Introduction epSOSThe goal To develop a practical
eHealth framework & an

European healthcare
systems - Patient
summaries and
ePrescriptions.
The ChallengeOne major challenge was
to translate all the
selected coding systems
used in Patient Summaries
and ePrescriptions in
order to exchange their
contained information
across country borders.
EU Recommendation5 general recommendations have be given to future similar work:
1. Value set selection criteria’s
2. IP and license
3. Tooling
4. Common Import formats for code systems
5. Value set Metainformation
Criterias to define a Value set1. If the coding system is adopted and translations are available and it is fully in scope with the
associated Value Set:
- The full coding system is adopted as Value Set e.g. ATC for active ingredients
2. If the Coding System is adopted and translations are available, but there are terms non related to
the Value Set:
- Terms related to the Value Set are selected e.g. ISO for countries, EDQM for routes and dose form
3. If the selected coding system is not adopted (transcoding is required) and the translations are not
available:
- A careful analysis is performed with the domain experts (physicians, pharmacists) in order to identify the needed Value Set to provide adequate capability of covering the clinical cases e.g. all the SNOMED-CT related Value Set and ICD-10 for Problems
Was ist ein Value Set?● Ein Value Set entspricht dem möglichen Vokabular, welches für einen
bestimmten Anwendungszweck angemessen ist („meaningful use“)● Dieses kann Vokabular verschiedener Terminologien beinhalten● Es kann aus Klassifikationen oder einfachen Listen bestehen● Internationale Standardwerke ● Templates sind definierte medizinische Informationseinheiten, die das
medizinische Wissen mit der dazugehörigen Datendefinitionen und der dementsprechenden Vokabularanbindung („binding“) über Domainen hinweg verfügbar machen

Vorgehen1. Value Sets und Modelle fachlich definieren
(HL7/IHE-Standards + AWMF)a. Vorarbeiten aus Deutschland (Alpha-ID, MIV etc.) und Europa (epSOS) berücksichtigen
2. Kodierung u.a. mit Snomed CT, daa. deutsche Kataloge nicht umfassend sind
b. ein Zusammenwirken mit HL7 gegeben sein muss (TermInfo-Herausforderung)
3. Einfache techn. Verfügbarkeit (Terminology

Tooling -Terminology Management System● Lesson learned - Creating and storing the code system and the value sets
first started out in epSOS with the use of spreadsheets which resulted in
errors around versioning and missing traceability why epSOS decided to
implement a terminology server and tooling (HealthTerm)
● Recommendations - Terminology server and tooling is needed:
○ Code system Browser
○ Value set selection tools
○ Mapping workflow and Translation tools
○ A central terminology repository
Empfehlungen für Deutschland● Die Entwicklung von Value Sets für existierende und
neue Anwendungen im Deutschen Gesundheitswesen ist überfällig und erfordert polit. Unterstützung
● Auf Basis der Vorarbeiten der Standardisierungs- organisationen müssen diese Arbeiten sofort koordiniert, umgesetzt und implementiert werden
● IT-Werkzeuge sind notwendig
