I. OVERVIEW
The Accountable Care Directory 2018, compiled by MCOL’s HealthQuest Publishers, offers a unique
resource for Accountable Care stakeholders and others monitoring the industry. Included is an
Organizational Directory of 793 selected Accountable Care Organizations, with all Medicare ACO
participants as of the time of publication, as well as a wide range of organizations involved with
commercial or other programs. Also included are Executive Profiles of Accountable Care executives and
thought leaders, as well as indexes for referencing individuals and organizations included in this
Directory.
The Organizational Directory includes summary information about the 793 selected ACOs, as well as a
listing of 3,901 key persons with leadership or operational involvement with the ACO. Summary
information provided for each ACO includes:

The Organizational Directory is presented alphabetically by state, with the state corresponding to the
primary state in which services are rendered. There are organizations which are administered outside the
service area state, and in these situations the contact information for the organization will correspond with
a different state than the service area state in which the organization is listed. Also, please note listings
for each organization are designed to all be provided on the same page. Thus some pages only contain
one organization, while other pages may contain two.
793 ACOs were selected for the Organizational Directory, and undoubtedly additional commercial or
other ACO programs are under development or potentially operating at this time. While the Organizational
Directory lists all Medicare ACOs as of the time of publication, the commercial ACOs included in the
Organizational Directory were selected on the basis of prominence and documented evidence of ACO
activity, as opposed to inclusion of “ACO-like” arrangements or past announced potential organizations
that do not have current documented activity, which have been included in some third –party reports and
census on ACO activities. Furthermore, some MSSP and Next Generation contracts changed status from
2017 to 2018 (switching from MSSP to Next Generation, etc.)

As a result, the total number of new Medicare ACOs listed in this directory for 2018 is different than the
total number of new MSSP and Next Generation contracts for 2018 announced by CMS, as the CMS
announced number included contract status changes.
Some employees are listed in multiple organizations, based upon their simultaneous assumption of
responsibilities for applicable organizations.
The Accountable Care Directory also includes Executive Profiles of 233 selected executives and thought
leaders nationally identified with Accountable Care. The Profiles are provided in alphabetical order by the
individual’s name. Each Executive Profile includes the following information:

The following Indexes have been provided in the Accountable Care Directory, which incorporate

entities that are not ACOs (and were included because the individual was involved with ACO thought
leadership.)

Profile:
Dr. Jeffrey Aalberg has been serving as a healthcare leader and practicing medicine for nearly 20 years. His deep commitment to promoting patient engagement and experience brings a singular focus to his central leadership role in MaineHealth ACO development and implementation. Jeffrey joined the MaineHealth Accountable Care Organization (formerly the MMC Physician-Hospital Organization) in 2009 and serves the organization as Chief Medical Officer. He is also the Senior Medical Director of Ambulatory Care for the MaineHealth System. In these roles, he oversees programs in care coordination, prevention, chronic disease, advanced primary care, palliative care, behavioral health, elder care, and post-acute care.
Prior to 2009, Jeffrey was the Medical Director of a large urban practice and residency training program serving 16,000 patients and overseeing care delivered by 40 physicians. He also served as Medical Consultant for MaineHealth from 2000 to 2009, supporting programs focused on infectious disease, pharmacy and supply chain management.
Jeffrey holds an MD degree from the University of Massachusetts, an MS degree from University of Oregon and a BS from Rensselaer Polytechnic Institute. He has completed advanced studies in physician leadership, Integrative Medicine and advanced procedure training and has a 2014 Family Medicine Board Certification. He has served on numerous Boards, and most recently joined the Southern Maine Agency on Agency Board of Directors. Jeffrey also serves as Associate Clinical Professor at Tufts School of Medicine.

Profile:
Karin earned her degree in Information Technology from Duquesne University in 2001. Karin has over 14 years of experience in data analytics with a more recent focus on healthcare IT. She was an Interface and Data Analyst at Tahoe Forest Hospital for the past 6 years where she excelled in data mining and interface development for more than 9 Health Information Systems. At the Tahoe Forest, she focused on data migration, business intelligence, HL7 interfaces, report writing and other high level integration development projects at the hospital.
Karin joined Caravan Health as the Director of Information Systems and Services in September, 2014. Her current focus is the integration of the Medicare claims data from CMS, building clinical interfaces, and optimizing the data warehouse to effectively manage patient populations. Karin is pleased to be a part of this organization to help facilitate providers in improving the quality of care, while reducing unnecessary costs with the use of data.
