Note: On this day, 10.13.21, DSA completed child’s HELP re-evaluation. Throughout session DS was present, observed and assisted DSA whenever necessary. DSA explored with MOB her thoughts about what she believes to be most important regarding her child’s development. When asked to describe the child, MOB was asked open-ended questions in order to obtain knowledge about child’s skill level. DSA was able to engage child in tasks in order to gather information about child’s developmental skill level. Throughout the session the DSA modeled providing praise to the child for the tasks completed and attempted. DSA explained that over the next week DSA would develop Intervention Plan and that DS would present it during next session. DSA thanked the family for their participation and engagement.

Note: 10/13/2021 IS-HELPDS and DSA were on a face to face zoom session to complete assessment. DSA was conscious of the family resources in the home while administering the tool with fidelity. Family was very cooperative in answering the questions from the evaluation. DSA completed the parent questionnaire along with structure and the child was engaged for the most part. Family reported that language, potty training and fine motor skills are a concern at this time. The child knows lots of words; however he is unable to use them to communicate effectively. DS and DSA thanked the family for engaging and participating. DSA informed the family that DS will contact her once IP is ready for reviewing.

The Developmental Consultant Liaison offered services to ASQ activities in Communication domain based on the results of the assessments. Following the completion of the activities for three week, the Developmental Consultant Liaison will conduct the ASQ3 assessment again, if concerns show up again a referral to Child Find and First Steps to Success will be made. The Developmental Consultant Liaison explained to the mother the program and how it will help their child and the family. OBSERVATIONAL ANALYSIS MOB was singing along with the client. The child was repeating sayings from a cartoon show. MOB said he repeats those sayings all day. MOB was asking the child to label animals. MOB disclosed to the Developmental Consultant Liaison that they are in the process of moving and she is stressed about the move.

The Developmental Consultant Liaison provided information regarding general health and wellness for the child and the family. A 211 contact list was emailed or texted to the parent and information regarding the EveryParentApp was provided. OR Information about 211 and the EveryParentApp was provided to the parent.

The Developmental Consultant Liaison provided her name and phone number to the mother. Mother was told that she can contact the Developmental Consultant Liaison at any time if she has additional questions or concerns or is feeling any signs of postpartum depression. Mother was also informed that HomeSafe can complete free developmental screenings up until the child is five years old and the contact information for HomeSafe was provided. Further assessments were completed with the mother as part of the Coordinated Intake and Referral process.
