Field Accomplishments for the Month of July 2014
Narrative Report on Field (July 14-28, 2014)
Based on the July 2014 monthly work plan for field activities, the team has come up with the following accomplishments: 1. Social Investigation to area
The 10 Community Empowerment Facilitators (CEF) were formally deployed to 29
barangays of the municipality starting July 18 – 25, 2014 for the social investigation. They conducted key informant interview to barngay officials. The Technical Facilitators (TF) were also visited some hardware/business enterprises within the center of the municipality to gather needed data relative to infrastructure support and facilities.
Based on the needed information from
the Municipal Profile and Barangay Profile pro-forma, the team gathered data from the municipal and barangay offices. Among the important data that were collected at the municipal level were the draft 2012-2020 Municipal Comprehensive Land Use Plan (MCLUP), Municipal Comprehensive Development Plan (MCDP), 2014 Annual Investment Plan (AIP) for 20% Development Fund and Local Poverty Reduction Action Plan (LPRAP) 2014-2017. The team also gathered other health profiles from the Municipal Health Office.
At the barangay level, the CEFs collected few outdated Barangay Development Plans
(BDP) such as Barangay Jubang and Cabawa, and the 2014 Annual Investment Plans (AIP) of all barangays.

The team formally met the Municipal Social Welfare and Development Officer
(MSWDO) Nancybelle D. Trazares and DSWD-13 4Ps Municipal Link Caryl Kate T. Orquina
just after the team’s first dock at Dapa Port on Sunday, July 13, 2014.

The Area Coordinator (AC) Technical Facilitators (TF) with Finance Facilitators (FF)
performed special courtesy visit to Municipal Budget Office, Municipal Engineering Office,
Municipal Accounting Office, Municipal Planning and Development Office, Municipal Treasury
Office, and Municipal Human Resources Management and Development Office on the same
day.
With MSWDO Trazares, the AC attended the SB session on July 14 and presented
very brief discussion on KC-NCDDP for the approval
of the SB resolutions 99 and 100.
c. Courtesy meeting with BLGU officials
The 10 CEFs conducted courtesy call which
started on July 18, 2014 at the mainland barangays due unfavorable weather and security condition. Later, after ensuring peace and order situation from MSWDO and municipal PNP with favorable weather within the municipality, the CEFs were deployed to the island barangays.

The CEFs presented the introduction letter signed by Mayor Ruaya to Punong Barangays during the first visit to their assigned barangays which served as the official presentation of the team especially the assigned CEFs to barangay.
3. Follow-up MLGU submission of Enrollment Forms and requirements
The team follow-up the MLGU completion of its enrollment for KC-NCDDP. On July 22, the team sent communication letter to Mayor’s office and copy furnished to MPDO, MSWDO, MLGOO, Budget and Accounting offices asking the copies of CLUP, CDP, AIP, MDC resolution, and fare matrix and other documents needed for the enrollment requirements.
The following documents are submitted to RPMO on July 25, 2014 thru Ms. Caroline
Catalan:
a) SB Resolution No. 99 series of 2014 dated July 14, 2014. Resolution expressing commitment and support to the KC-NCDDP under the Department of Social Welfare and Development for KC-NCDDP implementation.
b) SB Resolution No. 100 series of 2014 dated July 14, 2014. Resolution authorizing the Local Chief Executive, Hon. Yulie Y. Ruaya, to enter and sign Memorandum of Agreement (MOA) in behalf of the LGU Dapa, Surigao del Norte and between the DSWD for the implementation of KC-NCDDP.
c) Letter of intent from the Office of the Municipal Mayor Yulie Y. Ruaya dated June 10, 2014 d) Executive Order No. 017 series of 2014 dated June 15, 2014. Adopting the strategy on
convergence of DWSD Core Social Protection Program and unifying the Municipal Advisory Committee (MAC) and Municipal Inter-Agency Committee (MIAC) into Municipal Convergence Advisory Committee (MCAC). List of members composing MCAC.
e) Executive Order No. 016 series of 2014 dated June 15, 2014. Providing municipal space with complete facilities at 2nd floor multi-purpose building (newly constructed) for the purpose of KC-NCDDP project under Department of Social Welfare and Development.
f) Executive Order No. 015 series of 2014 dated June 10, 2014. An order creating and constituting the Municipal Coordinating Team (MCT) for the Kapit-Bisig Laban sa Kahirapan-Comprehensive and Integrated Delivery of Social Services-National Community Driven Development Program (Kalahi-CIDSS-NCDDP) under the Department of Social Welfare and Development. List of members composing MCT.
g) Executive Order No. series of 2013 dated July 2, 2013. Designation of the Municipal Gender and Development (GAD) focal person of the Municipality of Dapa, Surigao del Norte. The MSDWDO Nancybelle D. Trazares as the GAD Focal Person.
h) Special Order series of 2014 dated April 8, 2014. Designation of Eusebio I. Ruaya as the Municipal Disaster Risk Reduction and Management Officer (MDRRMO).
i) Municipal LGU Enrollment Form dated June 15, 2014. j) Local Poverty Reduction Action Plan (LPRAP) FY: 2014 – 2017.

Part of the follow up activity on the commitment of the MLGU to provide the counterpart contribution includes the MLGU pledge to provide the conducive office space which is located at the 2nd floor nearby the municipal police station.
4. Team meetings (weekly/ monthly) a. Regular ACT meeting
The team conducted regular ACT meetings
on July 14, 15, 16, 17, 21, 25 and 28. On the July
14, the main agenda was the municipal orientation
facilitated by MSWDO Trazares who provided brief
orientation about the municipality of Dapa and
assisted the division of the staff on areas of
assignment. The fair arrangement of the area
division for the CEFs, TF and FF was done through
drawing of lots/ raffle.

A pre-deployment orientation to barangays was the main agenda of the team meeting
on July 17, 2014. The MSWDO Trazares and HRMO Bernadette S. Porpayas presented some
important reminders about the do’s and don’t’s on
the proper dealings with the communities/people
in the barangays. The team also review of
Barangay Profile pro forma to clear some
important details needed in the profile.

(SI) issues and concerns, targets and agreements
for the submission of the narrative reports.
The team discussed on July 25 meeting the
Social Investigation (SI) narrative report suggested
format and barangay profiling coaching session, information and agreements for the general
staff meeting on July 30.

The team conducted another ACT meeting on July 28 for the monthly planning, preparation for the accomplishment reports, and tasking for the supplies, reminders for the submission of the reports.
b. Special meetings with DAC/TF and MFA/FF
A special meeting were conducted with AC, TF, FF last July 25. The purposed of the special-close door meeting is to ensure proper team management, team’ internal arrangements for the completion of the field activity notice information request (FANIR) and field activity report of accomplishment (FARA) as tool for the team monitoring. The meeting also discussed personal evaluations and learning/encouragement sessions; reviewed of the IPCRs; agreements for the submission of feedback reports.
5. Other Area meetings/ activities a. Attendance to Flag ceremonies/ flag retreat
It has been the custom of the team since July 14 to join during the flag ceremonies
every Monday and flag retreat every Friday of the week.
b. Brief orientation on KC-NCDDP and Potential Problem Analysis (PPA) The team participated the brief KC-NCDDP and PPA workshop on July 15, 2014 with
Engr. Macrino Sumagang, Ms. Caroline Catalan, and Ms. Lorna Bacus. The activity attended by the municipal office heads, municipal officials and Punong Barangays of the 29 barangays. The main output of the activity were the presentation of the KC-NCDDP, the official introduction of the team to barangay officials, and the PPA workshop output.
c. Coordination for the MLGU work and financial plan
A coordination meeting with the MLGU finance team was conducted on July 24, 2014
as a response to the invitation letter dated July 23 for a finance meeting. It was attended/facilitated by the MGLU finance meeting with MSWDO Trazares, Municipal Budget Officer Zosima M. Cuizon, Municipal Accountant Marry Anne Uy Coro and OIC Municipal Treasurer Ilsara B. Andales for the review and finalization of the Work & Financial Plan (WFP). The AC and FF joined the meeting to formally facilitate the WFP completion which was finally approved on the said day.

d. Attended the Municipal Convergence Action Committee (MCAC) meeting
The team joined the MCAC meeting last July 21, 2014. The team presented the brief
orientation about the KC-NCDDP to members of the MCAC chaired by MSWDO Trazares;
introduced the team and assigned barangays to the MCAC; and provided brief updates of
team’s activities.
e. Participated the Internal Municipal Action Team (MAT) meeting
On July 21, the team met for the MAT where they formally introduced with the DSWD’s convergence team composed of 4Ps and SLP staff assigned in Dapa. Among the significant discussion on the meeting were the orientation about the convergence, municipal transition plan (MTP), and OPCR.
6. Other Unplanned Activities Undertaken
a. Coordination with MSWDO and PNP for Red Alert Status With the security alert status in
the municipality, the team closely coordinated with MSWDO and municipal PNP for the security updates.

b. Creation and updating of social media accounts The team created the facebook
page at http://facebook.com/teamdapa posting the activity photos of the team. A google account was also created using the e-mail: [email protected]. The social media are utilize to disseminate relevant information about the updates of the KC-NCDDP particularly in the Municipality of Dapa.
7. Report preparation and submission of (weekly & monthly status report)
The staff prepared their own reports based on the personal journal they maintained.
The narrative reports of the CEFs were submitted to RPMO via e-mail on July 23. Every staff
prepared and submit DTRs/Accomplishment Reports on July 15 which was sent to RPMO via
LBC dated July 18, 2014 while the July 31, 2015 DTRs/Accomplishment Report was
personally submitted on August 1, 2014.

2. KC-NCDDP Program Orientation

Attended the KC-NCDDP program orientation led by Ms. Elsa S. Montemor, the Program Coordinator on Community Driven Development Program and Engr. Macrino Sumagang, the Regional Infrastructure Specialist. The highlights of the orientation were on the brief background of the KC-NCDDP program, local counterpart among others.

11. Deployment to Dapa, Surigao del Norte
July 13 (Sunday)
Team travelled to Dapa, SDN. Majority of the team met at Butuan City bus terminal at one o’clock in the morning and arrived in Surigao City transport terminal at about 4:00 am. At Pantalan 2, the team ride in LPQ boat (lantsa/ ferry boat) which sailed around 6:00 am and arrived Dapa around 9:00 am. Conducted courtesy meeting with MSWDO Nancybelle D. Trazares, Pantawid Municipal Link Carl and PINSP Elmer Dela Victoria Ligalig, the OIC of Municipal Police Station.
12. Courtesy visits to MLGU officials and employees
July 14 (Monday)
Formal introduction of the team members to the MLGU officials during the flag ceremony. Courtesy visit to municipal head offices of budget, accounting, planning & development/ assessor, treasurer, human resources management, engineering. Attended the SB session around 2:00 pm at SB session hall with MSWDO Trazares.
13. Municipal Orientation for team
July 14 (Monday)
MSWDO provided brief orientation about the municipality of Dapa and assisted the division of the staff on areas of assignment Division of the CEF, DAC and MFA/CFF areas on their area / barangay of assignment was agreed thru raffle/ drawing of lots. For CEFs the following were the agreed area of assignments:
1.) Nelmar C. Gomez (CEF) – 2 barangays: Corregidor (farthest isolated island) & Poblacion 13 (mainland)
2.) Harold A. Apat (CEF) – 3 barangays: San Carlos (island), Poblacion 10 (mainland), & Poblacion 11 (mainland)
3.) Michael Jade Vincent G. Delarmente (CEF) – 3 barangays: Dagohoy (island), Monserat (island), & Poblacion 2 (mainland)

Not all Punong Barangays, SB members, and MLGU heads participated on the brief orientation and PPA workshop. Disappointment of former Mayor Peter Ruaya on Kalahi-CIDSS prior to KC-NCDDP enrollment due to prioritization of MLGU to Kalahi program via raffle/drawing of lots.
Encouraged the MSWDO Nancybelle D. Tranzares to lead the workshop activity.
Explained to the former Mayor Peter about the prioritization and selection process of the DSWD that was made before with the new process.

Hire an administrative aid to regular help checking the attachments of the reports and transmit it to RPMO on time.
E-mail the scanned reports to RPMO should be acceptable in case unfavorable weather condition do not permit the staff to travel to submit reports.
Preparation for the Work & Financial Plan
Delay the submission of the work and financial plan (WFP) due to unavailability of the MLGU finance team.
Formally write a letter to the Mayor inviting the MLGU finance team composed of MPDC, Mun. Budget Officer, Mun. Treasurer, Mun. Accountant & MSWDO on July 23.
Properly explained to the MLGU finance team during the finance meeting that it is their full responsibility to make the WFP and the MAF/CFF only facilitate it.
Always provide good rapport and clear communication with the MLGU finance team.
Inspired the MAF/CFF to empower the MLGU finance team to independently make the WFP.
Continue facilitate the MLGU finance team.

Monitor the deployment updates of CEFs to assigned barangays
Most barangays has no Barangay Development Plans (BDPs) for barangay profiling
Dialect barriers by some CEFs who faced with the community
Some barangay officials are computer illiterate so that most of their official documents are brought to poblacion in Dapa for computerization
No digital camera that can be used to collect baseline photos in the community during the social investigation
CEFs gathered relevant info from alternative sources of info like data from BHWs, AIPs among others
Properly explained to the communities about difficulties dialect problems and asked translator with team and/or community who can translate local dialect.
