• Dr Debs Robertson

Scope of Presentation
• demonstrating competence and developing your portfolio for revalidation
• planning for revalidation: getting appraisal systems right
• developing competence and assessing competence in line with revalidation requirements
• demonstrating and expanding Nursing Competence
Introduction
• What is a competency?
– Knowledge, skills, motives & personal traits
• What is a competency framework?
– Collection of competencies thought to be central to effective performance
Background
• Demonstrating competence is familiar to HCP’s
• Benner- ‘Novice to Expert’
• The apprenticeship model ‘see one, do one, teach one?’
• Skills development
– Benner, P. (1984). From novice to expert: Excellence and power in clinical nursing practice.
Menlo Park: Addison-Wesley, pp. 13-34.
But WHY?
• To pass assessments?
• As part of performance/development review?
• To get a new/better job?
• To maintain safety and improve care?
NMP- an example
• Introduced in the 1990’s
• Numerous changes /legislation/professions
• Devolved ‘medical’ care
• Nurse led areas
• Increased pharmacy involvement
• AHP prescribing
• Improved practice
• Range of support
• Developing/advancing roles
• Better access to medicines
• Service redesign
Prescribers’ competency framework
• Been around for a while, in different guises
• Nurses competencies?
• Pharmacist/AHP competencies?
• Medics/Dentists?
• Unified professional framework – NPC Single Competency Framework 2012
Domains of Competence
• What good prescribing looks like
• 3 domains – The consultation
– Prescribing effectively
– Prescribing in context
• Each has 3 dimensions of competency (9 in total)
Purpose of the framework
A tool to:
• Facilitate good prescribing practice by all health care professionals who can prescribe.
• To help ensure that patients receive the same high quality of care irrespective of the professional background of the prescriber.
• To allow assessment and development of the prescriber in line with professional requirements?
Uses of the framework
• Curricula development • Support CPD • Training needs assessment • Preparation to prescribe • For further development by professional
organisations / professional groups • By individual prescribers, managers or professional
bodies- revalidation (NB – there is a Word version that can be used to insert comments /
evidence)
Maintaining Competence
• Ongoing Performance development and review through appraisal processes
• Use of ‘a’ framework
– The single competency framework can easily be used as an appraisal tool
– Clinical and theoretical applications
– Allows for informal assessment in practice
– Can be a self assessment process for individuals
Appraisal Processes • Yearly meeting between you and an appraiser
( usually line manager)
• Looks at performance AND development
• Plans for the next year of practice
• Identifies training needs
• Looks at expanding competence
• Sets goals
• Validates your role
• 6 month ‘check in’ if necessary
Revalidation
• Competence must be demonstrated through appraisal processes and in your practice
• The registrant is responsible for collating information and evidence
• The confirmer is responsible for confirming the information and evidence collected is appropriate
• The registrant then submits to NMC
Confirmation for Revalidation- What the NMC say
• Nurses and midwives will need to demonstrate to an appropriate person that they have met some of the revalidation requirements.
• A confirmer is being asked to assess whether the nurse or midwife has demonstrated that they have met the requirements for revalidation, not whether they are fit to practise.
• We recommend that, where possible the confirmer is a nurse or midwife’s line manager.
• They do not need to be an NMC registered nurse or midwife.
• They should act honestly and in good faith
• You will not be held responsible if you inadvertently provide information that proves to be incorrect.

Revalidation- A Portfolio
• A portfolio is a collection of evidence;
– 450 practice hours
– 35 hours CPD
– 5 pieces of practice related feedback
– 5 reflections
– Reflective discussion
– Health & Character Declaration
– Professional indemnity arrangement
– Confirmation

• Use it as part of your professional portfolio
• Reflect on good experiences and good practice as well as episodes that didn’t go so well
• Apply to your area of practice
• Link to the Code
