Paraphrasing the forward, penned by Noah Chivian, DDS (Rutgers School of Dental Medicine and University of Pennsylvania School of Dental Medicine), “The Art and Science of Contemporary Surgical Endodontics looks to replace the trio of text books, Endodontic Surgery (Harper & Row, 1981), Surgical Endodontics (Blackwell, 1991) and Practical Lessons in Endodontic Surgery (Quintessence, 1991) that have been primary teaching resources for the past 25 years. In 2001, Color Atlas of Microsurgery in Endodontics (Saunders) introduced the profession to the modern understanding of true endodontic microsurgery. Since 1991, few books have addressed the surgical needs of modern- day dentists who want to adopt contemporary and evidence-based surgical endodontics methods into their practices.” Dr. Rubinstein, a leader in the field of surgical endodontics, co-authored The Art and Science of Contemporary Surgical Endodontics, recruited the collaborative chapter authors. Ibuprofen Use for Acute Odontogenic Pain

In acute pain conditions, an analgesic’s onset speed is critical not only because of the relief experienced but also because rapid onset reduces the risk of

excessive dosing. When rapid pain relief is not achieved, patients are likely to use more than the recommended dose or supplement the analgesic with a different type of analgesic, resulting in adverse effects and systemic complications.

Considered the gold standard nonsteroidal anti-inflammatory drug (NSAID) in clinical trials on acute odontogenic pain, ibuprofen acid has been used for decades to alleviate odontogenic pain. Unfortunately, it is not readily soluble, which affects its onset of action. Enhancements in ibuprofen acid’s pharmacokinetics have led to the development of ibuprofen salts with a faster dissolution rate and quicker onset of action.

In a randomized, double-blinded, controlled, parallel-group trial, Taggar et al from the University of North Carolina at Chapel Hill compared the analgesic effect of a single dose of ibuprofen sodium dihydrate with a comparable dose of ibuprofen acid in 41 healthy patients (age range, 18 to 60 years) diagnosed with symptomatic irreversible pulpitis and symptomatic apical periodontitis who were experiencing moderate to severe pain (defined as more than 30 mm on a 100-mm visual analog scale [VAS]). Patients with a known hypersensitivity to NSAIDs, a history of gastric or duodenal ulcers, or gastrointestinal bleeding were excluded from the study, as were pregnant and lactating women and patients classified as American Society of Anesthesiologists class III through class V.

The study had 3 outcome measures:

• onset of 50% pain relief• reduction in spontaneous pain • attenuation of mechanical allodynia

Patients reported their pain’s intensity on a 100-mm VAS (0 mm equals no pain; 100 mm equals the worst pain imaginable). Using a digital bite force transducer, the researchers measured mechanical pain thresholds on the painful and contralateral asymptomatic (control) teeth and calculated mechanical allodynia as the difference in mechanical pain thresholds between the controls and affected teeth.

Patients randomly received either a single dose of 400 mg ibuprofen acid or an equivalent dose of two 256 mg ibuprofen sodium dihydrate tablets. After the drugs’ administration, patients were given a stopwatch and were instructed to indicate when they felt a 50% decrease

in pain intensity. Sixty minutes after administration of the drug, patients rated their pain on the VAS, and the mechanical pain thresholds of the painful and control teeth were determined. Tests and data collection were performed by a single examiner masked as to the type of ibuprofen formulation given to the patients.

Results showed that the median time to onset of 50% pain relief in the ibuprofen sodium dihydrate group was 26.5 minutes, compared with 44.0 minutes in the ibuprofen acid group (p equals .087). Twelve patients (8 who received ibuprofen acid, 4 who received ibuprofen sodium dihydrate) failed to experience 50% or more pain relief within the 60-minute study period.

ConclusionA single dose of ibuprofen sodium dihydrate provided faster onset of odontogenic pain relief and a greater reduction in spontaneous and evoked pain than did ibuprofen acid.

Farmington Hills Study Club ~ Celebrating 5 Years

An annual tradition for our Rochester office is “kicking off fall” by hosting an evening out for the staff of our referring doctors. This was the 11th year at the Rochester Mills Brewing Company, with a large turn out of 135 staff from 35 different dental offices. The Rochester Mills Brewing Company is a great venue to hold our annual party; With a fun atmosphere, delicious food and beverages that makes mingling with the guests that much more enjoyable. “We talk to a lot of these ladies daily, its nice to be able to put a face with a name and get to know each other”.

Every year a generous raffle is held consisting of several coach purses and a wide variety of gift cards. “All of our staff gets involved in the raffle and the guests hoot and holler when anyone wins” says Patti (Rochester Office Manager). Its safe to say the guests have as much fun at this event as the EA Rochester staff does! This is an important event for staff on both ends and helps build relationships in and outside of the workplace. We want to thank everyone for attending and hope to see you all again in the years to come.

Since 2012, The Farmington Hills doctors and staff have held a study club every other month. From small to large gatherings of dentists and endodontists, it is a collaborative learning environment using case based examples to explore the intersection of dentistry and endodontic care.

The study club has been nationally recognized at the American Association of Endodontists national meetings as a role model of partnership learning between dentists and endodontists. The effort truly speaks to Endodontic Associates commitment; “an extension of your practice”.
