Preparation of an adoption contract is one of the last steps prior to a dog officiallybecoming part of their new family. The process is simple and all the informationnecessary to prepare the contract can be found on our Wiki(www.daresouthflorida.pbworks.com) If you need a username to log in, pleasesend an email to [email protected] and we can set you upwith one!

It is important to remember that you are drafting a legal document and want to beas thorough as possible on the information that we are providing to the foreverfamily to help them get their new doxie settled into their family as quickly aspossible.

Please be sure to utilize the Contract form dated

02.2010 which can be found on the Wiki in the

Dachshund Rescue South Florida folder under

Forms

Contract Basics

•Date – Enter date you believe the contract will

be executed

•Adopter’s Name – Who will be adopting the dog.

Name to match application

•Dog’s Name – Self Explanatory

•DRSF ID -- Taken from Registration on Wiki

•Breed – Either Doxie or Doxie Mix

•Estimated Age – Taken from Registration on Wiki

•Color Coat – Taken from Registration on Wiki

•Gender - Taken from Registration on Wiki

•Mini/Tweenie/Standard – Sliding scale based on

weight

•Spay Neuter – Answer is always Yes

•Weight - Taken from Registration on Wiki

•Microchip Co - Taken from Registration on Wiki

•Chip ID - Taken from Registration on Wiki

•Weight - Taken from Registration on Wiki. If the

dog has either gained or lost weight, please use

most recent weight available from vet records or

from foster home.

•Spay Neuter – Answer is always Yes

•Microchip Co - Taken from Registration on Wiki

•Chip ID - Taken from Registration on Wiki

Payment

Sliding scale based on age. Amount is

determined by age of dog, and adoption amounts

are listed on the DRSF Site

Heartworm Status

Information on weather the dog received

heartworm treatment can also be found on

the wiki. It is important to note the

specified information if the dog tested

positive while in DRSF’s care. Information

on last date of treatment can be found on

the vetting records on the Wiki.

Other Health Conditions

Similar to the behaviors section, the health

issues section is where we disclose any

medical issues the dog experienced or was

treated for while in our care. This

information can often be taken from the

medical records on the wiki, obtained by the

foster family, or the DRSF Medical

Committee.

Behavior Information

The behavior section is where any particular

behaviors (aggression, training issues, etc)

are noted for the dog and this information

can often be determined by the notes on the

Wiki, but verified by the foster family. It is

especially important to be up front and note

any negative or aggressive behaviors as DRSF

needs to fully disclose any issues with the

dog.

Adopter Execution

•Adopters Signature - Printed Name, Drivers

Licence Number, Address, Email and Work Phone

•Witness Info – Signature, Home Phone & Cell

•DRSF Representative and DRSF Adoption

Coordinator – Same person doing adoption and

are volunteers

DRSF Execution

•DRSF Rep – Signature, Address, Daytime Number,

Email, Relationship to DRSF

•Methods of Payment – Note check number in

writing below check box and for Cash note

“received in Cash $X”

Likes/Dislikes

This information helps tell the foster

family about their new dog and can all

be obtained from the foster family. If a

foster family is adopting their foster

dog, this info does not need to be filled

out and should be noted in handwriting

on the side that a foster family is

adopting.

Adoption Completion – What It’s All About!

The process of completing the adoption is the

columniation of all our hard work and is the best part!!!

The process is simple:

•Contract – need two copies to end up with two fully executed originals.

One copy for the adoptive family, one copy for DRSF

•Medical Records – If you are completing the adoption, print or email copies

of the medical records for the dog from the Wiki so the new family has their

new dogs medical history.

•Read the contract prior to execution so that you are familiar with each

clause, what it does, and to make sure you get all the adopter initials at

each place they are required

•Be sure to get a “happy tales” photo of the new family for distribution on

the DRSF site and DRSF Facebook page. This photo is time sensitive, and

should be sent to [email protected] or via phone to

585.314.5051 the same day the contract is completed

•The DRSF copy of the contract along with payment should be mailed to our

mailing address (contact us tab on the website) or delivered to a board

member

•If possible, please scan a copy of the completed contract and send to

[email protected] as well
