Department of Education Region III Division of City of San Fernando San Fernando North District CALULUT INTEGRATED SCHOOL Calulut, City of San Fernando (P) ACCOMPLISHMENT REPORT FOR SCHOOL YEAR 2016 — 2017 I can honestly say that I have always performed my multi-faceted role as a School Principal, to the very best of my abilities. This school year, S. Y. 2016 — 2017, proved to be a very challenging but rewarding school year because we observed our Centennial Year Celebration with the full support of our school stakeholders and all the valuable gifts endowed by our DepEd and local officials and generous benefactors. We enthusiastically journeyed towards our Centennial Mark and carried on successfully with all our planned activities for our Grand Centennial Celebration. We launched our observance last November 10, 2016, through our "100 Gifts to 100 Kids" where we distributed gifts shared by our school alumni to 100 indigent learners. We earnestly concluded our celebration with a Thanksgiving Mass and Awarding of our Most Outstanding Alumni last February 18, 2017. We were overjoyed and inspired by the gift of time, presence and dependable assistance which manifest the unwavering support of our school stakeholders in all our endeavor. We continually engage in our zealous, collaborative efforts to improve learning outcomes and promote the well-rounded development of our learners. Our concerted efforts are paying off well in terms of overall school performance and awards won. Our School Performance Indicators have posted significant improvement, with our Promotion Rate now at 99.32% from 98.35% last year, Completion Rate increased from 98.86% to 99.18% while Repetition Rate dropped from 1.65% to 0.52% and Drop Out Rate from 0.14% to 0.15% this school year. Many of our student contestants won honors and awards in the various district, division and provincial level competitions where they represented our school. Sean Abet M. Henzon capped the 3' Place in the Division Nutri Quiz Bee, with Teacher Coach Mrs. Maribel F. Pare, while Michael Angelo L. Basilio was adjudged 3rd Place in the Poster Making Contest with Teacher Coach Mr. Julius Gonzales. Sean Abet also won 3' Place in the DSPC Science Feature Writing in English, with Teacher Coach Mrs. Czarina L. Gomez. Jhonel A. Musngi emerged as the Third Placer in the DSPC Science Feature Writing in Filipino, also under Mrs. Gomez. Samantha B. Lorenzo garnered the 4 th Place in the DSPC Editorial Writing. JhayArkhy G. Santos was awarded the 5th Place in the DSPC Pagsulat ng Balitang Sports while Roj Matthew G. Vital won the 9th Place in the DSPC Pagkuha ng Larawang Pampahayagan, under the training Mrs. Elena T. Villa as their Teacher Coach. Our Radio Broadcasting Team, headed by Mrs. Ruth D. Briones, garnered the 3rd Place in the DSPC Radio Broadcasting in Filipino, and won special awards for their Infomercial, Technical and Scriptwriting. In sports, Anna Rose P. Nogoy, successfully hurdled the 2 nd Place finish in the 400 meter dash of CSDAM, after winning 1st Place in the district level, and 2" Place in the Long Jump district level competition, under the training of Mr. Joven Q. Flores, as her Teacher Coach. Daniella Mae D. Miranda also capped the 2' Place in the 800 meter dash district level, while Lea Joy M. Sarmiento, was awarded 3rd Place in the district level 200 meter run, with Mr. Joven Q. Flores as their teacher coach. Our boys' volleyball team also garnered the 1st Runner-up finish in the district level competition, with Mr. Gerald T. Pineda as their Teacher Coach. Meanwhile, our pride, the CIS Drum and Lyre Band, was adjudged the 2' runner- up in the 2016 Sinukwan Drum and Lyre Competition held last November, 2016. Our talented band majorette, Jasmine Anne F. Gonzales, was again chosen as the Best Majorette for her back to back win in the same competition last year. This is quite an accomplishment, considering that they competed with eleven other experienced Drum and Lyre Bands in the City of San Fernando and all over Pampanga. This was made possible through the expert training of Mr. Archie P. Mararac, the knowledgeable guidance of Ms. Jean Rafaela D. Quiambao, as their teacher coach, the very dependable support of our Barangay Officials led by Brgy. Captain and ABC President Angel M. Wijangco, together with the indefatigable efforts and loving concern of our CIS Drum & Lyre Parents' Club.

Through the funds provided by DSWD, and the painstaking efforts of some key Mother PTA officials led by President Citadel B. Bundalian, School Health Coordinator Mrs. Aida G. Urbano, together with some supportive, hardworking parents, we were able to provide regular, substantial lunch for more than 155 severely wasted, indigent pupils within 120 days.

In terms of our staff development, we conducted regular classroom observations, faculty meetings, trainings and learning action cells to keep our teachers well-informed and keep them abreast with the latest trends and development in the field of education. We encouraged, motivated and supported our teachers, especially the newly-hired-Senior High School teachers to enable them to attend seminars and trainings to. continually improve their teaching competence in the different learning areas and lev Instruction was further enhanced with the provision of much needed teaching guides a d learning materials along with Science, Mathematics, ICT and other laboratory equipment through the zealous initiative of our dynamic DepEd officials led by SDS Dr. Imelda P. Macaspac fully supported by our local officials headed by Hon. City Mayor Edwin D. Santiago.

In terms of physical facilities development, we were given two new buildings by DepEd and the LGU of City of San Fernando which now house our secondary students in our School Annex. We are also very fortunate to have been chosen as the recipient of the Security Bank Foundation Building and another SEF school building in the elementary level. Along with these new school buildings, we were given sufficient number of armchairs, teachers' tables together with four room air conditioners and CCTV to monitor our two school campuses. With the help of our school alumni, we were able to improve the façade of our school by repainting the dirty walls of old buildings and the school fence. We were also able to install a higher, stronger iron gate in our side fence through the concerned assistance of one of our supportive school alumnus, Mrs. Leila Urquico-Patag.

All these noteworthy development further kindle our zeal, enthusiasm and inspiration in our collaborative efforts to deliver the best quality instruction which will endow 21st century skills to our students. We become more confident and more determined in the pursuit of our common vision, mission, goals and objectives, knowing that you are always behind us in all our efforts. We are deeply grateful for all your unwavering support and assistance, and we will continue to count on you as we strive towards the realization of our fervent hopes and wishes for our learners, our school, our community, our city, our country and our world. Thank you very much.

STATE OF THE SCHOOL ADDRESS FOR SY 2016 - 2017

School Year 2016 — 2017 was a most blessed, meaningful and memorable year

for us here at Calulut Integrated School. We earnestly embarked on our Centennial

Year Celebration with all the gracious blessings and valuable gifts endowed upon us

through our Department of Education Division of City of San Fernando and Local

Government of City of San Fernando, Security Bank Foundation and our supportive

school alumni and stakeholders. With thankful hearts, we enthusiastically journeyed

towards our Centennial Mark and carried on successfully with all our planned activities

for our Grand Centennial Celebration. We launched our observance last November 10,

Coach Mrs. Rowena D. Gonzales.
