independently. Over 30% of the tenants received home care services.

HOME CARE PROGRAM: The McGregor Foundation awarded a capacity building grant to validate the need and design an innovative, sustainable, home care program for tenants in our independent housing units to help them “age in place”.

HOUSING SERVICE COORDINATOR: In response to the social needs and increased frailty of tenants, a Clinical Service Coordinator was added to address tenant crises, referrals to social service agencies, family issues and wellness.

QUALITY ASSURANCE INITIATIVES: Staff was involved in quality assurance and performance improvement projects that focused on enhancing quality of life and health outcomes. Seventy-six (76) nursing staff received training using INTERACT tools to manage when a resident experiences a change in condition to better communication with the health care team to reduce hospital re-admissions.

THE REHAB CENTER AT ELIZA BRYANT VILLAGE: The average length of stay for skilled care patients transitioning from hospital to home after an illness or surgery was less than one month. Our caring rehabilitation team worked together to help individuals regain their independence and restore them to their highest level of functioning.

PALLIATIVE CARE PROGRAM: Under the direction of our Medical Director, Dr. Olusengun Odukoya, a resident and family centered palliative care approach was implemented to relieve pain, to ensure physical and spiritual comfort, and to improve the quality of life for residents diagnosed with a potential life-threatening, chronic, debilitating disease.

Partners in Caring: Our VolunteersOne unique factor in Eliza Bryant Village’s success in helping to meet the unmet safety needs of Cleveland seniors is our Board of Trustees and three hard-working Auxiliaries. We are blessed that volunteers have always been an integral part of our history and a partner in helping to enhance the quality of life each year for the 1,200 seniors we serve annually.

The three Auxiliaries have been a vital partner to Eliza Bryant Village serving as ambassadors, raising essential funds, and sponsoring programs to enhance the quality of life for seniors.

• Auxiliary Presidents joined together to raise funds to build a Gazebo and Memorial Garden for seniors living in The Village and surrounding neighborhood. The goal is to enrich the quality of life for seniors and make Eliza Bryant Village’s 17 acre campus a more beautiful place.

• Auxiliary I Annual High Tea and Luncheon was held July 19, 2014. The event was reminiscent of the In Gatherings held by the Lady Board of Managers, the original name for Auxiliary I before the name was changed in 1948.

• Auxiliary II Annual Holiday Mart, Fashion Show & Luncheon was held on November 1, 2014 with more than 400 people in attendance. The “Annual Spring Fling” held in May and the “Fun Day” in August was enjoyed by all the residents.

• Auxiliary III - Annual Spring Luncheon and Music Fest was held on Saturday, May 10, 2014. The traditional “Parade of Hats” once again impressed and awed everyone in attendance. The Annual Holiday Party for residents in December included performances by church choirs, delicious cookies baked by Auxiliary III members and a special visit by Santa and his elves bearing gifts for each resident.
