INTRODUCTIONThe City of Derby (City) owns and operates the City of Derby Municipal Parking Garage (Garage)

located between Olivia Street and Elizabeth Street with the north side of the building facing

Thompson Place.

Due to a recent incident involving a piece of concrete falling from the ceiling of the Garage, the City

ordered the Garage to be closed.

On February 28, 2014, The City asked Prime AE Group, Inc. (PRIME) to evaluate the existing

condition of the Garage to determine whether it could be safely reopened.

OBSERV TIONS

Site Visit - Friday, Feb. 28

2014PRIME visited the Garage on Friday February 28, 2014 to conduct a preliminary inspection of the

buildings condition. Based on this preliminary site visit, PRIME noted the following:

The Garage appeared to have been constructed circa 1975; The surrounding terrain is on slope; The Garage was constructed using a steel frame, cast in place concrete slabs and pre-cast

parapets;

The columns and beams are wide section painted steel members and are joined with bolted

connections;

The steel members are not protected against fire; Cracks with leaking water marks were observed;

Spalling damage was observed on the ceiling in numerous locations;

Exposed and rusting rebar was observed at numerous locations;

Rusting and loss of paint was noted on the structural steel frame members, to varying

degrees;

Concrete appeared to be in fair condition in other areas observed.

Photographs taken during this site visit are provided in Appendix A.

Site Visit - Saturday, March 1st,

2014

PRIME visited the Garage on Saturday March 1, 2014 to mark the areas where there is a chance of

debris falling from the ceiling. These areas were marked on the floor with a large yellow cross in the

parking lots identified as A through H.

These markings are noted as a large red Xon the Figures provided in Appendix B.

RECOMMEND TIONSBased on this preliminary site visit, PRIME recommends that the City take immediate action to

ensure the safety of the parking garage users and to commence a rehabilitation or replacementprogram.

Immediate

The structure appears to be in fair condition and does not appear to be in danger of immediate

collapse as there are no visible signs of distress which would normally precede a failure (i.e. no

excessive deflections were observed in either concrete or structural steel). However, areas where

falling concrete debris can occur should not be used, to avoid vehicular damage and personal injury.

PRIME has identified these areas throughout the garage and recommends installing physical barriers

to prevent their use.

Near Term

The City should perform a thorough condition assessment of the Garage and develop alternatives for

rehabilitation or replacement. This assessment should be completed no later than June 3, 2014

and should evaluate the following options:

Repair: Repair or replace severely damaged concrete and steel throughout the building.

Partial Replacement: Replace the floor slabs and rehabilitate the existing steel frame.

Full Replacement: Demolish and rebuild a new garage.

The alternatives should be developed with opinions of probable costs and should evaluate otherconsiderations such as loss of revenue, life span, etc.

CONCLUSIONBased on the preliminary site visit detailed herein, it appears that the City can safely reopen the

Garage if the recommendations noted in this Report are followed. However, since this Conclusion is

based on a very cursory evaluation, it could change in the event that adverse revelations are

discovered during subsequent building investigations or the review of existing plans and reports.
