Viking 4D/X25 plywood box failure

• ORPS report NA--NVSO-NST-NTS-2014-0008 (6/12/14)

• Structural failure

– The structural panel stapled to the main short side panel

serving as the assembly surface/platform for a lifting

handle fully dislodged from the main short panel while

being moved by an NSTec High Explosives Facility

Explosives Handler. This action caused the left side of the

box to drop (3 to 5 inches) and impact a wooden pallet

positioned on the floor underneath

• Use of the Viking boxes has been suspended
