The format is modeled after the very successful program implemented by City Manager James Spore and available to all the City staff. The purpose of this communication path is to give you an unrestricted communication path to me. Deputy Chief Kiley recently delivered to each of you an email that included the expectations for use. He stated, “EMS highly respects the chain of command, however utilizing this email will not constitute breaking the chain of command. If you have a question or concern, please use this email address anytime. This email should not be used for personnel issues. It is designed to increase general communications and provide a pathway to connect with the EMS Chief.” I continue to explore new paths to connect with each of you (all 1,323 of you) in order to provide you the time and attention you deserve. As our organization continues to grow, flourish, and provide the best patient care and services, it is more important now than ever to ensure open lines of communication. One of the areas for improvement that arose in both the Town Hall meeting I asked Deputy Chief Kiley   to   hold   in   January,   and   the  City’s   biannual  member’s   survey,  was   to   increase   avenues   of  communications directly between you, our valued and trusted member, and me as your Chief. I am pleased to announce, that under the direction of Deputy Chief Kiley, the Department of EMS has created a new email address [email protected]. The format is modeled after the very successful program implemented by City Manager James Spore and available to all the City staff. The purpose of this communication path is to give you an unrestricted communication path to me.

In addition to current OSHA, in order to be an AIC, Interns must complete a HazMat course and IS 100, IS 200, IS 700 and IS 800 courses prior to being released. Please ensure we have the certificates on file or visit the websites below to complete the courses which are available online and free!

Save the date! A request was made in our January Town Hall meeting, that EMS create a leadership course specifically designed and offered to Virginia Beach EMS personnel to help in gathering those skill sets necessary to advance to the level of Squad Commander or perhaps Brigade Chief. Therefore, we ask you to save the dates, the evening of Friday July 26 and Saturday July 27. This Friday evening and Saturday day class will go over the leadership strategies and organization philosophy of the Department to make you a better leader. All  current  and  potential  leaders  are  welcome…  MORE  TO  COME.

Coming Soon We are working on creating electronic evaluations for our Interns. The evaluations would be on Oscar Track. We are still in the testing phase and will be advising when they go live. More to follow soon.

Are all of you logged on? Every member on duty MUST be logged into CAD via the MDT and MUST be listed on the patient care reports in the EMR. Please make sure that everyone on the call is listed in both places including interns and students. If there is a crew member who is not available on the list in the EMR, there is an issue and they should not be on duty until you can resolve the issue.

Governor McDonnell Issues Proclamation Designating Fire and EMS Memorial Week

-Ceremonies to honor Fire and EMS personnel to be held in Richmond, Accomack County, Roanoke County, James City County and the City of

Petersburg- RICHMOND – Governor McDonnell issued a proclamation today designating the week of June 2-8, 2013  as  Virginia’s  first  ever  Fire  and  EMS  Memorial  Week.  The  memorial week will serve to honor and remember those who have made the ultimate sacrifice while serving the Commonwealth as Fire or EMS personnel. Official events for the week will begin with the annual Virginia Fallen Firefighters Memorial on Saturday, June  1,  2013  at  the  Richmond  International  Raceway’s  Main  Exhibition Hall. Speaking  about  the  proclamation,  Governor  McDonnell  said,  “These  brave  men  and  women  have demonstrated remarkable courage in the face of danger. It is critically important that we take

It is fitting that we pay tribute to them for what they have done and express our deepest sympathies to the families of these heroes. I encourage all Virginians to observe Fire and EMS Memorial Week by remembering these true public servants and extending your gratefulness to the  first  responders  in  your  community.” Speaking about the memorial week, Secretary of Public Safety Marla Graff Decker stated, “First  responders  have  tremendous  dedication  and  commitment  to  saving  lives  and  property.  That  dedication  often  leads  them  into  harm’s  way.  We  all  mourn  the  loss  of  these  true  heroes  and  we  are  forever grateful for the price they paid to keep us safe. This memorial week will enable us all to reflect on their supreme sacrifice and to express our heartfelt gratitude. Additionally, the week will allow us to show our appreciation and utmost respect for all of the Commonwealth’s  first  responders  who  serve  knowing  they  may  one  day  be  called  to  make  the  same  sacrifice.” Secretary  of  Health  and  Human  Resources  Bill  Hazel  commented  on  the  tribute,  “Virginia’s  first responders step forward on a daily basis to serve others. Fire and EMS personnel risk their lives every day when they rush into burning buildings or provide life saving aid to someone in need. Fire and EMS Memorial Week is our opportunity as a Commonwealth to memorialize those first responders who sacrificed their lives in an effort to save another. It is also an opportunity to express our appreciation and gratitude for the sacrifices made by these brave men and women and their families  every  day.”   Regions and localities are invited and encouraged to host memorial events of their own during  this  memorial  week.  The  focus  of  this  year’s  memorial  week  is  to  honor  the  memory  of  the  individuals who died in the line of duty in 2012; however, localities are urged to recognize all of those who have been lost in the line of duty in their jurisdiction. End of Semester ALS Student/Intern Note ALS students completing their clinical field internship are in crunch time as the semester is rapidly coming to a close. Beginning last year, TCC enhanced students are not allowed to start their field internship until approximately 6 weeks into the semester, or later, as TCC began to implement a phased approach to the enhanced program adding to the further competition for ALS FTOs as April comes to a close and for the first few days of May. The support of our sponsored students, to assist them in their scholastic achievements, is vital to the future of our system and continued enhancements to the care we provide. ALS preceptors are assigned to students to assist them in their achievement but, unlike ALS interns working to be released under general supervision, the ALS training institute (TCC) sets the clinical and field competencies. Therefore, beginning last year we began to allow ALS students completing their field clinical internship to obtain their ALS certification by scheduling ride time with ANY ALS FTO on an ambulance or zone. Additionally, we changed the process to require evaluations for any student / intern to be completed and turned in to us (signed by both the student and FTO) within 24 hours of their duty. Students must also forward a copy of their evaluation to their ALS clinical coordinator at TCC. Training staff, under the direction of the EMS Captain assigned to training works closely with operations staff to ensure student expectations and assignments are equitable and achievable. Operations, through the coordination of the EMS Captains and Brigade Chiefs, work diligently to ensure the seamless coordination of ALS students and interns with the ALS FTOs assigned to their shift to maintain the integrity of the complimentary missions. Thank you for your contributions to our student achievements! Any concerns or suggestions for improvement should be addressed through your chain of command and we will work to resolution as appropriate.

Definitions:

Student: a member that has been formally preapproved into a sponsored certification program by VBEMS and is currently enrolled in an accredited ALS certification course. Intern: a member certified at one of the three ALS levels (enhanced, intermediate, paramedic) and that has been formally approved for precepting in the VBEMS ALS release program.

Note: Beginning May 20th or thereabouts, the current Fire Recruit Academy will begin their enhanced certification class with TCC. This coincides with the TCC summer semester. Their classroom will be accelerated so they will be out for their student field clinical time approximately 2 weeks later, about June 4th. Their clinical field competencies should be complete within 2-3 weeks at which time the TCC regular enhanced certification will be preparing for their field clinical time. This scheduling worked well last year providing a good mix of students and interns that prescheduled their ride times to better enhance their certification and care provided to our community.

CE Credit and EMS IDs

What do the two items in this articles title have in common? The OEMS has been providing barcoded VDH certification cards for the past couple of years to all providers upon initial certification and recertification. You may have noted that the Department of EMS has been barcoding the reverse of VBEMS IDs for the past year. Beginning in FY14, the Fire Department will also barcode their IDs with this information. This barcode is your VDH EMS certification number and level. This is a unique identifier for all providers. This barcode provides the opportunity to be greener, reduce bubbling errors on the current CE cards and reduce the time from CE class completion to CE credits posting in your OEMS Portal. Therefore, beginning in the first quarter of FY14, all CE programs offered through the Department of EMS will move to only scanned CEs and elimination of the bubble card. At such time, you will have to have a valid and current Virginia Beach EMS or Fire ID properly barcoded or your barcoded VDH state certification to receive CE credit from the Department for programs completed. Please stop by EMS Admin or Training to obtain your new ID (yes, they will check to see if your OSHA, CPR and EMS certification is current.)

Mass Casualty Training Training staff has developed a sustainable mass casualty incident management (MCIM) training program beginning with our EMT students and continuing through continuing education classes for both ALS and BLS providers. From now on, MCIM I/II courses will be offered quarterly.

The Department has also partnered with MMRS to pilot their new three hour hands-on mass casualty incident (MCI) drill. This drill focuses on the ability of participants to apply regional MCI Guide protocols and focuses on the first alarm assignment for medical crews operating at a MCI. The scenarios involve small groups working together and CE credits have been applied for. Should this program be well received and attended we can incorporate it in FY 15 programming and beyond.

Expanded E-Learning Courses To meet current and future needs of our Members, Human Resources/Learning & Development (L&D) is always working to obtain and develop new and enhanced learning opportunities. These efforts have enabled a greatly expanded E-Learning library of courses that are available to all Members. L&D has also obtained an instrument that will provide the freedom of customized courses and content through multiple devices/sources. These additions further enrich our Mobile Learning environment, and provide more tools to empower all Members to take full advantage of and elevate their learning experiences.

As of January 2013, the E-Learning library now offers more than 270 courses. With course subjects ranging from Computer Fundamentals, Leadership & Management, and Office & Personal Productivity,  there’s  sure  to  be  one  that  interests  you.    To  view  a  complete  list  of  available  courses,  please visit the e-learning home page on beachnet (Benefits & Services > Training & Development > E-Learning). Please discuss your training needs and opportunities with your supervisor and your department’s  training  representative.    

L&D’s  Mobile  Learning  platform  will  be  introduced in a few months. This tool, powered by LearnCast, provides endless educational opportunities. Digital learning is quickly becoming the standard. Mobile Leaning courses will provide a medium for concise, clear and targeted content. With the ability to distribute these courses through email, URLs, QR-Codes, SMS text and Mobile apps, managers can improve efficiency and reduce costs. ALS Classes Applications ALS Application Deadlines are now set for the year. If you are interested in taking ALS classes at TCC or need field time for competencies, please make sure your application is submitted within the time frame listed below. For those already certified at the ALS level, you may submit your ALS application for affiliation at any time. Summer semester classes: Applications will be accepted March 1 – March 30

Fall semester classes: Applications will be accepted June 1 – June 30 Spring semester classes: Applications will be accepted October 1 – October 31

***NO LATE APPLICATIONS WILL BE ACCEPTED*** Approved Applicants must: 1. Applicant must meet all current Commonwealth of Virginia eligibility requirements. www.vdh.state.va.us/OEMS 2. Applicant must meet all current regional eligibility requirements. www.tidewaterems.org 3. Applicant must be an active member in good standing of the Department of EMS. 4. Applicant must be an Attendant-in-Charge and a released driver with the Department of EMS at the time of application. 5. Applicant must have current CPR certification at the time of application. 6. Applicant must have current Virginia certification as an EMT, EMT Enhanced or EMT Intermediate as appropriate for the course for which they are applying. 7. Applicants for the Paramedic Course must have current ACLS certification. 8. Applicants must have completed all annual Department of EMS training requirements and required protocol updates. 9. Individuals who elect to continue to Intermediate, without being released at the Enhanced level, may request a waiver to be sponsored to Intermediate.

a. DEMS will not pay for tuition for Intermediate unless the provider is released as an enhanced at time of sponsorship application. b. Sponsorship will be for one semester. Continuation of sponsorship beyond one semester requires that the student successfully completes Intermediate certification exams.

10. For sponsorship in the Paramedic program, the applicant must be certified as a National Registry or Virginia EMT Intermediate or Virginia Cardiac Technician. 11. Defaulted training contracts must be rectified prior to application.

Application Process: 1. Applications will be available at EMS Training. 2. Applications will be accepted and processed only at announced times prior to the beginning of each semester/course. 3. The applicant must complete his/her part of the application, including a Release of Information Form, and attach any required copies. 4. The completed application with attachments must be returned to EMS Training or sent to [email protected]. Incomplete or late applications will not be accepted. 5. Applications will be reviewed for sponsorship consideration. Submitting an application does not mean automatic approval. 6. The Chief of EMS will make the final decision on acceptance or denial of all applications. The Chief or his designee must sign all applications. 7. Applicants will be notified in writing regarding the status of the application within a reasonable time after the deadline. 8. The Training Division will assist with registration for sponsored students.

Approved Applicants Will Complete: NCIC, DMV, and Training Contract Books and Test Fees will be provided to approve applicants. Medical Evaluation and shot records must be provided by the individual.

LVAD vs. Total Artificial Heart

LVAD - Heartmate II or Heartware

Continuous pump attached to left ventricle Patient will not have a palpable pulse Patient will have a ECG rhythm BP only obtainable via Doppler and will only get MAP If patient is unconscious, listen to LVAD to see if it is working. LVAD patients should be treated  “wetter”

o Continuous flow of LVAD pump and patient being used to fluid restriction from years of CHF often leads to hypovolemia. Patient may have a syncopal episode from hypovolemia. LVAD patients can tolerate fluid boluses

CPR can dislodge LVAD causing lethal hemorrhage. CPR is last resort and only after all other trouble shooting has been done (i.e. change batteries, controller) If CPR is needed, compression location slightly to left of sternum.

Defibrillation and ACLS medications OK o Patient maybe in VF but since LVAD functioning, patient may have good LOC so

treatment not needed.

Total Artificial Heart – TAH Freedom Driver

Mechanical ventricular pumps attached to native atria Full eject device – every ml of blood in each pump is ejected with each pump. Patient will have a pulse and BP TAH  patients  are  treated  “Dry”  - Treat patient as if they have severe CHF with fluid

restriction TAH pumps do not stretch like normal ventricles

o Native  heart  follows  Starling’s  Law  of  Heart:  increase  ventricular  filling causes ventricles to stretch = increased cardiac output

o TAH does not stretch so any increased volume from fluid administration will back up into lungs causing flash pulmonary edema

NEVER GIVE EPI – will cause vasoconstriction, decreasing flow to TAH which will be lethal for patient

No CPR – cannot compress metal ventricles No defibrillation No Pacing No ECG – will not get a tracing since native ventricles removed Most common problem is fluid overload, hypertension, CHF, flash pulmonary edema

o CPAP OK o Nitro, lasix OK

ALS Release Classes

ALS Release Class Sessions 1, 2, and 3 are required for all EMT-I and EMT-P interns before they are released to function independently (Session 1 is required for ALL EMT-Enhanced). It can be taken at any point during the internship – interns are encouraged to take it early because you are not allowed to use equipment such as CPAP until you have taken the class. These classes cover the ALS functions and skills unique to patient care in VBEMS.

Paramedics: Take ACLS, PALS and Sessions 1-8, which will give 48 hours of Category 1 credit. You also need 24 hours of Category 2 credit. Intermediates: Take ACLS, PALS, and Sessions 1-8. This will give you all of your Category 1 and Category 2 credits to maintain your Virginia certification. National Registry requires an additional 24 hours of Category 2 credits. Enhanced: Take Medical & Respiratory for EMT-Enhanced, Session 3, Session 4 and Session 5, which will give 10 hours of Category 1 credits plus some Category 2 credits. You need a total of 26 hours of Category 2 credit. Please note that EMT-E providers are not allowed to take PALS, ACLS or 12-Lead EKGs (Session 6).

EMSAT

EMSAT is a satellite-based program produced by the Virginia Office of EMS. The program is

shown at the EMS Training Center on the third Wednesday of each month from 7:30 pm to 8:30 pm, and each program offers one hour of continuing education credit for ALS and BLS providers. EMSAT courses are open to any EMS provider in the region. Pre-registration is mandatory to ensure correct instructor-student ratios. Walk-in students may not be able to be accommodated. Be Ready for Severe Weather Hurricane/flooding season starts June 1

Last year in the U.S., more than 450 people died and nearly 2,600 were injured as a result of severe weather. Hurricane Sandy contributed to more than 280 of those deaths and has become the second costliest  hurricane  in  our  country’s  history.     Hurricane/flooding season starts June 1 – are you ready? Here are important tips for staying safe before, during and after severe weather:

Listen to local radio and TV stations for updated emergency information. Get the latest weather information from the National Weather Service at www.weather.gov. Purchase a NOAA Weather Radio or battery-powered/hand crank radio with a NOAA

Weather Band to get weather warnings directly from the NWS. Make sure you have a safe place to go when severe thunderstorm or tornado warnings are

forecast. Safe areas include basements and interior rooms such as bathrooms or closets on the lowest level of a sturdy building. Stay away from windows.

Do not drive or walk through floodwater. Drowning is the number one cause of flood deaths. Turn around and find another route if a road is flooded – it is almost always more dangerous than it appears.

Have a family emergency plan. Everyone should know what to do in case family members are not together when severe weather happens. Get a free emergency plan worksheet at www.ReadyVirginia.gov or from the Ready Virginia app.

Evacuate immediately if advised to do so. Keep  emergency  supplies  on  hand,  including  at  least  three  days’  non-perishable food and

water, flashlights and batteries, first aid kit, medicines. Follow the instructions of local and state officials.

Money-Saving Tips for Cooling Your Home

Air conditioning is essential to the comfort of your family, but summer cooling costs can really make you sweat. The energy-saving tips that follow will help to reduce your utility bills while providing adequate cooling:

Close window treatments during the day to prevent heat gain from the sun. Have your air conditioner serviced before cooling season begins to ensure that the system is

running efficiently. Use exhaust fans while cooking and showering to lower the humidity inside your home. Plant leafy trees and shrubs on the south and west sides of your home to provide shade

during the hottest hours of the day. Use natural ventilation. Change furnace air filters often; dirty air filters increase cooling costs. Make sure your home is insulated according to recommended levels from the U.S.

Department of Energy. Caulk or weather-strip all of your windows and doors; eliminating unwanted air infiltration

significantly reduces cooling costs. Check ductwork in your attic or crawlspace to ensure it has not become disconnected or

obstructed. Make sure attic ductwork is covered sufficiently with insulation. Install a programmable thermostat that will automatically raise the temperature setting at

night and when you are away from home. Set the thermostat to the highest temperature at which your family is still comfortable. For

each degree you set your thermostat below 78°F you add up to 3 percent to your air-conditioning costs.

Consider using a whole-house attic fan to circulate air through the house and move warm attic air to the outside.

If your air-conditioning system is older or frequently needs repair, consider replacing it with a new unit that is sized correctly and qualifies for ENERGY STAR certification. In some climates, air-source or geothermal heat pumps are a highly efficient option for year-round heating and cooling.

ENERGY TIPS FOR YOUR HOME

Install a programmable thermostat and set it to adjust the temperature throughout the day. Repair, seal and add insulation to existing heating and cooling ducts, and the attic, to prevent losses. Seal

around  pipes  and  wires  that  enter  or  exit  your  home’s  living  space.  Close  the  damper  on  your  fireplace  when  you’re  not  burning  a  fire.

Consider replacing old windows with new high-performance, dual-pane windows. Wrap your water heater with a water heater blanket. Don’t  over  run  kitchen  or  bathroom  exhaust  fans. Consider placing solar film on your home windows. This can help keep the hot sun out. Draw shades or

drapes to block sunlight during the hottest part of the day. Clean or replace furnace/air-conditioning filters monthly. Have your heating and cooling systems checked seasonally by a professional to ensure they are operating

at peak efficiency.

APPLIANCES (Washer, Dryer, Refrigerator, Stove, Televisions) Only wash full loads in the dishwasher and use the energy-saving setting for the drying cycle (or air dry). Dry  only  full  loads  of  laundry  and  keep  the  dryer’s  lint  filter  clean.  Create  less  heat  in  your  home  by  

drying clothes in the early morning or later in the evening. Vacuum refrigerator coils often and make sure your refrigerator door seals are airtight. If your refrigerator is at least 10 years old, consider replacing it with an ENERGY STAR® qualified

model.  Don’t  keep  your  refrigerator  too  cold,  the  ideal  temperature  is  between  32°-39°. Consider using a gas (instead of electric) range or oven. Use  a  pot  or  pan  that  completely  covers  the  stove’s  heating  element,  exposed  portions  waste  heat  &  

energy. Reheating meals uses less energy than cooking them for the first time. Try cooking larger meals and

reheating  the  leftovers.  When  it’s  warm  outside,  grill  out.  Cooking  outside  keeps  the  heat  outside  and  the  cool  air  inside.  When  considering  the  layout  of  your  kitchen,  don’t  place  the  refrigerator  near  an  oven  or   other heat source.Like to fall asleep with the TV on? Use a sleep timer!

COMPUTERS Plug electronics into power strips with surge protection. Turn the strip off when the equipment is not in

use. Activate  your  computer’s  sleep  mode  when  you  leave  your  desk.  This  reduces  electricity  consumption to

less than 5% of full power. Use the energy-saving feature on computers, monitors, printers, copiers and other business equipment.

Turn them off at the end of the day - consider a laptop over a desktop as laptops use less power.

LIGHTING Turn off lights when you leave a room. Replace  regular  light  bulbs  with  Compact  Fluorescent  Lights  (CFLs)  or  LED’s.   Install lighting occupancy sensors, dimmers, or timers. Use  task  lighting  when  possible.  Focus  light  where  it’s  needed  rather  than  lighting  an entire room.

WATER Take shorter showers and use low flow shower heads. Lower your water heater to a temperature of 120-125 degrees. Consider warm or cold water for washing clothes.

OUTSIDE Plant deciduous trees on the south and west sides of your home to block the afternoon and evening sun. Shade air conditioning units and single story homes with trees. Need a new roof? Choose lighter-colored roofing to reduce heat absorption from the sun.
