Instructor cooperation with the student and the Disability Services Office is fundamental to the success of the Institute's commitment to provide equal access to educational opportunities for students with disabilities. After careful review, the Disability Services Office recommends that the above named student receive the following accommodations: Testing Accommodations: Extended time 1.5x Use of alternate testing location Test materials made available in electronic format for the testing center or in hard copy with enlarged font and/or specific font type Enlarged font size required = 16 pt. Classroom Accommodations: Class materials made available in electronic format or in hard copy with enlarged font prior to class Enlarged font size required = 16 pt. OUTLINE OF RESPONSIBILITIES AND PROCEDURES Testing Accommodations STUDENT RESPONSIBILITY: Give a copy of this DSA to your Instructor and discuss your testing accommodation needs with them. If taking the test in the classroom, discuss with the Instructor a plan for how to make use of your testing accommodations. If taking the test in the testing center: o Provide advance notice to the Academic Accommodations Office (AAO), SAU #1150. o Submit a test request form at AAO or online at www.rit.edu/aao . o Give your Instructor a test envelope with the information section on the left side completed about one week before the test date so they have ample time to get the test to the AAO. o Arrive on time to the testing center for your scheduled testing session. The agreement is also known by its acronym, the DSA. The DSA originates with the Disability Services Director & is tailored to the needs of each individual student. The DSA is reviewed by the student and the Disability Services Office (DSO) on a yearly basis. The DSA does not identify the disability; rather it recommends the kinds of accommodations needed. While these are “recommendations,” accommodations that appear here were selected only after a disability was documented and the Disability Services Director and the student discussed what was needed. For some accommodations, the DSA will include detailed descriptions of instructor & student responsibilities.

FACULTY RESPONSIBILITY:

• At the initiation of the student, discuss the logistics of the recommended testing accommodations. • If the student is taking the test in the classroom, discuss a plan for how to make use of their testing

accommodations within the classroom. • If the student is taking the test in the testing center:

o The student will give you a test envelope: provide information on the right side of the envelope, including contact information and return instructions.

o Submit the envelope containing the test to the Academic Accommodations Office (AAO) by dropping it off at SAU #1150 or using the assigned drop-box in your college (listed on the envelope). The AAO requests receipt of exam at least 1 day in advance of testing.

o Contact the AAO as soon as possible if there are any changes to the test or additional information provided to the class during the exam.

RIGHT TO APPEAL Student and/or faculty have the right to appeal the decisions made by the Disability Services Office. The Disability Liaisons are available to assist informal resolutions. If this step does not satisfy both parties, a formal appeal process can be initiated. However, while the academic appeal is in process, the student will be provided accommodations originally recommended by the Director of Disabilities Services unless otherwise arranged by the Provost. Copies of the RIT Disabilities Services Process can be obtained online at www.rit.edu/dso or by contacting the Disability Services Office or a College Disability Liaison. Both students and faculty may appeal the decisions and the kinds of accommodations the DSO recommends.
