• Key commitment in guidance to establish a regional field force to:

• Provide support and development to commissioners

• All CCGs and Area Teams

• Raise the bar in relation to individual and collective participation, as opposed to ‘doing the doing’

• Sustainability fundamental to approach

• Funding made available from the national Patient and Public Voice Team at NHS England

Project summary – Patients in Control (Kent & Medway)

There is increasing focus on supporting patients to be more in control of their own health

care; however feedback from patients (and carers) is that they don’t have the appropriate

support to do this.

This programme aims to address this by exploring what’s needed from the patient and carer

perspective and creating mechanisms for co-design of future services, to be truly built around

the needs of the patient. This programme will:

• use a co-design approach to build on current insights and feedback, exploring in detail

what patients and carers perceive as useful support/intervention to help them manage their

own conditions more effectively

• co-design a range of resources/materials based on these findings, to support further co-

design and delivery at CCG/AT level

Project summary – Patients in Control (Kent & Medway)

• actively involve patient and carer champions in staff development and training, to influence

culture and behaviour change in future health and care commissioning and provision

• work with patient and carer champions in providing support to CCGs in adopting a local co-

design approach, using the resources and creating a bespoke approach to engaging with

patients and carers in local self-care/management strategies

• work with regional/local voluntary and community services and networks as key

contributors to this agenda

Project summary – Patients & Public Participation (South West & South)

This programme will be co-designed with PPP leads from across the region and delivered in a

three staged approach;

• Embedding national guidance throughout commissioning organisations by creating an

exciting and engaging message across the region to support PPP leads, utilising our

expertise in communications, product design and technology

• Creating a virtual network to share good work across the region led by a field force, which

will develop strong connections with Commissioners across the South to deliver innovative

approaches. We will collaborate with PPP leads to highlight examples of best practice and

share learning, as well as create effective networking across Area Teams, CCGs, CSUs

and Third Sector organisations

Project summary – Patients & Public Participation (South West & South)

• Incentivise sharing of good practice and acknowledge success at a local and regional level.

Firstly, by creating an innovation fund to pump prime original ideas, and secondly, by

recognising and celebrating creative approaches through awards at a regional level. We

will create a portal of best practice based on national and international research both within

and beyond healthcare, and pioneer innovative products for training and development

The Programme will be supported and challenged by a ‘semi-virtual’ Advisory Panel, including

clinicians and patients/carers, who will ensure we deliver collaboration and innovation.

Project summary – Patients in Control (South West & South)

The programme will use a co-designed baseline assessment to begin to understand attitudes,

current working/existing best practice around personalisation and shared decision making.

The majority of the programme funding will be allocated directly to a ‘Development Fund’. An

Advisory Panel will be established consisting of clinicians, patients and lay members to

assess applications to the Development Fund.

This Fund is designed to incentivise creative approaches to participation and put in place

mechanisms to spread that best practice based on a ‘dragons’ den’ approach, which will

provide funding for innovative concepts that encourage patients to become more active in the

choices that they make about how and when they receive their care. Funding is available to

voluntary and third sector organisations as well as to commissioners and encourages

collaboration

This programme will provide pan-regional support to CCGs and Area Teams to allow them to

look for new and innovative strategies for the personalisation of NHS services.

• Teleconferences – to provide information about this new resource and start a dialogue about tailoring it to meet differing needs of each organisation

• Developing more detail of the projects – in partnership with CCGs and Area Teams
